HomeMy WebLinkAbout(0067) M&C 21-0604City of Fort Worth, Texas Mayor and Council Communication DATE: 08/24/21 M&C FILE NUMBER: M&C 21-0604 LOG NAME: 14HEALTHCENTERAMENDMENT SUBJECT (ALL) Authorize an Amendment to City Secretary Contract Number 50597 Titled Master Services Agreement with UT Southwestern Accountable Care Network d/b/a Southwestern Health Resources Accountable Care Network to Add an Additional $500,000.00 to the Contract Annually and to Adjust the Terms to Allow for Unlimited Renewals at the City's Discretion RECOMMENDATION: It is recommended that the City Council authorize an amendment to City Secretary Contract Number 50597 titled Master Services Agreement with UT Southwestern Accountable Care Network d/b/a Southwestern Health Resources Accountable Care Network to add an additional $500,000.00 to the contract annually for the addition of a population health quality management program and to adjust the terms to allow for unlimited renewals at the City's discretion. DISCUSSION: The purpose of this Mayor and Council Communication (M&C) is to approve an amendment to City Secretary Contract Number 50597 titled Master Services Agreement the City has with UT Southwestern Accountable Care Network d/b/a Southwestern Health Resources Accountable Care Network ("SWHR") to add $500,000.00 to the contract to add a population health quality management program and to amend the terms of the contract to allow for unlimited contract renewals at the City's discretion. The City entered into the contract with SWHR to establish three (3) dedicated health centers and five (5) satellite centers in 2018. Since the inception of the health centers, the City has seen a decrease in medical claims costs for the active employees for the past three years. This is, in part, attributable to the contract with SWHR. The evolution of the arrangement with SWHR is to add a population health quality management program to the contract. This is a program specifically designed for the City of Fort Worth to with measurable goals to help close gaps in care among City members. The objectives of the program focus on four core areas: • Patient Navigation to ensure timely care transitions occur, support the member and providers to ensure clinical information is shared efficiently to avoid duplicate services and/or; • Care Management which provides added services for those with highly complex medical conditions and those that may need help adhering to treatment plans. This includes patient outreach specialists, pharmacy specialists, social workers, referral coordinators, post discharge outreach and risk stratification to identify high risk and complex members in need of care management; • Care Coordination which is a team of skilled care managers, social workers and pharmacists to identify members with complex conditions or chronic conditions to provide resources to ensure there are no gaps in care; and • Post -Acute Care which includes home health services designed to provide the right level of care, at the right place and time (both in and out of the hospital) to provide more cost effective and efficient care. In addition to the new population health quality management program, City staff is requesting a change to the terms of the existing contract. City staff would like to amend the contract from a five year contract with two one year renewals to a contract allowing for unlimited renewals at the City's discretion. As the City's health center program continues to expand, allowing unlimited renewals would ensure active employees and retirees have access to the quality primary care providers they have grown accustomed to seeing. FISCAL INFORMATION / CERTIFICATION: The Director of Finance certifies that upon approval of the recommendations, funds are available in the current operating budget, as appropriated, of the Group Health and Life Insurance Fund and that, prior to an expenditure being made, the participating department has the responsibility to validate the availability of funds.
