(ENGINEER) made and entered into City Secretary Contract No. 39765, (the CONTRACT) which was authorized on the 12 day of January, 2010 in the amount of $ 175, 075. 00; and WHEREAS, the CONTRACT involves engineering services for the following project. THOMPSON ROAD, City Project#:01368, Unit/Section: North Riverside Dri-ve to Flowertree Drive, FIAIC: C223 541200: and it has become necessary to execute Amendment No. 1 to the CONTRACT to include an increased scope of work and revised fee. NOW MRE, CITY and ENGINEER, acting herein by and through their duly authorized representatives, enter into the following agreement, which amends the CONTRACT: 1. Article I of the CONTRACT is amended to include the additional engineering services specified in a proposal dated August 31, 2012, a copy of which is attached hereto and incorporated herein. The cost to City for the additional design services to be performed by Engineer totals $2 6, 810. 00 (See Attached Funding Breakdown Sheet, Page -3-) 2 . Article II of the CONTRACT is amended to provide for an increase in the fee to be paid to Engineer for all work and services performed under the Contract, as amended, so that the total fee paid by the City for all work and services shall be an amount of $201, 885. 00. 3. (CRIADO) is pleased to present this Amendment No.1 to the Engineering Services Contract for City Project Number 01368 (City Secretary Contract No. 39765) "Thompson Road, from North Riverside Drive to Flowertree Drive. This amendment includes four separate parts, as described below: PART 1: CHANGES TO FINAL SUBMITTAL, NEW SPECIFICATIONS SYSTEM AND NEW BID ITEMS CRIADO revised Final Submittal plans and specifications to meet the conditions established in the new Construction Standards adopted by the City of Fort Worth in the last quarter of 2011. A new Project Manual was prepared for the City's review and the plan set bid items were modified to meet the City's new standards. This task included the time for Project Manager, Project Engineer and CADD Operator for performing the preparation of the new bid documents. CRIADO spent$10,140.00 to complete this task. PART 2: CONSTRUCTION STAKING, NORTH RIGHT-OF-WAY A.N.A. Consultants, CRIADO's Surveying Subconsultant for this project, performed the construction staking of the north right-of-way line for the full length of this project in September 2011 and in May 2012. This task included the time for Survey Tech and Survey Crew for two construction stakings. Our subconcultant, A.N.A. Consultants spent$2,400.00 to complete this task. PART 3: EVALUATION OF PLAT IINTERPRETATION- PARCEL P11 CRIADO and A.N.A. Consultants performed an evaluation of Final Plat(Lot 1, Block A Residences at Old Denton Road recorded 411 0108) to clarify the Right-of-Way Acquisition for Parcel P11. This task included CRIADO Principal, RPLS, Project Manager time for review of documentation, field verification, conference calls, and correspondence and A.N.A. Consultants time required to resolve this matter. CRIADO spent $5,237.50 and A.N.A.spent for$2,500.00,for a combined total spent of$7,737.50 to complete this task. PART 4: PLAN REVISIONS OF RETAINING`HALL,WATERLINE LOWERING,GATE RELOCATIONS CRIADO prepared 5 plan sheet revisions to the construction set of Thompson Road at the request of the City. This revision was made in order to obtain a 2% gradient across the south parkway of Thompson Road, near Fox Chase Drive. New concrete retaining wall (275 linear feet), 12-inch waterline lowering (355 linear feet), and other design revisions (three gate relocations along the north parkway) were included in this revision. We are asking the City of Fort Worth to approve this Amendment NO in the amount of$28,810.00, incurred to perform Items f, 2, 3 and 4 above. In accordance with the City of Fort Worth Mayor and Council Communication, Reference Number C-24017 of January 12, 2010, CRIADO's MWBE participation ibr this project is 28 percent. In order to achieve the MWBE participation commitment, CRIADO proposes to increase A.N.A. Consultants original contract by having there assist with the task of preparing 'KM-Builf plans for a total effort of $2,000 and will reduce CRIADO's original contract by the same amount. This task includes CADD Operator's time to provide CADD updates to the plans utilizing the Contractors As-Built Mark-ups. The breakdowns of the"Original Contract"and the"Original Contract with Amendment No.V are included in Attachment 1 and 2. For ease of evaluation, the values of the MWBE firm tasks are shown in red in these tables. With Amendment NO and the As-Built task revision under the original con'trwt between CRIADO and A.N.A. Consultants, CRIADO's MWBE participation for the project remains at 26%. We look forward to working with the City of Fort Worth Transportation and Public Works Department on the next project, With the approval of this amendment,the total contract will be$201,885.00 with a minority participation of 2£ 0 M/WBE.
