Pedestrian Crosswalk Removal and Replacement. The work under this change order would first remove the current crosswalk which does not meet ADA requirements and replace or install a new crosswalk that meets ADA requirements. A full description of this issue is on the attached explanation from the construction project manager, D. Johnson. 2. Traffic Stop Bar Removal and Replacement. As a result of adjusting the crosswalk, the traffic stop bar will require adjustment. The work under this change order would remove the current traffic stop bar pavement marking and replace with a new traffic stop bar the required feet (typically 4 ft.) from the crosswalk. 3. Replacement of Damaged Loop Detector(s). During the construction phase, the vehicle traffic loop detectors at the intersection of East Lancaster and Handley were damaged. The work under this change order would replace the damagetl loop detectors with a new one. 4. Time Extension. This change order will also allow for a time extension of five (5) working days due to the additional work added to this project as described above. City staff reviewed the contractor's request for time extension and proposed costs and considers both to be fair and reasonable. It is understood and agreetl that the acceptance of this Change Order by the contractor constitutes an accord and satisfaction and represents payment in full (both time and money) for all costs arising out of, or incidental to, the above Change Order. Box 750 Fort Worth, TX 76101 817-927�4388 817-927-4389 Fax vnvw.lainao.com June 6, 2014 Subject: Handley Urban Village (CSJ: 0902-48-558) CO 3 Attachment In reference to item 677 -2005, 6�7-2007, 666-2041, 666-2048 and 0688-2003 through 5 on form 2146-L, this change order will allow for the construction and installation of the Pavement Marking at Handley Ave and E.Lancaster Ave as well as the Vehicle Loop Detector. During the demolition phase of the Handley Urban Village project, loop detectors were severed at the intersection of E. Lancaster and Handley Ave. At the request of the City of Fort Worth, Fain was asked to submit a price for the repair of the detector loops (0688 2003). To account for OHP, it would put the average low bid unit pricing above what is recommended. The difference being $1033.65 thus mak�ng that total requested for the change order for this particular item (0688 2003) $7924.65 Also at the request of the City of Fort Worth, Items (677-2005, 677-2007, 666-2041 and 666-2048) at the corner of E. Lancaster and Handiey Ave, it is necessary to remove and replace the stop cross bar and the pedestrian striping to insure ADA coznpliancy. The pricing of these items has to take into consideration the mobilization, barricades and traffic control items to ensure safety and quality control. This ultimately affects pricing associated with the TXDOT Average Low Bid Unit Price thus justifying the increase. The City has agreed and accepted the proposed bid amount submitted by the contractor for the above said work and understands the rules and regulations set farth by the THI�OT Average Low Bid Unit Price Statewide (Average B). Due to the nature of the work involved, the contractor respectfully request 5 working days to compiete the added items. The city is in full agreement with the contractors request for the additional working days for time, material and equipment that will be necessary to complete the added items in a timely and proficient manner. DESCRIPTION: ELIM EXT PAV MRK & MRKS (12"), and ITEM: 666-2041. DESCRIPTION: REFL PAV MRK TY 1(W) 12" (SLD) (090MIL) At the time of bid opening, TxDOT planned to install a new crosswalk at the location of East Lancaster Avenue and Handley Drive. Due to timing issues with the TxDOT contractor the crosswalks were not able to be installed during the streetscape construction timeframe. The City of Fort Worth instructed its contractor (The Fain Group, Inc.) to install the crosswalks based on the design plans from TxDOT. Those design plans were followed by the City's contractor and later discovered that the crosswalk does not align correctly with the south and north ramps and would not meet American Disabilities Act (ADA) requirements. The work under this change order would first remove the current crosswalk which does not meet ADA requirements and replace or install a new crosswall< that meets ADA requirements. Due to costs associated with mobilization, barricades, and traffic control, the following contractor's proposed unit price to remove and replace the pavement marking is above the TxDOT average low bid prices. o Pavement marl<ing removal: $2.50/LF, a 0.45C/LF difference higher than the TxDOT average low bid price of $2.05/LF. o Pavement marking replacement: $4.00/LF, a$1.12/LF difference higher than the TxDOT average low bid price of $2.88/LF. City staff considers the contractor's above proposed unit prices to be fair and reasonable. Traffic Stop Bar Removal and Replacement ITEM: 677-2007. DESCRIPTION: ELIM EXT PAV MRK & MRI<S (24"), and ITEM: 666-2048. DESCRIPTION: REFL PAV MRKTY 1(W) 24" (SLD) (100MIL) As a result of adjusting the crosswalk, the traffic stop bar will require adjustment. The work under this change order would remove the current traffic stop bar pavement marl<ing and replace with a new traffic stop bar the required feet (typically 4 ft.) from the crosswall<. The contractor's price to remove the pavement marl<ing is below the TxDOT average low bid price but is higher to replace the pavement marking due to costs associated with mobilization, barricades, and traffic control o Pavement marl<ing removal: $2.50/LF, a 0.78C/LF lower than the TxDOT average low bid price of $3.28/LF. City staff considers the contractor's above proposed unit prices to be fair and reasonable. Replacement of Dama�ed Loop Detector(s) ITEM: 0688-2003. DESCRIPTION: VEH LOOP DETECT (SAWCUT) 4AWG BLK During the construction phase, the vehicle traffic loop detectors at the intersection of East Lancaster and Handley were damaged. The work under this change order would replace the damaged loop detectors with a new one. The unit price presented by the contractor is $5.90/LF which is higher than TXDOT's average low bid unit price of $4.00/LF with a difference of $1.90/LF. City staff considers the contractor's proposed unit price is fair and reasonable. Time Extension This change order will also allow for a time extension of five (5) working days due to the additional work added to this project as described above. City staff reviewed the contractor's request for time extension and considers it fair and reasonable. If additional information is needed, please let me know.
