SECOND AMENDMENT TO
CITY SECRETARY CONTRACT NO. 55543
This Amendment ("Amendment") is made and entered into by and between the City of
Fort Worth ("City") and AB Christian Learning Center ("Agency") by through their duly
authorized agents. City and Agency may be referred to individually as "Party" and jointly as
"Parties."
WHEREAS, the City of Fort Worth ("City") and AB Christian Learning Center
("Agency")made and entered into City Secretary Contract No. 55543 ("Contract");
WHEREAS, City receives grant monies from the United States Department of Housing
and Urban Development (HUD) through the Community Development Block Grant (CDBG)
Program, Program No. B-20-MW-48-0010, Catalog of Federal Domestic Assistance No. 14.218;
WHEREAS, City has received an additional allocation of CDBG grant monies under the
Coronavirus Aid, Relief, and Economic Security Act ("CARES" Act), referenced by HUD as
"CDBG-CV3";
WHEREAS,pursuant to Section 3 of the Agreement,the contract may be extended if such
extension is necessary for completion of the program and is authorized, and the Agency is
requesting an amendment to extend the term of the agreement through September 30, 2022, as
reflected in the attached amended Exhibit A;
WHEREAS, City believes that the requested amendment will help Agency meet the
program's goals and further the interests of both Agency and City; and
WHEREAS,it is the mutual desire of City and Agency to amend the Contract to complete
the Program and meet Contract objectives.
NOW,THEREFORE, City and Agency hereby agree to amend the Contract as follows:
I.
EXHIBIT "A" — PROGRAM SUMMARY attached to the Contract are hereby deleted and
replaced in their entirety with EXHIBIT"A"Revised 06/01/2022, attached hereto.
H.
This amendment is effective as of June 1, 2022 and Agency may request reimbursement
for allowable costs directly related to implementation CDBG CV3 funds.

All terms and conditions of the Contract not amended herein remain unaffected and in full
force and effect,are binding on the Parties and are hereby ratified by the Parties. Capitalized terms
not defined herein shall have the meanings assigned to them in the Contract.

Capitalized terms not defined herein shall have meanings assigned them in the contract.
PROGRAM:
The Program will continue service to provide a safe place for children to receive assistance with on-
line learning while parents return to work,or for parents who are unable to navigate the technology of
on-line learning. The program responds to the effects of the COVID-19 Pandemic by helping children
catch up on learning they may have missed due to closure of regular in-person school. Through on-
line assistance, students receive instruction and lessons directly from their Fort Worth ISD (FWISD)
teachers via digital and virtual means,they also benefit from the aid of a Teacher Assistant to help them
understand the digital lessons and navigate through the online learning technology.
Eligible clients are defined as low-income students, and will be required to provide Income
Certification Form to confirm eligibility.
CDBG-CV funds will be used to pay for direct cost of delivering the program, including salaries,
related costs of,cleaning, site use rental,cleaning supplies and services,and PPE.
Agency's office is located at 5005 Brentwood Stair Rd. Suite 200,76112,with operating hours Monday
thru Friday 8:30 am to 5:00 pm.

Minimum Fort Worth Agency Program Performance Outcome: Agency will maintain
documentation which verifies that 5 1% of clients served by the Program are Income Eligible.
Minimum Number of Fort Worth Clients to be Served: The Program will serve a minimum
of 60 Unduplicated Clients from Fort Worth as shown by the monthly reports on Attachment III.
