I Mike Bennett UNIT or DEPT I TPW Original Contract Amount Extras to Date Credits to Date Allowance Funds Previously Allocated Revised Allowance Balance CA#L1 J Date 1/19/2023 City Inspectorl Brian McGill Water Sewer UNIT 4 or Dept Totals IN ACCORDANCE WITH THIS CONSTRUCTION ALLOWANCE REQUEST, THE CONTRACT SHALL: Furnish all labor and material as per attached proposed Construction Allowance Request (with attachments): purpose for this allowance is to add construction items that were a) in the bid, but not included in a specific construction unit, or b) not included in the original bid. The are justifications for the additions: -The cement stabilized sand is necessary for water and sewer construction but was left off the bid items list in the Water Sewer Unit, but was included in the Stormwater unit. The Stormwater prices are going to be used for the Water / Sewer units. -Construction phasing for the water line on W. Seminary Dr. had to change to avoid a severe conflict with Westcliff Elementary School traffic. This change was required due to construction delays from shallow bedrock and further extended the construction schedule. This includes new construction items. -The items being added to Westcliff Drainage Improvements, Unit 3 are to facilitate the relocation of a franchise utility that is in conflict with storm drain construction. This will help the City contractor stay on schedule for a critical phase of construction that involves a temporary closure for Trail Lake Dr. This includes new construction items. Contract items and quantities being added with this allowance were found to be necessary during construction. Contract items and quantities have been evaluated by City staff and were found to be reasonable when compared to City standard prices, the City Stormwater projects Westcliff Drainage Improvements Ph. 2B and Loving Ave Channel and Culvert Improvements, and the City sewer project Sanitary Sewer Rehabilitation Contract 91 Pt. 1. It is understood and agreed that the acceptance of this Construction Allowance Request by the contractor constitutes an accord and satisfaction and represents payment in full (both time and money) for all costs arising out of, or incidental to, the above Construction Allowance Request. The Project/Program Manager signing below acknowledges that he/she is the person responsible for the monitoring and administration of this contract, including ensuring all performance and reporting requirements.
