You may accept the AMENDMENT to Current Amendment 46594 Al set forth in this letter by signing the enclosed copy of this letter and returning it to me for receipt on or before September 14. 2015. You ("City of Fort Worth") agree to enroll three groups of 15 or more employees in Introductory Governmental Accounting Part l; Introductory Governmental Accounting Part ll and Intermediate Governmental Accounting ("the Courses") offered by the Board of Regents of the University System of Georgia by and on behalf of The University of Georgia Center for Continuing Education Conference Center and Hotel, Athens, Georgia ("the University") subject to the terms and conditions of standard enrollment but at the following rates per participant enrolled: Actual agreement based on enrollment commitment. WAVE #1 Course Candidates beginning April 8, 2015 through December 23, 2015 in the three -course series. Course Title Introductory Governmental Accounting Part 1 Introductory Governmental Accounting Part 11 Intermediate Governmental Accounting Course Fee per Person * for candidates beginning April 8, 2015 through December 23, 2015. $419 — Standard Fee X $359 — Group Rate for 15 or more candidates $419 — Standard Fee X $359 — Group Rate for 15 or more candidates $ 619 — Standard Fee X $529— Group Rate for 15 or more candidates WAVE ##2 Course Candidates beginning mid -year 2015 through May 27, 2016 in the three -course series. Course Title FY16 Revised Course Fee per Person * for candidates beginning their studies mid -year 2015 through May 27, 2016. Introductory Governmental Accounting Part I $419 - Standard Fee X $379 — Group Rate for 15 or more candidates Introductory Governmental Accounting Part 11 $419 - Standard Fee X $379— Group Rate for 15 or more candidates Intermediate Governmental Accounting $619 - Standard Fee X $549— Group Rate for 15 or more candidates *Prices are listed per person in US funds. The Course fee does not course extension, course transfers, etc. The City of Fort Worth shall enroll employees total under this Agreement, which represents a maximum expenditure of $96,305 for the 3-course series. WORTH, TX Return to: Special Projects & Curriculum Development, University of Georgia Center for Continuing Education, Athens, Georgia 30602-3603 USA +1-706-542-3537/phone +1-706-583-01 80/fax pam.bracken@georgiacenter.uga.edu City of Fort Worth AMENDMENT (Wave #1 & Wave #2 candidates) August 21, 2015 Page 2 If, during the period of this Agreement, the City of Fort Worth fails to enroll the number of students agreed to, the City of Fort Worth agrees to pay the University the lesser of (a) the actual number of students enrolled during the enrollment period (i.e., Wave #1 and Wave #2) the group rate applicable to that number of students or (b) the rate checked above times the minimum number of student enrollments which would have entitled City of Fort Worth to that group rate, regardless of actual enrollments and without the right to enroll any students subsequent to the end of the enrollment period (i.e., Wave #1 and Wave #2), less amounts previously paid by City of Fort Worth to the University related to enrollments under this Agreement. The University shall bill City of Fort Worth for additional amounts due and City of Fort Worth shall pay the University within 30 days of the billing date. If, during the enrollment period (i.e., Wave #1 and Wave #2) of this Agreement, the City of Fort Worth enrolls more than the required number of students agreed to, no additional discounts or reimbursements shall be due to City of Fort Worth. If, during the enrollment period (i.e., Wave #1 and Wave #2) of this Agreement, the City of Fort Worth enrolls more than the required number of students agreed to, no additional discounts or reimbursements shall be due to City of Fort Worth. This Agreement contains all of the terms agreed upon by the parties with respect to the subject matter of this Agreement, i.e., group discount Course enrollment fees, and supersedes all other agreements, arrangements and communications between the parties concerning such subject matter, whether oral or written, but shall have no effect upon any other term or Cnr;ditinn nf er,rnllmer,t in the rnvPrn•rnvntri! arrn►inting R-Cniircp CpriPc nrngram by any lndivirii ial. The rights under this Agreement may not be assigned nor the obligations delegated by either party without the prior written consent of the other. This Agreement may not be amended or modified except in a writing executed in the same manner as this Agreement. Waiver by either party of any term or condition of this Agreement or any breach shall not constitute a waiver of any other term or condition or breach of this Agreement. The University will maintain student/participant records in compliance with the requirements of the Georgia Open Records Act and the requirements of the Family Education Rights and Privacy Act of 1974, as amended, and any regulations promulgated under the same.
