RE: REQUEST FOR CONTRACT AMENDMENT
City of Fort Worth Project 102070
High Woods Storm Drain Reconstruction
Dear Mr. Wellbaum:
LAN previously completed and submitted signed and sealed design drawings for the construction
of a retaining wall parallel to the Woodhaven County Club Estates property line adjacent to the
shared driveway of 913 High Woods Trail and 917 High Woods Trail. On April 16, 2021, LAN
was informed that an existing wall to remain in place was removed during construction. The
removal of the existing wall caused a field change to the designed retaining wall and pier support
configuration. Subsequently, LAN redesigned the connection to the existing wall and the pier
supports as provided in drawings submitted on April 22, 2021 . The revisions were completed to
not delay construction progress on the material field change. The task/hour breakdown for specific
activities within this requested Amendment 1 is attached, but, in summary, the following items are
included in this request of $6,952:
• Task 6.1: Construction Support — The effort to prepare and design the reconfiguration
of the retaining wall and pier support design and future unexpected field changes during
construction.
The following presents the project fee summary to date, including this request:
Original Contract $ 40,918
Amendment No. 1 $ 6,952
Revised Contract Amount $ 47,870
As noted on the attached breakdown, the additional effort associated with tasks totals $6,952.
Please review this as a formal request to consider and process Amendment 1 to City Project
Number 102070 in the amount of $6,952. Should you require further documentation for this
request, please contact me to support your consideration. Should you have any questions or
require additional information, please contact myself at 817-338-7512.
Respectful ly,
Lockwood, Andrews & Newnam, Inc.
