IN ACCORDANCE WITH THIS CHANGE ORDER,THE CONTRACTOR SHALL: Water flow into the North Beach Ground Storage tank has historically been controlled
by a 36"butterfly valve with electric actuator,installed in 1979. During the design of the Repainting of the Interior and Exterior of the North Beach 5.0 MG Ground Storage
Reservoir Project,it was determined that a 36"gate valve would need to be installed on the existing 36"tank fill line in order to hydraulically isolate the tank,since the
existing butterfly valve would not seat properly,resulting in substantial leakage. This fall Water Field Operations was able to locate,dig up and raise a valve vault to a
downstream butterfly valve. This butterfly valve was tested for shutout,with satisfactory results. With shutout available from the downstream butterfly valve,the decision
was made to replace the existing 36"electric actuated butterfly valve instead of cutting in a new gate valve. Accordingly,plans were developed for the replacement of the 36"
butterfly valve and electric actuator,as well as modifications to the existing valve vault and mechanical componems(see attached design). The design was provided to the
Contractor who has agreed to replace the existing 36"butterfly valve at the same price as the installation of a new 36"gate valve. An itemized breakdown of the work is
attached as well.
Staff has reviewed this change order and is in agreement with the scope of work and costs associated with the work.
It is understood and agreed that the acceptance of this Change Order by the contractor constitutes an accord and satisfaction and represents payment in full
(both in time and money)for all costs arising out of,or incidental to,the Change Order.
