HomeMy WebLinkAboutContract 43534-A2 (2)AMENDMENT No . 2 TO CITY SECRETARY CONTRACT WHEREAS, the City of Fort Worth (CITY) and Criado & Associates, Inc., (ENGINEER) made and entered into City Secretary Contract No. 43534, (the CONTRACT) which was authorized by M&C C-25758 on the 24t' day of August, 2012 in the amount of $184,347.00; and WHEREAS, the CONTRACT was subsequently revised by: Amendment Number 1 in the amount of $47,460.00 which was authorized administratively on October 2, 2013; and WHEREAS, the CONTRACT involves engineering services for the following project: Water and Sanitary Sewer Replacement Contract 2011, WSM-A. WHEREAS, it has become necessary to execute Amendment No. 2 to the CONTRACT 'to include an increased scope of work and revised fee. NOW THEREFORE, CITY and ENGINEER, acting herein by and through their duly authorized representatives, enter into the following agreement, which amends the CONTRACT: 1. Article I of the CONTRACT is amended to include the additional engineering services specified in a proposal dated March 3, 2014, a copy of which is attached hereto and incorporated herein. WORTH, TX Page -2- Transportation •Municipal • Lantl Development March 3, 2014 Roberto Sauceda, PE Project Manager City of Fort Worth Water Department 1000 Throckmorton Street Fort Worth, Texas 76102 Dear Mr. Sauceda: CRIADO &Associates, Inc. is pleased to submit this proposal to provide the following supplemental survey services for Water and Sanitary Sewer Replacement Contract 20011 WSMA-A project: SCOPE OF WORK Research Existing Public Records Obtain all subject and adjoining boundary, subdivision; right-of-way dedications, and current ownership, etc. from Tarrant County Appraisal District Records, Tarrant County Court House, and City of Fort Worth Transportation and Public Works Survey Department. Contact Abstract Company for additional research as needed. Field Data Collection -Horizontal Control and Property Corners Locate and verify horizon#al control and property corners previously field tied by the City of Fort Worth. All GPS data (raw, unadjusted, and adjusted) will be submitted to the Survey Program Manager. All unadjusted conventional field data, along with a sketch, will be submitted to the Survey Program Manager for review and approval. All horizontal data will be represented as surface distances in the CADD files. All horizontal coordinates will be represented as grid coordinates. Boundary Analysis Perfann a boundary analysis by comparing existing public records with the field data. Any discrepancies found when comparing the boundary analysis performed by the City of Fort Worth and the boundary analysis performed by CRIADO will be documented and submitted to both the Survey Program Manager and the Project Manager for the Water and Sanitary Sewer Replacement. It is assumed two (2) parcel acquisitions are required for the Water and Sanitary Sewer Replacement project. Application fees and coordination with the Department of Property Management for parcel acquisition is not inchtded in this proposal. Depending on the boundary analysis, the permanent sewer facility easement requirements may initiate subsequent tasks for additional parcels or easements. A new proposal will be submitted at that time for those additional services. Page 1 of 3 CRIA_DO CIVII GNGINUMING l LAND SURN IYING Prepare Permanent Sewer Facility Easement Field Notes, Exhibits, S et Monuments Exhibits and Field Notes will be prepared for two (2) permanent sewer facility easements. The checklists and templates available on the City of Fort Worth Buzzsaw site will be used in the preparation of the exhibits and Field Notes. All permanent sewer facility easement lines will be monumented with 5/8-inch diameter iron rods and a "CRIADO" plastic cap with a length of 16- inches or monumentation approved by the Survey Program Manager. The exhibits and field notes will be submitted to the Survey Program Manager for review and approval and will be signed and sealed by CRIADO. It is understood at this time that right of entry has been obtained for all owners affected by this proposal. Documentation of right of entry will be provided in conformance with the checklists and templates available on the City's Buzzsaw site. Depending on the final boundary analysis performed by CRIADO, the services required to be performed by CRIADO may change significantly. New monumentation may be required to be established for the entire alignment as well as revisions of all locative information on the Field Notes and Exhibits. All surveying services will be performed according to the "Act &Rules" of the Texas Board of Professional Land Surveying and the "Field Notes Guidelines" of the Department of Public Works and Transportation Surveying Services of the City of Fort Worth, The Act & Rules will supersede the Field Note Guidelines if any discrepancies exist between the two. We propose a lump sum fee of $5,U00.00 to perform, those services. The budget does not include sales tax of 8.25w mandated by House Bill 61, second called session of the 70th Texas State Legislature to perform these services. Proof of tax exempt status shall be provided to CRIADO prior to preparing final invoice in order to preempt sales tax collection. These services cover all necessary requirements if the boundary analysis performed by CRIADO agrees with the boundary analysis previously performed by the City of Fort Worth. If a discrepancy between the two analyses is identified during the boundary analysis (being performed by CRIADO) then all subsequent work will be stopped. We will schedule a meeting with the City of Fort Worth to address those discrepancies. After the meeting, CRIADO will prepare a new proposal covering the additional services that will be required and a commensurate adjustment to the fee will be made. Weather permitting; we can deliver the survey services within 3 weeks from the time we receive your written order to proceed, either by fax or mail. This fee includes 2 delivery charges. Additional prints and delivery charges will be billed separately at actual cos# in addi#ion to the lump sum fee. Due to possible changes in our work load, the delivery date will be adhered to if we receive your written order to proceed by March 14, 2014. After March 14, Zt} 14, it is possible that eve may Head to change the amount of time needed to deliver the survey. This quote will be honored for up to 30 days from March 14, 2014. Unless otherwise stated, fees quoted in this proposal exclude state and federal sales taxes on professional services. All records research will be provided by CRIADQ. Additional research; surveying services required to address comments, etc. outside the original scope of this proposal; or changes made to the survey after completion (if so required), will be considered additional services and can be provided to you on an hourly basis and a separate budget.
