1. For projects disturbing a surface area of more than 1 acre The City specifications require the
contractor to submit a project specific storm water pollution prevention plan (SWPPP). TxDOT
has a predesigned SWPPP under its general storm water permit in its standard detail sheets.
Both stipulations are contained in the project contract documents with bid items for the TxDOT
plan, but no bid item for the project specific plan. The contractor and City inspection understood
that TxDOT wanted the project specific plan to be implemented from a response from TxDOT at
the preconstruction conference. After the contractor proceeded in good faith, it was
determined that the specific SWPPP was redundant and not required.

This letter serves to designate either a person or specifically described position as an authorized person for
signing reports, storm water pollution prevention plans, certifications or other inforination requested by the Director
or required by the permit. This authorization cannot be used for signing an TPDES permit application (e.g. Notice
of Intent (NOI)) in accordance witl� 30 TAC Section 305.128. The following person or position is hereby attthorized
to sign reports, plans or certifications nther than the NOI application:
By signing this authorization, I confirm that I meet the following requirements to make such a designation
as set focrth in 30 TAC Section 305.128.
(1) For a co�poration, the application shall be signed by a responsible corporate officer. For purposes of this paragraph, a
sponsible corporate officer means a president, secretary, treasurer, or vice-president of the coiporation in charge of a principal business
.unction, or any other person who performs similar policy or decision-making functions for the corporation; or the manaber of one ar more
manufacturing, production, or operating facilities employing more than 250 persons or havii�g gross anmial sales or expenditures
excecding $25 million (in second-quarter 1980 dollars), if autliority to sign documents l�as been assigned or delegated to t}►e m�nager in

The cost of this change order is $1,400.83 as outlined in the contractor's (The Fain Group) letter
dated April 10, 2013. The cost of this change order will be covered by City funds and not the
FHWA grant.
