COUNTY OF TARRANT CITY SECRETARY CONTRACT NO. WHEREAS, The City of Fort Worth and Quorum Architects (Architect) made and entered into City Secretary Contract No. 44432 (The Contract) which was authorized by the City Council by M&C C-26239, the 7th day of May, 2013; and WHEREAS, further amendment is necessary to reimburse the Architect as part of the design services agreement. NOW THEREFORE, City and Architect, acting herein by and through their duly authorized representatives, enter into the following agreement which amends the Contract: 1. Article 1.1, SECTION I— SERVICES OF THE ARCHITEC7, the following paragraphs are added to read as follows: Provide additional design services for the design for the placement of temporary office facilities for Code Compliance. The cost of this additional service is $7,400.00 plus $0.00 in reimbursable expenses. The scope of services is described more fully in the Quorum Architects Proposal dated January 20, 2014, subject: Proposal for Additional Services, North Service Center, Phase 1, Quorum Project No. 12113, PO-13-00077713, CS44432 North Service Center which is attached hereto as Attachment "A" and made a part of this Agreement." 2. Paragraph 1 of SECTION V- COMPENSATION TO ARCHITECT, is amended as follows: ' The total compensation for all of the assignments to be performed by Architect as described in CHARACTER AND EXTENT of ARCHITECT'S SERVICES hereof shall be a fixed fee of $1,177,465.00 plus up to $20,000.00 in reimbursable expenses. " 3. All other provisions of this Contract which are not expressly amended herein shall remain in full force and effect. The project includes the design for temporary buildings on a 36,000 sqvare foot site. I have reviewed the project and offer the following proposal for your consideration. I. Land Surveyinq Services - 0.75 Acre Site A. Locate all Topographical Features Survey of topographical features as identified. Will include locating the existing structures, driveways, utilities and other planimetric features to include tree ties. Obtain elevations and set a project bench mark. Sub-total B. Office Work a. Water and Wastewater Plan JDEI will prepare a Water and Wastewater Plan for the construction of new domestic water and new domestic sanitary sewer. New service lines will be shown from their source to within 5' of the proposed building footprint. Connection points at the b�ilding footprint to be provided to JDEI. Sub-total $ 1,200.00 C. Grading Plan Construction plans for proposed on-site grading and paving. On-site grading will be in compliance with current ADA standards. Plans will be created to balance cut and fill volumes and minimize construction costs. � Sub-total $ 1,000.00 D. Coordination for the Relocation of Transformers, Site Electric, and Site Lighting JDEI will coordinate with the Architect, the Mechanical/Electrical Engineer, Lighting specialists and Utility Companies for the relocation or installation of electric and site lighting. Drainage Plan and Analysis Adhering to local storm water management policy, JDEI will develop existing and proposed site drainage plans. This plqn will show the - existing and proposed storm wate'r drainage paths. JDEI assumes that no detention design will be required for site development and all storm water will retain its existing drainage patterns. Sub-total $ 1,000.00 B. Erosion Control Plan JDEI will develop an erosion control plan utilizing current best management practices (BMP's). The erosion control plan will include, but not limited to, a stabilized construction entrance, silt fence, inlet protection, a concrete washout area, check dams, and a materials staging area. Sub-total $ 500.00 Total Additional Services Fee $ 1,500.00 JDEI will perform research, design, coordination, and cqlculations as needed for the preparation of the survey pnd site/grading plan. JDEI anticipates the fees listed above for the various elements of the project.
