HomeMy WebLinkAboutContract 44636-A1�� $��:�� AMENDMENT No . 1 �;�i��� ��. ��� i-f��,`)�F' _ l� � TO CITY SECRETARY CONTRACT No. 44636 WHEREAS, the City of Fort Worth (CITY) and Dunaway & Associates, L.P., (ENGINEER) made and entered into City Secretary Contract No. 44636, (the CONTRACT) which was authorized by M&C C-26319 on the 9th day of July 2013 in the amount of $ 387,787.00; and WHEREAS, the CONTRACT involves engineering following project: Bellaire Park Court Drainage Improvements; and services for the WHEREAS, it has become necessary to execute Amendment No. 1 to the CONTRACT to include an increased scope of work and revised fee. I�70W THEREFORE, CITY and ENGINEER, acting herein by and through their duly authorized representatives, enter into the following agreement, which amends the CONTRACT: 1. Article I of the CONTRACT is amended to include the additional engineering services specified in a proposal dated May 30, 2014, a copy of which is attached hereto and incorporated herein. The cost to City for the additional design services to be performed by Engineer totals $43,785.00. (See Attached Funding Breakdown Sheet, Page -3-) 0 2. Article II of the CONTRACT is amended to provide for an increase in the fee to be paid to Engineer for all work and services performed under the Contract, as amended, so that the total fee paid by the City for all work and services shall be an amount of $431,572.00. 3. All other provisions of the Contract, which are not expressly amended herein, shall remain in full force and effect. EXECUTED and EFFECTIVE as of the date last below. City of Fort Worth Transportation & Public Works Department Fort Worth, Texas 76102 Reference: Amendment No. 1 Bellaire Park Court Drainage Improvements City Project No. 01081 City Secretary Contract No. 44636 Mr. Perkins: This letter constitutes our request for suppiemental authorization for tasks outside the scope of services of our current contract. The additionai work includes the following tasks: General 1. Design Management and Coordination — The original project included 12 months of ineetings, communications and reporting functions. Due to the additional effort needed to include additionai features related to water and sewer, an additional two months is anticipated. 2. Park Conversion Documents and Coordination — On 3/12/2014, a meeting was heid with PACS staff to present and discuss the drainage outfall in Overton Park. As a result of the meeting, the outfall design was adjusted to minimize erosion and impacts to trees. A 60% review meeting was held on 4/14/2014 with SWU staff to discuss drainage improvement plans. As a result of that meeting, Dunaway was requested to develop further improvements to rninimize impacts to Overton Park in the location of the outfall. As part of this process, documents related to Park Conversion will be required and will include the following tasks: a. Coordination meetings with Parks and Community Services Department and other City Department staff (Up to 3 meetings). b. Park Land Conversion Administration —(Prepare Parl< Conversion forms, prepare letter of request to PACS, including justification documentation, deed research and restoration & mitigation exhibits.) c. Metes and bounds easement document. d. Staking of proposed easement. e. Water Department Coordination — On 4/28/2014, a meeting was held with Water Department staff and their consultant Teague Nall and Perkins, to discuss coordinating this project with the Bellaire Drive South 24-inch water line and sanitary sewer improvements. It was decided that the pians will be combined with the water line plans and that additional coordination will be necessary. Updating the pians and specifications to include references to the water department plans and updating specifications. Additional coordination to answer questions during bid phase will be required. It is anticipated that one (1) additional meeting will be required. 4. Water and Sanitary Sewer Relocation — in Bellaire Park Court, from Station 20+25 to Station 23+06, the proposed storm drain is in conflict with an existing 6-inch water line and an existing 10-inch sanitary sewer line. It is anticipated that 1 water plan sheet and one sanitary sewer plan and profile sheet will be needed. 5. Easements — The project will require a two (2) Permanent Drainage Easements. The original contract includes one easement, so one additional easement will be required to complete the project. 6. Construction Phase Services — The project will require support for the construction phase as follows • Construction Support: a. Attend one preconstruction conference. b. Visit the project site to observe and report on progress. Assume that six site visits are required. c. Review shop drawings, samples and other submittals by the Contractor for general conformance with the design concepts and requirements of the contract for construction. This shall not relieve the Contractor from its responsibility for perFormance in accordance with the contract for construction, nor is such nor is such review a guarantee that the work covered by the shop drawings, samples and submittals is free of errors, inconsistencies or omissions. The Engineer shall log and track all shop drawings, samples, and other submittals in Buzzsaw. Assume two submittal reviews are required. � Requests for Information / Change Order Review • Attend one "Final" project walk-through and assist City in preparation of a final punch list. • Prepare record drawings from information provided by the City depicting any changes made to the final drawings during construction. The City shall provide the as-built survey, red-Iline markups from the Contractor and City Inspector, copies of approved Change Orders, and copies of Approved Substitutions. The Engineer shall modify the final drawings electronically and place a stamp on the plans indicating that they represent record drawings of the project as constructed. Each sheet shall clearly indicate all changes which apply to that sheet. One mylar set of the sealed Final Drawings, modified and stamped Record Drawings shall be provided the City along with Amendment No. 1 May 30, 2014 Fage 3 original red-lined drawings. Record drawings shall also be provided to the City in Adobe Acrobat format. A surnmary of the total project is as follows: Basic Fee 1. Original Contract 2.
