WHEREAS. the City of Fort Worth (City) and Wade & Associates, Inc. ,
(Engineer) made and entered into City Secretary Contract No. 29667, (the
Contract) which was authorized by the City Council by M&C
C-19973 on the 24th day of February, 2004 and subsequently revised by one
amendment; and
WHEREAS, the Contract involves engineering services for the
following project:
Sanitary Sewer rehabilitation Contract LX (60) .
WHEREAS, Wade & Associates, Inc. assigned its rights and obligations
under Contract No. 29667 to CH2M Hill, Inc.
WHEREAS, it has become necessary to execute Amendment No. 2 to said
Contract to change Engineer to CH2M Hill, Inc. , to include an increased
scope of work and revised maximum f ee.
NOW THEREFORE, City and Engineer, acting herein by and through their
duly authorized representatives, enter into the following agreement which
amends the Contract:

Engineer shall mean CH2M Hill, Inc.

Article I of the Contract is amended to include the additional
engineering services specified in a proposal letter dated October 3,
2011, a copy of which is attached hereto and incorporated herein. The
cost to City for the additional design services to be performed by
Engineer total $49, 636.40 (Water: $28,337.00; Sewer $10,814.00 and TPW:
$10,485.00) .

Article II of the Contract is amended to provide for an increase in
the maximum fee to be paid to Engineer for all Work and services
performed under the Contract, as amended, so that the total fee paid by
the City for all Work and services shall not exceed the sum of
$565,861.00.
4.
All other provisions of the Contract which are not expressly amended
herein shall remain in full force and effect.

Contract 60 Sanitary Sewer Rehabilitation

Dear Ms. Schwinger:
The purpose of this letter is to request authorization for additional funding in the amount of
$49,856. This additional funding request is for additional services to Contract 60 Part 2
($25,412) and Contract 60 Part 4 ($24,224) as summarized below:
Contract 60 Part 2 Additional Services
Proposed Water Line Plan
Per your e-mail dated August 05,2010,CH2M HILL understands that the City plans to
remove and replace approximately 2,040 linear feet (1f) of water line along Tierney Road
from Cliffview Drive to Barnett Street.
CH2M HILL will provide design services necessary to remove and replace the existing 6-
inch waterline with an 8-inch water line along the same alignment. CH2M HILL will show
the service connections on the same sheet. CH2M HILL will determine quantity changes of
the water line pay items the City estimated for Change Order No.2 as provided in email
dated September 27,2010 in lieu of an engineer's cost opinion and bid form for the water
line improvements. Additionally,CH2M HILL will provide applicable water specifications
and details that are not currently included in the existing bid package for Contract 60,Part 2.
Additional survey work will be required for the water line tie-in near the intersection of
Barnett Street.
The cost for design services associated with the water line removal and replacement is
$14,927.00 as summarized in Exhibit 1.

Plan
It is our understanding that the City of Fort Worth would like a final review set of the water
line drawings two weeks after notice to proceed. The additional survey required near the
intersection of Tierney Road and Barnett Street will take approximately 1 week to complete.
CH2M HILL will deliver a review set to the City two weeks after the notice to proceed.
Assumptions
CH2M HILL will deliver 7 full-sized sets and 1 full-sized Mylar of the proposed
improvements signed and sealed by a registered Professional Engineer in the State of Texas.
It is assumed that the City of Fort Worth will deliver the drawings to the contractor and that
CH2M HILL will not be required to deliver complete drawing sets.
It is assumed that the proposed water line plan will take 8 sheets to complete.
CH2M HILL will deliver 7 sets of relevant technical specification sections and details. It is
assumed that the City of Fort Worth will deliver these to the contractor and that CH2M
HILL will not be required to deliver complete specification sets.
It is assumed that the new water line will be shown in plan view only.
It is assumed that no additional survey work will be required beyond that aforementioned
near the intersection of Tierney Road and Barnett Street. If additional survey work is
needed CH2M HILL will halt work and inform the City of Fort Worth.
It is assumed that no hydraulic design or capacity conformation is required for this work.
It is assumed that CH2M HILL will attend a maximum of 1 review meeting.

Proposed Top of Curb Plan and Profile and End of Road Barricade Plan
Per your e-mail dated August 05,2010,CH2M HILL understands that the City plans to
remove and replace approximately 700-1f of curb and gutter along Tierney Road from
Kemble Street to Barnett Street.
CH2M HILL will provide design services necessary to design the top of curb plan and
profile for the roadway segment described above. The design shall include replacement of
the driveway approach and driveway sidewalk section. In addition,CH2M HILL will
provide an end of road barricade design to be located at the north end of Tierney Road.
The cost for design services associated with the Top of Curb Plan and Profile and End of
Road Barricade Plan is$10,485. Exhibit 2 is a summary of costs.

The design shall be delivered to the City of Fort Worth for approval four weeks after notice
to proceed.
Assumptions
It is assumed that the top of curb P&P and End of Road Barricade Plan will take 3 sheets to
complete.

CH2M HILL will deliver 7 full--sized sets and 1 full-sized Mylar of the proposed
improvements signed and sealed by a registered Professional Engineer in the State of Texas.
It is assumed that the City of Fort Worth will deliver the drawings to the contractor and that
CH2M HILL will not be required to deliver complete drawing sets.
It is assumed that this work will be performed under the City of Fort Worth's TPW Street
Maintenance Contract,and no engineer's estimate or specifications will be required.
Applicable quantities in the Microsoft Word document format provided in the August 5,
2010 dated email,however,will be required.
It is assumed that no additional survey work will be required. If additional survey work is
needed CH2M HILL will halt work and inform the City of Fort Worth.
It is assumed that CH2M HILL will attend a maximum of 1 review meeting.
It is assumed that additional Traffic Control Plans are not required for this work
It is assumed that additional modification of the existing SW3P plans is not required for this
work.
Contract 60 Part 4 Additional Services
Water Line Design Changes
Per our November 1,2010 meeting,CH2M HILL understands that the city desires CH2M
HILL to remove all 1,176-feet of currently designed water lines from the Contract 60 Part 4
design package.
These water line segments,summarized in Exhibit 3,will be completed and delivered to the
city as individual designs. It is our understanding that the designs will be shelved and
included as part of future bid packages completed by others.

The design change for the water line along 6th Avenue will now be increased in size from a
proposed 8-inch line to a proposed 12-inch line. This will require CH2M HILL to perform
additional design services and include a profile per the City of Fort Worth Design
Standards. The design change for the water line along Lake Street will include the revising
the proposed alignment to the existing alignment in lieu of the 25-foot offset from the west
street right-of-way as previously directed by the City.
The cost for design services associated with the Water Line Design Changes is$13,410.00.

Assumptions
CH2M HILL will deliver 1 full-sized mylar for each of the segments shown in Exhibit 3.
The improvements will be signed and sealed by a registered Professional Engineer in the
State of Texas.
It is assumed that the City of Fort Worth will "shelve" the designs for future use.
It is assumed that once "shelved" the designs will no longer require updating by CH2M
HILL.
It is assumed that no specifications will be required for these water line segments.
It is assumed that,other than the segment along 6th Avenue,the water lines will be shown in
plain view only.
It is assumed that no additional survey work will be required and that the profile along 6th
Avenue will be developed from record drawings provided by the City of Fort Worth.
It is assumed that no hydraulic design or capacity conformation is required for this work.
It is assumed that CH2M HILL will attend a maximum of 2 review meetings.

It is assumed that additional Traffic Control Plans are not required for this work
Seger Line Design Changes
Per our November 1,2010 meeting,CH2M HILL understands that the City of Fort Worth
desires to relocate a segment of Line F(L-2677 Sta. 0+00 to Sta. 1 +83). Currently,the line is
designed south of W.Magnolia Avenue in the alley between Washington Avenue and
College Street.
As currently designed,this segment of Line F would require trenchless construction to
traverse under several trees and backyard sheds.As an alternative,the City has directed
CH2M HILL to abandon the line in the alley and replace it in College Avenue (416 ft) and
Washington Avenue (322 ft). This will require re-design and additional survey.
The cost for design services associated with the Sewer Line Redesign is$10,814.00. Exhibit 5
is a summary of costs.

Proposed Schedule
Once this contract amendment is approved CH2MHILL can deliver final plans,
specifications,and details for Contract 60,Part 2 within 3 weeks.
Contract 60 Part 4 will require additional work from our Survey Subconsultant. This work
will be initiated upon receipt of the executed amendment. It is anticipated that once started
that review sets of the individual water line sheets, and the new sewer design can be
delivered in six weeks.

The original M/WBE contract goal for this project was 22 percent and our commitment was
25 percent.To date the M/WBE participation by CH2M HILL has been 34 percent. We
request to use the City of Fort Worth Acceptance of Previous M/WBE Commitment Form to
maintain our original commitment of 25 percent for the proposed revised total contract
amount up to and including Amendment No. 2.
