Home My WebLink AboutContract 52575-A4 CSC NO. 52575-A4 AMENDMENT NO. 4 TO CITY SECRETARY CONTRACT NO. 52575 SUPPLIER SERVICES AGREEMENT This Amendment is entered into by and between the City of Fort Worth (hereafter "City"), a home rule municipal corporation organized under the laws of the State of Texas, with its principal place of business at 200 Texas Street, Fort Worth, Texas, and Life Technologies Corporation (hereafter "Supplier"), a Delaware Corporation. City and Supplier are each individually referred to herein as a"Party" and collectively as the"Parties." WHEREAS, the Parties have previously entered into City of Fort Worth City Secretary Contract No. 52575 (the "Contract"); WHEREAS, the Contract was executed for Supplier to provide the City with laboratory supplies and instrument services on an as-needed basis for the City of Fort Worth Police Department; and WHEREAS, the City and Supplier now wish to amend the original Contract to include the updated Exhibit B—Price Schedule as of November 1, 2022. NOW, THEREFORE, City and Supplier, acting herein by the through their duly authorized representatives, enter into the following agreement to amend the Contract: 1. AMENDMENTS TO CONTRACT City shall pay Supplier in accordance with the revised price schedule of Supplier personnel who perform services under this Agreement in accordance with the provisions of this Agreement and Exhibit"B,"—Price Schedule, attached hereto. 2. ALL OTHER TERMS SHALL REMAIN THE SAME All other provisions of the Contract which are not expressly amended herein will remain in full force and effect.
