The origin of the CMR is described in the CDM Smith memorandum. The original proposal from
Landmark was received, and at that time, additional information was requested by CDM Smith.
Additional information, in the form of the SJ Louis proposal, was received and reviewed by CDM
Smith and subsequently discussed with Landmark.
As noted in our previous discussion, the quantities in the proposal from SJ Louis were well in excess
of what would be expected based on the original installation detail provided in the CDM Smith
memorandum, including allowances for construction excavation tolerances. It was understood that
Landmark agreed with this, and upon review of the probable construction requirements,
subsequently suggested that the proposal be refined to reflect the construction and provision of the
barrier at one-third of the original excavation, materials, and labor. CDM Smith discussed the
revised proposal with the City and it was determined to be acceptable. As such, it is understood
that the CMR-01 proposal is for a total value of $ 3,960.00.

The original Landmarlc proposal included a request for additional contract time. As the requested
proposal brealcdown information was not provided as requested, the completion of the worlc did
not iinpact substantial completion, and Landmarlc did not otherwise complete unrelated punchlist
items in advance of the CMR-01 worlc, the additional time is considered not applicable and is not
approved, It should be noted, that the substantial and final completion dates for operational
availability of the tanlc and project completion, respectively, are considered acceptable and the
application of liquidated damages is not being considered at this time.

Subject: Northside IV Elevated Storage Tank and Pipeline Project
Pipeline Groundwater Barrier
In follow-up to the November 17, 2014 meeting involving City of Fort Worth, SJ Louis, Landmark,
and CDM Smith regarding discussion of the identified groundwater concerns related to the NS-4
pipeline tunnel, this memorandum is to address an action item assigned to CDM Smith.
As discussed in the meeting, Landmark advised migration of groundwater within and along the
pipeline trench towards the elevated storage tanlc foundation is a potential concern. Landmarlc was
to confirm with their structural engineer regarding this potential situation and advise the City and
CDM Smith of his/her comments. However, in effort to inhibit the potential groundwater
movement along the pipeline trench, the City requested that CDM Smith provide a detail for a
controlled low-strength material (CLSM) pipeline collar. Accordingly, the attached Detail BE, as
shown on page 2 of this memo, is proposed for installation at approximately Station 5+60. The
collar shall be at minimum of the dimensions shown and shall be placed in undisturbed earth. For
installation and placement, Groundwater flow shall be managed as required for placement and
curing of the CLSM.

Just to follow up on our meeting yesterday afternoon, please forward the batch tickets as requested by the City of Fort
Worth for the CLSM worlc depicted on Contract Drawing CZ-11:Detail EB for both the North and South side of the tunnel
within the next couple of days. This will mitigate the underlying concern I noticed that these were installed per plans
and specifications. I will keep in touch with CDM Smith throughout this weelc in regard to the additional detail to the
Vault and the their intentions. Once I receive the revised detail I will forward it on. As discussed, I want to maintain the
sense of urgency to finish up this project as soon as feasible and maintain both of our firms continued relationship with
the City of Fort Worth. It is our intent to have this tanl< ready to fill prior to the end of the year and finish up all site

I am digging through some of the information outstanding for the Northside Tanl< and I see that we still have the Change
Order for the CLSM install out there pending. I understand there has been a substantial amount of time since this worl<
was installed and it appears that the ball was left in our court to provide you some additional brealcdown
information. When I met with Pete Stahl on site to finish up the punch list he mentioned this to me and has since
provided some more information fairly quicl<ly as if it was provided, but I cannot tell that it had. The attached letter was
the original proposal sent in December 2014. You responded on 5/6/15 requested additional information and a
breal<down of these costs for evaluation. The second attachment provides this to include the invoice from Cow Town
Redi-Mix (referencing a ship date of 2/11/15), the original proposal provided in our December letter, a specific time
sheet to the worlc performed that day by SJ Louis, and a thorough breal<down of how the original number was
generated.
Lil<e I mentioned, I am somewhat lool<ing at this from the outside-in at this point but understand that the worl< was
completed per the change order due to the ground water concern we met about previously. Give me a shout to discuss
at your convenience and let me Icnow what else is necessary to continue this one for final approval before we close this
project out.

CMR-02 Cathodic Protection Supplement
Based on review of the submitted Landmarlc 0&M manual documentation, it was previously noted
that when tested, the cathodic protection system did not meet performance requirements. From on
further review of this information by CorrPro, it was their conclusion that a`short' in the cathodic
protection was occurring. Potential reasons for the short included metal-to-metal contact between
the pipe and tunnel liner plates and/or the presence of groundwater within the tunnel. CorrPro
advised that due to the `short' additional sacrificial anodes were required to protect the tunnel and
pipe system as a whole.
To provide a cathodic protection system that will be in compliance with the specifications due to
the new conditions, a revised CorrPro design was requested. Attached to this memo are CorrPro's
proposal and notes regarding CorrPro's evaluation commentary. CorrPro's proposal includes
provision of the labor, materials, shipping, testing and final report associated with the supplemental
cathodic protection elements.
It was requested that the installation of the CorrPro provided system be performed by Landmarlc.
Attached to this memo is the Landmarlc proposal for execution of this work. The cathodic
protection system shall be installed in accordance with CorrPro requirements so that the system is
fully protecting the installed piping/tunnel system in accordance with the specifications. If testing
determines that the system is not functioning to protect the entire system, remedial activities by
Landmark and CorrPro will be required.

Landmarlc shall provide and otherwise adhere to the following requirements for fulfillment of this
CMR.
• Extension of the cathodic protection system shall occur wholly within the limits of
the temporary and permanent construction easements associated with the project,
• Implementation of all applicable site safety measures including, as required, trench
safety measure
• All excavation spoils shall be removed from the worlc areas and grading repaired to
pre-construction conditions,
• Prior to initiation of the worlc associated with these proposals, Landmark shall
provide the City with 72 hour notice.
• Landinark shall also appropriately coordinate all utility locates within the area of
excavation and construction in accordance with State of Texas and contract
document requirements.
• Following installation, Landmarlc shall scarify and apply seed to the disturbed areas.
Further Inaintenance of the areas will not be required.
• Landmarlc shall provide marlc-ups to the previously submitted record documents
for review and update by CDM Smith.
Completion of worlc defined in the Landmark proposal shall be amended to include the additional
items and notifications identified above.

As requested, below is the pricing to furnish and install 20 additional anodes for the waterline/tunnel
cathodic protection system. Anodes will be installed per Corrpro Change Order dated December 21,
2015. 10 anodes will be installed at Station 1+00 and 10 anodes at Station 4+78, Anodes will be installed
at a depth of 5 feet and at 3 foot spacing. The new anodes will be connected to the existing system at
the respective test stations. Corrpro will issue a report after completion and testing. Please issue a
change order so we can proceed with this worlc.
Additional Cost:
Corrpro (Re-design, Materials, Testing) $5,990.00
Dowager (Anode Installation) $10,000.00
Landmark Fee (10%) $1,599.00
Total Additional Cost $17,589.00
Additional Contract Time:
Contract duration will need to be adjusted accordingly to match actual delay through
completion of worlc.
Exclusions:
- We assume full access to both sides for anode installation
- No backfill density testing
- No greenery restoration
- No additional submittals will be provided for this work or installation.

To address the shorted casing, SJ Louis will need to install 10 anodes at 1+00 and 10 anodes at 4+78 per
drawing C-2. The anodes will need to be installed at a depth of 5 feet and at 3 feet spacing. The attached
change order includes materials, shipping, and a site visit for testing as well as a report. Once the signed
change order is received we can ship the materials to SJ Louis.

Call Summary Notes:
➢ CDM Smith to forward as-built documents by Landmark to CorrPro (partial set attached)
for CorrPro review of cathodic protection installation and development of additional
anode installation requirements. Recommend CorrPro refer to their submittal drawings
also. Encasement at the south bulkhead of the tunnel is not clearly
shown. Landmark/SJ Louis will need to provide additional information if requested by
CorrPro

➢ CorrPro clarified the following:
o See call notes below from previous discussion between CDM Smith and
CorrPro. CorrPro reiterated items noted below.
o Test readings less negative than -850 mV indicates lack of proper protection and
readings slightly more negative indicate that the designed 20-year protective life
of the cathodic protection system will not be provided.
o Comparative test readings from tunnel and pipe indicate short is present as test
readings at Station 1 were the same, Station 2 were within 10 mV.
o Remediation of the system in-place can be accomplished with additional
anodes, due to the `short', for protection of the combined tunnel/pipe system.
o CorrPro will update calculations to determine what amount of additional
protective anodes are needed. Expected next week once CDM Smith provides
Landmark as-built information.
o Installation of additional anodes will require vacuum excavation and trenching
for proper placement. Anodes would not need to be placed at pipe depth due
to issues with rock at lower profile. Installation can be approx 10-ft deep and 3-
ft apart.
➢ Landmark raised question as to who will pay for the additional installation. All parties
agreed to table discussion until CorrPro can provide summary of remediation
requirements.
Proposed that we reschedule as noted due to conflict - 11:00 to 12:00

11 AM.
• CorrPro advised that test information indicates there is a short between the liner plate
casing and the pipe. (upon check of project documents, spacers were to be installed
with 1/2" rubber pad). CorrPro advised that groundwater could impact system
isolation.
• CorrPro advised that likely remediation method for a`short' condition could possibly
be additional anode protection for the combined pipe and liner plate system.
• Secondary conference call to be scheduled with City, Landmark, CorrPro and CDM
Smith. Invitation of others to be coordinated by the individual invitees.
