This change order includes additional works required on this project to meet existing or changed site conditions: Atlditional works are
described as follows:
1. Replace existing deteriorated brick manhole with concrete on Belle Place for storm drain
2. Extend existing 27"storm drain on Dorothy Lane with new headwall to protect the newly constructed street
3. Construct concrete retaining walls with variable heights on portions of Dexter and Washburn Avenue. Delete block retaining wall as
4. Adjustments to increased quantities for waterline including additional 350 linear feet of 8" water main on Sutter Avenue to Lafayette
Avenue for closure , two 8" gate valves, five water meters and nine water service tapes on Dorothy Lane as requested by the Water
Department.
5. Adjustments to increase sanitary sewer items 10 linear feet of 12" SDR 36 , service connections, interior manhole paint and one 30"
manhole cone.
6. Additional 65 calendar days to the contract for extra labor and materials and the delays that were involved with the retaining walls and
heatl wall design modifications required to adjust to existing site conditions not shown on the plans.
