I have reviewed the PCM 011A for furnishing and installing braces and supports for the waste washwater pumps
discharge piping. The need for this was realized during the construction. This PCM includes all associated material and
labor costs and will not require contract time extension.
I am in agreement with this PCM 011A and it will be included in the next change order (CO #6) paperwork.

Transmitted herewith please find a co�y of Pepper-Lawson's NoveiYiber 12, 2014 price proposal for the
referenced work. The cost for this work is $15,215.00. No additional Contract Time has been requested.
Installation of the new Waste Washwater Pumps identified an issue with the clischarge pipe suppori
system and the plant operations sampling facilities. The existing discharge piping is supported on an oltl
fabricated half I beam, half concrete pier assembly that does nok provide piping support independent of
soil movement. The proposed pipe supports utilize the pump station foundation as the source of
support for the new piping eliminating potentiaf differential movement. A new sarnple collection pipe
and hose system was designed to allow samples to be taken if any of the three pumps are operating.
We have reviewed this price proposal and are of the opinion that it is a fair and equitable price. We
recommend the City accept this cost and issue a Change Order in the amount of $15,215.00. Please
advise if the Water Department desires this PCM be incorporated in a Change Order.

washwater pump discharge. Proposal also includes adding one foot of gravel to raise thc gradc around the pump station to
473.57. Proposal also includes installing a painted 1" black stcel pipe hcader to tie the sample lincs logethcr, rcmovable unions
at the pump, and 3/4" hose fittings from sample headcr with a 25' heavy duty hosc.

filters. Basically, the need for this additional work was realized during the construction and is detailed in the CMR

Transmitted herewith please find a copy of Pepper-Lawson's October 16, 2014 price proposal for the
referenced work. The cost for this work is $10,000.00. Six calendar days Contract Time has been
requested.
There is a discrepancy in the Contract Drawings between the mechanical sheets which show reusing old
instrumentation tubing in the effluent gallery to source the Loss of Head transmitters while the
Instrumentation details and specifications require new tubing. The existing tubing is not the size
specified for the new work and most of the tubing had to be replaced to accommodate the new location
of the meters. A negotiated settlement of $10,000 for this claim was reached.
We have reviewed this price proposal and are of the opinion that it is a fair and equitable price and
contract time adjustment. We recommend the City accept this price proposal and issue a Change Order
in the amount of $10,000.00 and six days time extension. Please advise if the Water Department desires
this CMR be incorporated in a Change Order.

Per the Engineer's request in the instrumentation mounting walk through on June 18, 2013, Pepper-Lawson Waterworks offers
a proposal to purchase and install new U2" stainless stcel tubing 1'or thc loss of he2d trinsmittcr. "The change is necessary due to
the notes on the contract dra�vings. F-l4 speciYies °Bxisting 1/2" Tubing 7'o Remain. Sec. t;-Sheets I'or Location Of New Loss
of Head Transmitter. (Typ. 20)". 5/E-15 shows the demolition in the gallery and Notc 3 details thc iterns that are to be
demolished (existing head loss transmitter, associated stand, conduit and wire). Il-I shows the connection to the sediment trap;
but doesn't specify new tubing. Also, existing SS tubing appears to be l/4" instead of l/2" as noted. Thc currcnt schedule
has been attached to sho�v the "As-Bid° time allotted for the electrician. While we recognize. there is an extra effbrt required for th
work in dispute, the Contractor has proceeded on a"good faith" basis in an efforl to not delay the project. A secondary
schedule is also attached showing the additional time required for the filters that are in scrvice. This time has been projected out
through the remaining work. Pepper-Lawson is reqi�esting additional time for the difference bctwecn the two schedules.
It is noted that the twenty-eight days of additional work requested by Texsun translates to six days on the critical path.
This change tvi!! result in a add to the Contract of $10,000 and an increase in contrnc/
duration of 6 calendar• days frovi the date of approval to c%live�• the mnterials to the site and
pef fornt t{ae tivork.
Upon receipt of approval from the City of Fort Worth and Freese and Nichols as to the acceptability of our proposal,

Transmitted herewith please find a copy of Pepper-Lawson's February 25, 2015 price proposal for the
referenced work. The cost for this work is $3,473.00. Twenty calendar days Contract Time has been
requested.
Initial operation of the scum screening system by the plant identified a change on operation needed
from the designed operation. Transfer of screened scum is best performed after the plant has received a
load of high strength waste at the Co-Digestion storage tank to provide a more uniform blend of high
strength feed stock to the digesters. The transfer pump will be operated in a manual transfer mode
rather than automatically. This change identified another problem with the specified programming; the
low level float was being activated before the run timer timed out resulting in stop/start/stop operation
of the pumps and generation of nuisance alarms to the operators. Attached is Pepper-Lawson's CMR for
costs associated with the programming changes and our email detailing the modifications needed.
We have reviewed this price proposal and are of the opinion that it is a fair and equitable price and
contract time adjustment. Texsun Electric (Pepper-Lawson's electrical subcontractor) has passed Prime
Control's proposal up to Pepper-Lawson without any markup. We recommend the City accept this price
proposal and issue a Change Order in the amount of $3,473.00 and twenty days time extension. Please
advise if the Water Department desires this CMR be incorporated in a Change Order.

Below are items from the SOP meeting.
Scum Trough
The attached red�lined cut-sht shows what should have been deliveced. What the Contracto�• provided was
slightly different than otu� dwgs in that the depth of trough is the same for the front and back. Our dwgs had the
front of the trough 1" lower than the back.
The weir has an adjustable height of 3". If all the way up, it is 1-inch lower than the top of trough. Given the
trough is installed 1" lower than the overflow weir, then if the weir is all the way up it should be 2�inch below
the water sucface (assuming no head over the weir).
1. I recommend to raise the weii° all the way up and see how it performs.

2. We also need the Contractor to cut the notch on the front plate of the trough to make it level all the way
across. Repair per manufacturer reco►nmendations.

Can you touch base with Robert on the elevation shots that were taken so we can incorporate them into the
SOP?
Main Control Panel
The following are control logic changes in the Main Control Panel to accommodate the low level stop. Normal
operations will have the Scum Feed Pumps ran in Remote Auto and the Scum Transfer Pump ran in Remote
Manual.
• Scum Pumn Controls (PSC-01 & PSC-02) - the control panel needs to include the following logic:
o The "Time Duration the Pump is to Run" will stop timing when either the following occurs: It
has timed out (as is currently programmed) OR when a PUMP LOCKOUT signal has been sent
to the Scum Pump starter.
o When either a planned stop (based on timer) or a Pump Lockout signal is triggered then the

■ It does not need to display it as an alarm on the screen that will require someone to

• Hot Water Solenoid Valve (SOV-03) — change indicated in red.
o When a signal has been sent to Stop the Scum pumps whether based on timer (normal stop) or
on level/pump lockout or when the pump has stopped during a call to run from the Main
Panel), a signal shall be sent to open the solenoid valve.
• Scum Transfer Pump & West Tank Low Level (LSL-02)
o The following applies to when in REMOTE Manual mode:
■ The operator may be able to start and stop the pump manually from the Main Control
Panel.
■ If the low level switch (LSL-02) has been triggered the pump shall send a pump lockout
signal to the pump starter.
■ Once the alarm/pump lockout has cleared, the PLC shall be programmed to require the
operator to manually start the pump before a Run signal is sent back to the motor starter
(this is to not allow the pump to restart once stopped due to the level triggering a pump
lockout signal).

CMR-010 Additional Asphalt Depth
Dear Ms. Goderya:
Transmitted herewith please find a copy of Pepper-Lawson's March 20, 2015 price proposal for the
referenced work. The cost for this work is $20,016.00, Seven calendar days Contract Time has been
requested,
The parking lot west of the Backwash Pump Station asphalt was to be replaced per the base bid.
Reconstruction required the existing subgrade be recompacted and new asphalt placed. The work was
to be paid for by unit price per square yard. Review of the existing road conditions at the end of
construction found a defective asphalt area in the RS8 parking lot east of the filters and found a portion
of the Backwash Pump Station to be in good condition that did not need replacement. The limits of the
asphalt repair area were modified, see attached sketches, and the work performed. Demolition of the
existing asphalt found the average thickness to be almost 10", 4" more than the repair detail inferred.
Also, the parking lot west of the Backwash Pump Station needed the grade raised an average of 2" to
provide proper drainage. The CMR provides payment for the additional excavation and haul off, the
additional flex base installation, and the additional equipment required to install the flex base.
We have revieweci this price proposal and are of the opinion that it is a fair and equitable price and
contract time adjustment. We recommend the City accept this price proposal and issue a Change Order
in the amount of $20,016.00 and seven days time extension. Please advise if the Water Department
desires this CMR be incorporated in a Change Order.

I have already given my approval on this PCM 010. I believe, the amount seems fair and reasonable too.

PCM010 — Extra Work Performed Under Time and Materials Method of Payment
Dear Ms. Goderya:
Transmitted herewith please find a copy of Pepper-Lawson's July 21, 2015 price proposal for the extra
work performed under time and materials. The cost for this work is $16,928.00. No additional Contract
Time has been requested.
During installation and startup of the filters several items of existing work were found to need
corrections to allow the filter complex to operate properly. Startup identified an issue with air locking in
the waste washwater flume which was corrected by addition of a 12" air vent. Several small design
changes were performed under this force account.
We have modified the proposal submitted to include appropriate authorization documentation. Several
small items due not have backup but the costs are appropriate to the work performed. We have
confirmed with Mr. Aaron James that the costs listed include Pepper-Lawson's appropriate overhead
and profit markups.
We have reviewed this price proposal and are of the opinion that it is a fair and equitable price. We
recommend the City accept this cost and issue a Change Order in the amount of $16,928.00. Please
advise if the Water Department desires this PCM be incorporated in a Change Order.
