Home My WebLink AboutContract 49493-A4 CSC No.49493-A4 FOURTH AMENDMENT TO FORT WORTH CITY SECRETARY CONTRACT NO. 49493 This Fourth Amendment to Fort Worth City Secretary Contract No. 49493 ("Fourth Amendment") is made between the City of Fort Worth ("Fort Worth"), a municipal corporation and AskReply, Inc. dba B2GNow, an Arizona corporation. ("Vendor"). WHEREAS, Fort Worth and Vendor entered into an Agreement identified as Fort Worth City Secretary Contract No. 49493 beginning August 7, 2017 (the"Agreement"); and WHEREAS, on or about January 26, 2018, the Agreement was amended via CSC 49493- A1 to revise Exhibit A, Statement of Work; and WHEREAS, on or about August 5, 2020,the Agreement was amended via CSC 49493-A2 to include additional renewal period options; and WHERAS, on or about May 19, 2021, the Agreement was amended via CSC 49493-A3 to include an additional scope of work; and WHEREAS, it is the collective desire of both Fort Worth and Vendor to amend the Agreement to include an additional scope of work. NOW THEREFORE, known by all these present,Fort Worth and Vendor, acting herein by the through their duly authorized representatives, agree to the following terms, which amend the Agreement as follows: 1. The Agreement is hereby amended by adding an additional scope of work at a one- time cost of$1,700.00. The additional scope of work is described in Exhibit B-3 , attached hereto and incorporated into the Agreement. 2. All other terms,provisions, conditions, covenants and recitals of the Agreement not expressly amended herein shall remain in full force and effect. We need to get the custom Certified Firms Listing report back to working as intended.Since recently adding the national certifying agencies to our available directory,the custom Certified Firms Listing report is not pulling any results.For this custom report we need the NAICS commodity codes with description,and the information tied to the certification such as ethnicity,gender,county and city location.The DBE,MBE,SBE,WBE certification information should come from the Dallas-Fort Worth Minority Supplier Development Council(DFW MSDC), North Central Texas Regional Certifying Agency(NCTRCA),Women's Business Council- Southwest(WBC-SW),and Texas Department of Transportation(TXDOT).There are no changes in the output of the report,everything should be as previously formatted. Add new option to select the pre-populated zip codes.This will be a seperate new option and different from the current parameters of Location and City.The parameters for Location and City do not have to selected for the pre-set zip code selection to be used.The end report result will still show up as the example in the"MWBE-BE"tab.
