JUSTIFICATION(REASONS)FOR CHANGE ORDER
IN ACCORDANCE WITH THIS CONSTRUCTION ALLOWANCE REQUEST,THE CONTRACTOR SHALL: Furnish all labor,equipment,and material as per attached
proposed Construction Allowance Request(with attachments)to the Construction Allowance Request.This construction allowance is to include the removal and replacement
of approximately 180 square feet of sidewalk and remove and replace approximately 145 square feet of 9 inch thick asphalt transition to the contract.During construction,the
need to(1)replace a portion of the exisiting damaged sidewalk network at Hemphill Street Bridge over Federal property and(2)replace the damaged asphalt pavement
transition adjacent to the approach slab of Mark IV Parkway Bridge over Little Fossil Creek was identifed.Staff considers these unit prices as reasonable.
It is understood and agreed that the acceptance of this Change Order by the contractor constitutes an accord and satisfaction and represents payment in
full(both time and money)for all costs arising out of,or incidental to,the above Change Order. The Project/Program Manager signing below
acknowledges that he/she is the person responsible for the monitoring and administration of this contract,including ensuring all performance and
reporting requirements.

180 SF @$ 27.50/SF = $ 4,950.00
Our pricing is for the removal and replacement of the deteriorated sidewalk discovered at
the Hemphill bridge approaches.

Dear Sir:
Upon starting the approach slab repairs at the Mark IV Parkway bridge, it was noted the existing
asphalt roadway was severely deteriorated where it transitioned to the approach slab. After
removing the existing approach slab,we found that a portion of the roadway will need to be
replaced to tie into the new approach slabs. Below please find our pricing for this additional
work.
Add Approach Slab Asphalt Transition
145 SF @$ 62.54/SF = $9,068.30
Our pricing includes removing a 5' x 29' section of paving and replacing it with approximately 9"
of Type D asphalt. I have attached our subcontractors quote for reference.

Scope of Work: Change Order
Macias Specialty Contracting will provide equipment and manpower to:
1) Remove approximately 145SF x 9" deep of asphalt and re-apply same qty of type
"D" asphalt as specified by the City of Fort Worth. Macias will remove 5' x 29' of
the existing asphalt and dispose. Macias will put in place Type "D" asphalt up to 9"
deep in approximately 3"lifts and using a roller in between each lift to have the final
grade match the existing asphalt and concrete grade/profile.
Quoted Price:
1) $56.85/SF at 145 SF = $8,243.25
PROPOSAL IS BASED ON THE EXCLUSIONS AND CONDITIONS BELOW.PLEASE READ CAREFULLY.
Additional charges will apply with change of conditions.
• Quoted price isbased off information provided bythecustomer.Anyvariances noted inthefieldwill be billed accordingly.
• Joint sealant at the new asphalt and concrete joints or existing asphalt joints.
• Layout,shoring and bracing,traffic control and barricades is excluded.
• Protection of all surrounding surfaces is excluded. All cleaning is excluded.
• Orientation,badging,drug testing,parking fees,permits,temporary walls and barricades,and retention,are all excluded.
