FIRST AMENDMENT TO CITY SECRETARY CONTRACT NO. 54951
This Amendment("Amendment") is entered into by and between the City of Fort
Worth ("City") and United Community Centers, Inc. (hereinafter "Agency") by and through
their respective duly authorized agents. City and Agency may be referred to individually as
"Party" and collectively as"Parties."
WHEREAS, City receives grant monies from the United States Department of Housing
and Urban Development ("HUD") through the Community Development Block Grant Program
("CDBG'),Program No.B-20-MC-48-0010, Catalog of Federal Domestic Assistance No. 14.218;
WHEREAS, the ESG regulations and related HUD guidance publications establish all
requirements for the CDBG funds;
WHEREAS, Section 3. the Contract permits Agency to request an amendment of the term,
subject to written approval by the Director of the City's Neighborhood Services Department;
WHEREAS,the Agency requested an amendment from City in order to extend the term of
the agreement through September 30, 2022; the City has included additional funding to meet the
goals of the contract and its extension.
WHEREAS, City believes that the requested amendment will help Agency meet the
program's goals and further the interests of both Agency and City; and
WHEREAS,it is the mutual desire of City and Agency to amend the Contract to complete
the Program and meet Contract objectives.
NOW, THEREFORE, City and Agency hereby agree to amend the Contract as follows:
I.
EXHIBIT"A"—PROGRAM SUMMARY attached to the Contract is hereby amended and
replaced in its entirety with EXHIBIT "A" Revised 10/15/2021, attached hereto. Additionally,
EXHIBIT "B" — PROGRAM BUDGET is hereby amended and replaced in its entirety with
EXHIBIT"B' Revised 10/15/2021, attached hereto.
II.

This amendment is effective as of October 1,2021 and Agency may request reimbursement
for allowable costs.

All terms and conditions of the Contract not amended herein remain unaffected and in
full force and effect, are binding on the Parties and are hereby ratified by the Parties. Capitalized
terms not defined herein shall have the meanings assigned to them in the Contract.

Educational Enrichment Program PROGRAM SUMMARY
PROGRAM SUMMARY

October 1, 2020 to September 30, 2022 $165,555.00
PERIOD AMOUNT
Capitalized terms not defined herein shall have meanings assigned them in the Contract.
PROGRAM:
The Educational Enrichment program will provide a child care literacy program.
The Program assist children pre-k to 13 years of age with reading, writing, comprehension and is
designed to increase literacy skills,United Community Centers, Inc. also provides a licensed child
care after-school program during the regular school year and full-day during the summer.
The after-school program classes are aligned with the Fort Worth ISD Calendar and provide a full-
day summer program. The Summer program is ten (10) consecutive weeks from early June to
mid-August.
UCC serves clients all day through October 08, 2020. Due to the COVID-19 Pandemic UCC will
follow social distancing guidelines and intends to resume normal program operations when it is
considered safe to do so. UCC is also reserving the right to possibly extend all day services if
families continue to need all day services.
CDBG Funds will be used to pay for a percentage of salaries for employees. The percentage of
any program employee's salary will not exceed the percentage of Fort Worth clients participating
in the Program and shall be cost allocated in accordance with the amount applicable to the
CDBG funded Program.
Agency's office is located at 3900 Meadowbrook Drive, Ste 122, Fort Worth, TX 76103.
After-School Program
Monday through Friday, 2:00 p.m. to 6:00 p.m.
Full-day Summer Program
7:00 a.m. to 6:00 p.m. for summer and on school waiver days/holidays.

Agency is a Fort Worth Agency which means at least 90% of clients served by the Program are
Fort Worth residents. Agency will submit documentation which verifies that at least 51% of all
clients served by the Program, whether Fort Worth residents or not, are Income Eligible Clients
with current household incomes at or below 80% of AMI.

Agency will maintain documentation that verifies that 51% of clients served by the Program are
Income Eligible Clients.
PROGRAM GOALS:
Minimum Fort Worth Agency Program Performance Outcome: At least 51% of all
Program clients must be Income Eligible Clients. Agency must furnish City a Certification of
Income Statement for all clients served. For example, if the Program serves 1,000 clients, 510
must be Income Eligible Clients. In this example,Agency would be required to furnish City
1,000 Certification of Income Statements that prove at least 510 of the clients served by the
Program are Income Eligible Clients.

Minimum Number of Fort Worth Clients to be Served: The Educational Enrichment Program
must serve a minimum of 231 Unduplicated Clients from Fort Worth as shown by the monthly
reports on Attachment III.
When determining the number of clients which constitutes 51%, HUD regulations require that
partial numbers must be rounded up.
