Home My WebLink AboutContract 52817-A5CSC No. 52817-A5 FIFTH AMENDMENT TO FORT WORTH CITY SECRETARY CONTRACT NO. 52817 This Fifth Amendment to Fort Worth City Secretary Contract No. 52817 ("Fifth Amendment") is made between the City of Fort Worth ("Fort Worth"), a municipal corporation and Resource Integrators, LLC, a Texas Limited Liability Company. ("Vendor"). WHEREAS, Fort Worth and Vendor entered into an Agreement identified as Fort Worth City Secretary Contract No. 52817 beginning September 19, 2019 (the "Agreement"); and WHEREAS, the Agreement was amended on or about March 3, 2020 to include extend the term and increase the total compensation to an amount not to exceed $271,000.00; and WHEREAS, the Agreement was amended on or about Apri127, 2020 to add an additional scope of work; and WHEREAS, the Agreement was amended on or about November 18, 2020 to extend the terms of the Agreement; and WHEREAS, the Agreement was amended on or about March 13, 2021 to add an additional scope of work; and WHEREAS, it is the collective desire of both Fort Worth and Vendor to amend the Agreement to extend the term of the Agreement to allow for the completion of all activities. NOW THEREFORE, known by all these present, Fort Worth and Vendor, acting herein by the through their duly authorized representatives, agree to the following terms, which amend the Agreement as follows: 1. Section 2. TERM of the Agreement shall be replaced in its entirety to read as follows: "Term. This Agreement shall commence upon September 19, 2019 ("Effective Date") and shall expire no later than October 31, 2021 ("Expiration Date"), unless terminated earlier in accardance with the provision of this Agreement or otherwise extended by the parties." 2. All other provisions of the Agreement that are not expressly amended herein shall remain in full force and effect. This Statement of Wark is to support the testing of these interfaces for Fire and Police departments and update them to suit the usage requirements. This activity will involve following tasks to be performed by Deloitte resources, - Defect resolution support for the interfaces during System Integration, User Acceptance and Parallel Testing acrivities. - Updates to functional and technical specification documents to reflect Police department requirements. We have made the following assumptions: • Tesring activiries for Fire and Police department interfaces are scheduled in May — October 2021 • Subject Matter Experts (SMEs) for Schedule, Time administration and Payroll processing for Fire and Police departments are available to clarify interface related questions • Pricing is based on an estimate of work to be performed.
