Kutilek d/b/a The Landscape Alliance (ENGINEER), made and entered into City Secretary Contract No. 44578 (the CONTR.ACT), which was authorized by City Council on the 4 day of June, 2013, in the amount of $ 77,800.00; and WHEREAS the CONTRACT involves engineering services for the following proj ect : Sandy Lane Park Improvements; and WHEREAS, the CONTRACT was subsequently revised by: Amendment Number 1 in the amount of $138,759.50, which was authorized by M&C #C-26715 on March 18 2014, for the following project: WHEREAS, the CONTRACT was subsequently revised by: Amendment Number 2 in the amount of $20,370.00, which was executed administratively on October 20, 2014, for the following project: WHEREAS, it has now become necessary to execute administratively Amendment No. 3 to the CONTRACT to increase the scope of work and revise the fee. NOW THEREFORE, CITY and ENGINEER, acting herein by and through their duly authorized representatives, enter into the following agreement, which amends the CONTRACT: 1. Article I of the CONTR.ACT is amended to include the additional engineering services specified in a proposal dated January 14, 2016, a copy of which is attached hereto and incorporated herein. The cost to CITY for the additional design services to be performed by ENGINEER totals $2,250.00. (See Attached Funding Breakdown Sheet, Page -3- ) 2. These services are based on additional requirements of the proj ect as requested by the USACOE to complete processing of our submitted permit application. As discussed in recent meetings with you, they have requested a limited archeological survey along the Cottonwood Creek portion of the park. The completion and submittal of this work should be the last step in securing the Corps permit. We have contacted AR Consultants, Inc. of Richardson, the same firm we used in recent work at Northwest Community Park. They will conduct the required field work, provide a report and necessary submittals to the Corps and Texas Historical Commission. The additional cost for this work is $2,250.00. The work will be conducted in an expeditious manner to avoid further delays in the project. Fees The proposed additional fees for the services defined above is outlined below. Fees will be billed on an hourly basis, based on services actually rendered. The City of Fort Worth will be advised in advance of any anticipated changes in services which will impact the fee. Included in the proposed fee are costs associated with reproduction and travel related to the production of the contract products. Items not specifically included in the estimated fee include: Permit application and review fees other than TDLR. 951 W.
