HomeMy WebLinkAboutContract 54270-A1CSC No. 54270-A1 FIRST AMENDMENT TO CITY SECRETARY CONTRACT NO. 54270 This Amendment ("Amendment") is made and entered into by and between the City of Fort Worth ("City") and The Women's Center of Tarrant County ("Agency") by and through their duly authorized agents. City and Agency may be referred to individually as "Party" and jointly as "Parties." WHEREAS, the City and Agency made and entered into City Secretary Contract No. 54270 ("Contract") on August 10, 2020; WHEREAS, the Contract provides funding to Agency pursuant to a grant received by City from the United States Department of Housing and Urban Development ("HUD") through the Community Development Block Grant (CDBG). WHEREAS, City has received an additional allocation of CDBG grant monies under the Coronavirus Aid, Relief, and Economic Securiry Act ("CARES" Act) referenced by HUD as "CDBG-CV"; WHEREAS, Section 5.2.2.2 of the Contract permits Agency to request an amendment of the budget, subject to written approval by the Director of the City's Neighborhood Services Department; WHEREAS, the Agency requested a budget amendment from City in order to adjust line item amounts, to include Scholarships, in available CDBG-CV funds; WHEREAS, City believes that the requested amendment will help Agency meet the program's goals and further the interests of both Agency and City; and WHEREAS, it is the mutual desire of City and Agency to amend the Contract to complete the Program and meet Contract objectives. NOW, THEREFORE, City and Agency hereby agree to amend the Contract as follows: I. EXHIBIT "A" — PROGRAM SUMMARY attached to the Contract and EXHIBIT "B" — BUDGET attached to the Contract are hereby amended and replaced in its entirety with EXHIBIT "A" and EXHIBIT "B" Revised 4/15/2021, attached hereto. II. This Amendment is effective as of the date of execution of this Amendment. III. WORTH, TX The Women's Center of Tarrant County Financial Stability Project PROGRAM SUMMARY (CDBG-CV) March 13, 2020- September 30, 2021 PERIOD $120,221 AMOUNT Capitalized terms not defined herein shall have meanings assigned them in the Contract. PROGRAM: The program Employment Solutions Department serves individuals with rapid employment and job matching services tailored to job seeker individual needs and situations. As prescribed by the Working Families Success Model, an evidence-based model for poverty reduction, including Rapid Employment Services, Intensive Financial Coaching, and Income Support/Resource Connection. The Center has developed the COVID 19 Financial Health Stability project to quickly help low to moderate income Fort Worth residents establish the resources necessary to weather the immediate crisis and anticipated aftermath. CDBG-CV funds will be used to pay for program Staff salaries and benefits for 2 Career Coaches, an Engagement Coach, and a Resource Coach. Cost to be covered also include contract labor costs for a Financial Coach, and Scholarships for career training. Agency's Program office and services will be available at 1723 Hemphill St., Fort Worth, TX 76110 and virtually. In office hours will be Monday through Friday, 8:30 AM to 5:30 PM. Occasional after-hours and weekend sessions will also be provided. Agency will submit documentation which verifies that at least 51 % of all clients served by the Program, whether Fort Worth residents or not, are Income Eligible Clients with current household incomes at or below 80% of AMI. EXHIBIT "A" PROGRAM SUMMARY CDBG-CV PSA CONTRACT 2020 — EXHIBIT "A" — PROGRAM SUMMARY Page 1 The Women's Center — Amendment 1 PROGRAM GOALS: Minimum Fort Worth Agency Program Performance Outcome: At least 51% of all Program clients must be Income Eligible Clients. Agency must furnish City a Certification of Income Statement for all clients served. For example, if the Program serves 1,000 clients, 510 must be Income Eligible Clients. In this example, Agency would be required to furnish City 1,000 Certification of Income Statements that prove at least 510 of the clients served by the Program are Income Eligible Clients. Minimum Number of Clients to be Served: The Program must serve a minimum of 225 Unduplicated Clients as shown by the monthly reports on Attachment III. When determining the number of clients which constitutes 51%, HUD regulations require that partial numbers must be rounded up.
