JUSTIFICATION(REASONS)FOR CHANGE ORDER
This final reconciliation change order allows the pavement markings and signage to be added back into the project,whereas these items were originally deleted with Change Order#1
for budgeting purposes at that time. In addition,the block sod quantity was increased to cover the swales and embankments for the project above original quantity assumed to only
sod the the parkways adjacent to the roundabout pavement limits. Additional Type B and Type D pavement was required in order to raise the roundabout to meet minimum pavement
grades around the gutter lines,and transition to adjacent projects taking place along Randol Mill Rd. The sanitary sewer lid was rusted upon final inspection and needed to be replaced
per City inpsector from Water Department. Additional width of sidewalk was added around the south part of roundabout to make City standard width of 5-feet width. Additional 6-inch
drain line was used to outfall into swale at a better angle to avoid buried power line. Portable message signs were used for a longer duration to help with detour and safety during
construction.
It is understood and agreed that the acceptance of this Change Order by the contractor constitutes an accord and satisfaction and represents payment in full(both time and money)for
all costs arising out of,or incidental to,the above Change Order. The Project/Program Manager signing below acknowledges that he/she is the person responsible for the monitoring
and administration of this contract,including ensuring all performance and reporting requirements.
