WHEREAS, it has become necessary to execute Amendment No. 4 to the CONTRACT to 'include are ncreased scope of work and revised fee., NOW THEREFORE, CITY and ENGINEER,, actinct herein by and through their duly authorized representatives, eater into the following agreement, which amends the CONTRACT,- 1 . Article 1 of the CONTRACT is amended to include the additional engineering services spec i f Ted in a proposal dated February 20,, 2013 , a copy of which is attached hereto and 'incorporated herein. The cost to City for the additional design services to be performed by Engineer totals $27, 678 . 00 . .Article II of the CONTRACT is amended to provide for an increase in the fee to be paid to Engineer for all work and services performed under the Contract, as amended, so that the total fee paid by the City for all word: and services shall be an amount of $1, 138 , 374 . 00 . Previous project work included survey in the project area; however, some additional survey will be required. It is anticipated that the survey will take approximately I week to complete. Assumptions Cl 12M HILL will deliver 3 1-tal f's ze sets )f drawin.gs at 60% and 90% design. CI 12MI-1111,will deliver 7 full-sized sets:and I full-sized Mylar of the 1010% final draw migs signed and sea-led by a reg"stered 'Professional Eng I ,i,ne(,,,,r in the S'tate of'Texas for the construction of the improvements by a City Contractor,. Bid Phase and Construction phase tasks,are not tricluled in this scope of work. C11 2M HILL will conduct o site visit to identify each affected water meter within the project limits. It is assumed that the proposed water line design w ill InclUde a total of 5 sheets. Amendment No. 4 February 20, 2013 Page 3 Preparation of tecl-inical specification sectio�ns is not included in the scope of work. It is assunied that the City of Fort Worth will deliver these to the contractor and that 0-12M HILL will not be required to deliver specirication sets. It is assumed that the new water line will be shown in plan view only. It is assumed that rio additional survey work will be required beyond Avenue G or Avenue M. If additional survey work is needed 0-12M HILL will halt work and inform the City of Fort Worth. It is assure-ied that no hydraulic design or capacity conformation is required for this work. It is assumed that C M HILL will attend a one project kickoff meeting, one review meeting at each of the 30% and 90% design phases. It is assumed that additional Traffic Control Plans are not required for this work It is assumed that additional SW3P is not required for this work; Erosion Control Plan to be used will be the existing Erosion Control Plan for the existing,construction job U'l the area. Design of permanent pavement is not included in the design effort. Proposed Schedule CH2M HILL will begin work Immediately upon notification from ffie City of the approval of this contract amendment.
