WHEREAS, The City of Fort Worth and Quorum Architects (Architect) made and entered into City
Secretary Contract No. 44432 (The Contract) which was authorized by the City Council by M&C C-26239,
the 7th day of May, 2013; and
WHEREAS, further amendment is necessary to reimburse the Architect as part of the design services
agreement.
NOW THEREFORE, City and Architect, acting herein by and through their duly authorized
representatives, enter into the following agreement which amends the Contract:
1.
Article 1.1, SECTION I— SERVICES OF THE ARCHITEC7, the following paragraphs are added to read
as follows:
Provide additional design services for the design for the placement of temporary office facilities for
Code Compliance. The cost of this additional service is $7,400.00 plus $0.00 in reimbursable
expenses. The scope of services is described more fully in the Quorum Architects Proposal dated
January 20, 2014, subject: Proposal for Additional Services, North Service Center, Phase 1,
Quorum Project No. 12113, PO-13-00077713, CS44432 North Service Center which is attached
hereto as Attachment "A" and made a part of this Agreement."
2.
Paragraph 1 of SECTION V- COMPENSATION TO ARCHITECT, is amended as follows:
' The total compensation for all of the assignments to be performed by Architect as described in
CHARACTER AND EXTENT of ARCHITECT'S SERVICES hereof shall be a fixed fee of
$1,177,465.00 plus up to $20,000.00 in reimbursable expenses. "
3.
All other provisions of this Contract which are not expressly amended herein shall remain in full force and
effect.

We are pleased to respond to your request for an Add(tional Service proposal for Architectural Servfces for the
North Service eenter, for the city of Ft Worth. It is our understanding that Code eompifance would like to set
up trailers near the slte for approximately 12-15 officers to use as an of�fce for a year or so, until the service
oenter is constructed. In order to avoid 1nCerfering with the constructlon act(vities, the C(ty would like to set the
traflers on the small triangular site immedlately east of the well slte, NE of the Hillshfre Dr(ve and sh(re Meadow
orive intersection. This proposal is to provide necessary desfgn services to asslst the Cfky In developing that
sfte to a level requlred to accommodate the trailer. Lfsted below is our proposed scope for thls proJecC.
Architectural — Quorum, along wlth consultants, wili provfde Architectural, deslgn to aliow the elty to develop
the parcel of land descrlbed above. We antic(pate geherally that our work will (nclude site survey and
development of utility drawings including water and sanitary. We will also provide enough grading to indfcate
accessible parking and an access raute in compllance with TDLR requirements. Quorum will include
prel(minary and final layouts for approval by the city, fnclud(ng trailerCs) location, pavement Cflex base) extent
and fencing. The tra(ler sizes wlll be provided to Quorum based on the eity's direction. We have fncluded
attendance at a P�c meeting.

ADDITIONAL INFORMATION
1. Quorum Architects, inc. will provlde the cl(ent with archftectural services as required and agreed upon for
satlsfactory and normal completion of this project. The architect shall exercise usual and customary
professional care In his effor�s to comply with those laws, codes, ordfnances, and regulatlons, which are in
effect as of the date of this agreement
HAZARDOUS MATERIALS
The Architect and their consultants shall have no responsibillty for the dfscovery, presence, handiing, removal or
disposal of or exposure of person� to hazardous materials�in any form at the ProJect site. The owner shall obtaln a
hazardous material evaluation, prior to beginnfng any demolition or construction on s(te.

Thank you for your request for this proposal for civil engineering services for the
referenced project. The project includes the design for temporary buildings on a 36,000
sqvare foot site. I have reviewed the project and offer the following proposal for your
consideration.
I.
Land Surveyinq Services - 0.75 Acre Site
A. Locate all Topographical Features
Survey of topographical features as identified. Will include locating the existing
structures, driveways, utilities and other planimetric features to include tree ties.
Obtain elevations and set a project bench mark.

A. Dimension Confrol Plan
Dimension control plan will include dimensions to all proposed improvements
by means of coordinates or dimensions. JDEI will utilize site layouts provided by
the Architect to develop a geometrically sound plan. Dimensions for parking,
and drive lanes will be checked. Plan will also include horizontal and vertical
control set by surveyor for use during construction.

B. Water and Wastewater Plan
JDEI will prepare a Water and Wastewater Plan for the construction of new
domestic water and new domestic sanitary sewer. New service lines will be
shown from their source to within 5' of the proposed building footprint.
Connection points at the b�ilding footprint to be provided to JDEI.

C. Grading Plan
Construction plans for proposed on-site grading and paving. On-site grading
will be in compliance with current ADA standards. Plans will be created to
balance cut and fill volumes and minimize construction costs. �

D. Coordination for the Relocation of Transformers, Site Electric, and Site Lighting
JDEI will coordinate with the Architect, the Mechanical/Electrical Engineer,
Lighting specialists and Utility Companies for the relocation or installation of
electric and site lighting.

Additional Services
(If required after Pre-Development Conference)
A. Drainage Plan and Analysis
Adhering to local storm water management policy, JDEI will develop existing
and proposed site drainage plans. This plqn will show the - existing and
proposed storm wate'r drainage paths. JDEI assumes that no detention design
will be required for site development and all storm water will retain its existing
drainage patterns.

B. Erosion Control Plan
JDEI will develop an erosion control plan utilizing current best management
practices (BMP's). The erosion control plan will include, but not limited to, a
stabilized construction entrance, silt fence, inlet protection, a concrete
washout area, check dams, and a materials staging area.

Total Additional Services Fee $ 1,500.00
JDEI will perform research, design, coordination, and cqlculations as needed for the
preparation of the survey pnd site/grading plan. JDEI anticipates the fees listed above
for the various elements of the project. All costs, associated with the identified scope of
work, will be lump sum for the scope provided and will be billed as a percentage of
completed work as determined by JDEI.

This scope of work assumes the following:
+ All review, submission of taxes, and filing fees will be paid by the owner.
+ Any services not specifically listed as part of this contract will be treated as an
additional service. The owner will be notified before any additional services are
performed.
