JUSTIFICATION(REASONS)FOR CHANGE ORDER
The change in Bid quantity and addition of Bid items resulted from the Engineer of Record(EOR)'s response to the contractor's request for information(RFI)numbers 6,7a,
and 8.
RFI 6 is about descoping the portion of the southbound paving from the contractor's contract.Per the contractor's RFI,the existing pavement is in good condition and
recommended not to replace.I coordinated the contractor's request with FOR and TPW Street Service.TPW Street Service field verify the condition of the existing
pavement.Per email from Juan Cadena(TPW Streets and Stormwater Operations),he agrees with the contractor's recommendation to leave the portion of the pavement
as-is.FOR also agree with the contractor's recommendation.
RFI 7a is about the Temporary Traffic Singal Phasing.The contractor installed the temporary Traffic Signal Phasing per Issue for Construction(IFC)document but TPW
Transportation Management made the contractor to revised the plan,which required accommodation of the LED's and signal heads.
RFI 8 is about using Off Duty Police Officer to control the traffic flow at the intersection.The project requires relocating the Oncor transformer from the existing pole to the
new pole.The existing traffic signal cabinet doesn't have a BBU to run the signal while power is disconnected.When relocating the Oncor transformer,the existing traffic
signal equipment will be out of the power source.The contractor recommended using Off Duty Police Officers to control traffic flow at this intersection while Oncor performs
work because the intersection has two(2)major arterial roadways.
It is understood and agreed that the acceptance of this Change Order by the contractor constitutes an accord and satisfaction and represents payment in
full(both time and money)for all costs arising out of,or incidental to,the above Change Order. The Project/Program Manager signing below
acknowledges that he/she is the person responsible for the monitoring and administration of this contract,including ensuring all performance and
reporting requirements.

Sheet 20 & 25 of the plans appear to show the existing curb to remain in place between station 223+48 to 222+00.
If the curb is to remain, the concrete paving in this section also appears to be in good condition and could also
remain in place saving the City a considerable amount of money.
1. Please clarify if we are to salvage the existing curb and pavement in this area.
2. If the curb remains in place, confirm the dimensions of the pavement to be left in place or how far off the
curb we need to make the sawcut for removals.
3. If the curb remains, should we also salvage the existing concrete pavement?Approximately $50,000
savings.
Contractors Recommendation: (Please do not leave blank. Provide the contractors recommended solution based on his
knowledge of the project and existing conditions to assist with accelerating the response)
1. Leave both the curb and concrete pavement in place, replace the outer limits of southbound Beach Street
outside of stations 223+48 to 222+00. Approximate Savings $50,000
2. Remove the full width of paving and curb to pour a new pavement section that is monolithic through the back
of curb. Approximate quantity/cost increase $4,300

1. The project requires relocating of the Oncor transformer from the existing pole to the new pole. Relocating
the Oncor transformer is not in our scope of work. The existing traffic signal cabinet does not have a BBU to
run the signal while power is disconnected. When relocating the Oncor transformer, the existing traffic signal
equipment will be out of the power source. How do you want to control the traffic flow while Oncor is
relocating the transformer?

knowledge of the project and existing conditions to assist with accelerating the response)
The intersection has two (2) major arterial roadways. The use of 4 way stop signs for a prolonged period is not
advisable. We recommend using Off Duty Police Officer to control traffic flow at this intersection while Oncor
performs work.

Potential Impact to the City of Fort Worth (Schedule, Scope, Cost):
Final charge will be based on actual amount of time officers are onsite, with a 4-hour minimum charge. Oncor
