HomeMy WebLinkAboutContract 54722-AD4CSC No. 54722-AD4 Addendum No. 4 Statement of Work This Addendum No. 4- Statement of Work (SOW) is made as of this 16th day of June, 2021, under the terms and conditions established in the MASTER PROFESSIONAL SERVICES AGREEMENT between the City of Fort Worth ("Cit�') and P I E Management, LLC ("Vendor"), the same being Fort Worth City Secretary Contract (CSC) No. 54722 (the "Master Agreement" or "MSA"). This Statement of Work is made for the following purposes, consistent with the services defined in the Master Agreement: Providing temporary staffing services to the City of Fort Worth for the positions listed below. In particular, Vendor shall perform those functions identified below in the Scope of Services, as such services are more fully detailed in the MSA. Section A. Scope of Services & Schedule The Neighborhood Services Department (NSD) is requesting temporary staff to serve as Custodians, Community Center Aides, Eligibility Specialist and Office Assistants. NSD provides an array of services that span from recreation services such as older adult, after school, summer and fitness programs to social services that provide support to persons experiencing temporary and chronic financial hardship. NSD operates using General Fund and receives numerous State and Federal grants used to provide financial assistance to persons that are low-income, experienced recent crisis. Additionally NSD has been granted funding for persons that eXperienced a loss of income due to COVID and/or seeking job training/certification to earn a livable wage. All positions are utilized year round and required to meet the demand of customers and clients utilizing the community. The following positions are required to fulfill the daily operations of both social service and recreation programming: Eligibility Specialist positions are utilized to fulfill the roles and provide services for the Community Action Partners Program (CAP). The role of this position is perform intake duties and determine applicant eligibility in assigned human services or employment and training program; and to perform a variety of administrative tasks in support of assigned area of responsibility. Eligibility Specialist are required to perform general clerical and data entry duties, answer telephones and respond to requests for information. Office Assistant (OA) positions are utilized in the Community Action Partners Program (CAP). This position is utilized to support the Human Service Specialist complete daily tasks. With the increase in request for financial assistance and additional funding received, this position is required to process client/citizens request. The OA is responsible for answering and returning client calls, assisting with processing payments, requesting information from clients, landlords and utility providers to enable staff to complete client files. Custodian position is critical to facility operations, particularly with the expected increase in customer volume and requirement to uphold cleaning standards to prevent the spread of COVID. With expansion of services and allowing the general public to access the facility, a clean facility is dire need. Community Center Aide (CCA) is a core position at community centers. Southside Community Center has a robust program that serves older adults. However, this position has been repurposed due to shortages to work with older adults and youth. This position has been approved to hire but filling it temporarily will allow staff to support the recent reopening of the facility and maintain daily service. oFFicia� RecoR� CITY SECRETARY FT. Weekly/10 Weeks (Total Hrs. 520) Hourly Billing Rate (Must be in Exhibit B of the MSA): $16.56 per hour Total anticipated cost: $6,624.00 Assignment #4 Position Requested (Must be in Exhibit B of the MSA): Community Center Aide # of above Positions Requested: 3 Location of Assignment: Various locations Projected Start and End Dates for Assignment: June 28 — December 31, 2021 Anticipated Total # of Hours: 40 Hrs. Weekly/27 Weeks (Total Hrs. 3240) Hourly Billing Rate (Must be in Exhibit B of the MSA): $15.94 per hour Total anticipated cost: $51,645.60 Section B. Compensation, Reimbursable Expenses, and Billing Terms: Processing and payment of fees, including the structure of payments and invoicing thereof, shall be in accordance with the Master Professional Services Agreement. The total fee that shall be paid by City to Vendor for these services shall be in accordance with the Master Service Agreement and, specifically, E�ibit "B"- Price Schedule. In submitting invoices, Vendor shall reference the appropriate Statement of Work. Further, the Vendor shall separately list the total amount for fees in which invoices were previously submitted during the then-current annual contract period (either initial term or renewal period) and the total amount of fees for the entire period, including those amounts being requested under the then-current invoice. Vendor will submit invoices for fees to Vella Edwards within the Neighborhood Services Department (vella.edwards(a�fortworthtexas.�) and also submit a copy to the Talent Acquisition Manager in the Human Resources Department, 200 Texas Street, Fort Worth, TX 76102.
