HomeMy WebLinkAboutContract 45259 (2)ECRET co�aNO.ARY 525 `I MEMORANDUM OF UNDERSTANDING Between ACH Child and Family Services and The City of Fort Worth, by and through its Police Department This Memorandum of Understanding (MOU) between ACH Child and Family Services (ACH) and the City of Fort Worth, by and through its Police Department (FWPD), outlines a collaborative effort to address the homelessness of pregnant and parenting runaway and homeless youth (RHY)/young adults ages 16-21 in Tarrant County, Texas, demonstrating, at a minimum, a close collaboration between FWPD and ACH Child and Family Services. Thus, the parties to this memorandum of understanding agree to do the following: 1. Work together to combat the issue of homelessness of pregnant and parenting RHY/young adults in Tarrant County via ACH's Maternity Group Home, a line of service provided by ACH's The LIFE (Learning Independence from Experience) Project. 2. Mutually establish appropriate protocol which FWPD can follow in referring pregnant and parenting RHY/young adults to The LIFE Project for services or bringing those individuals onsite for services; and 3. Enhance collaborative efforts between FWPD and ACH's The LIFE Project that leverage limited resources to strengthen but not duplicate existing efforts. In furthering these efforts, the parties to this memorandum of understanding agree as follows: Execution Copy — Issued 12.16.2013 1 1 JFFICIAL kEty t. 9&M4ftoUFwpD and ACH CITY SECR Mgy ' Page 1 of 3 • FWPD will endeavor to identify and refer pregnant and parenting RHY/young adults to ACH for comprehensive transitional living services in The LIFE Project, this may include RHY/young adults' voluntary transportation by FWPD to ACH's facilities. • FWPD, through FWPD's Homelessness Liaison and/or other officers, agrees to act as a direct referral source for ACH's The LIFE Project along with various homeless service providers, schools, other appropriate social service agencies, etc. • ACH will endeavor to serve pregnant and parenting RHY/young adults brought by or referred by FWPD for comprehensive transitional living services or referrals for services in The LIFE Project, including but not limited to: immediate, safe housing, case management, counseling, community service activities, life skills training (child development, parenting training, family budgeting, goal -setting, household management and budgeting, interpersonal skill building, family planning, life planning, money management, consumer education, comprehensive sex education, pregnancy prevention services), child care, nutrition and health services, mental and physical health care, educational/vocational advancement (education including GED preparation, high school diploma, post -secondary training or vocational education); job readiness (job readiness plan that includes steps to obtaining and keeping a job, including resume writing, interviewing skills, grooming for work, professional work attire and good work habits) and transportation. • The aforementioned referrals and services will be at no cost to the City of Fort Worth but funded by and through the Administration of Children and Families and ACH. Therefore, signatories agree to work cooperatively on the proposed project to join efforts in meeting the immediate needs of pregnant and parenting runaway and homeless youth and FY14 MOU-FWPD and ACH Execution Copy — Issued 12.16.2013 Page 2 of 3 pregnant and parenting homeless young adults to ensure implementation and success of the program. EFFECTIVE DATE: The terms of this MOU will become effective the date it is signed by the last party. DURATION: The term of this MOU will remain in effect for one year with four one year options to renew, unless terminated according to the provisions in this MOU. MODIFICATION: This MOU may be modified upon the mutual written consent of both parties. TERMINATION: Either party may terminate this MOU upon 60 days written notice to the other party.
