SUBJECT: CRITICAL POLICE INCIDENTS, JANUARY 2021 THROUGH
rrn DECEMBER 2022
The purpose of this report is to provide an update on all Fort Worth Critical Police Incidents (CPIs) from
January 1, 2021, through December 31, 2022.
The Major Case Unit, consisting of one sergeant, seven detectives, two officers, and two civilians, is
responsible for the criminal investigation of all CPIs occurring within the City of Fort Worth, along with
offenses designated as hate crimes, cases involving missing persons, and cases of economic crimes
(fraud) of a monetary value of$150,000.00 or more. The Major Case Unit submits all cases resulting in the
serious bodily injury or death of any person that involves a police officer acting in an official capacity to the
grand jury for review. The Internal Affairs Unit is responsible for the administrative investigations of all
CPIs.
What Is a Critical Police Incident?
CPIs are assigned to the Major Case Unit of the Violent Personal Crimes Section of the Criminal
Investigations Division. A CPI, according to Fort Worth Police Department General Order 356.01 A., is
defined as:
1. Any discharge of a firearm resulting in the injury or death of any individual involving a Fort Worth
Police Department officer, a Fort Worth Fire Department arson investigator, a City of Fort Worth
marshal, or a law enforcement officer whose ordinary jurisdiction does not include the City of Fort
Worth, while in the course and scope of their official duties while in the Fort Worth city limits;
2. Any event where a Fort Worth Police Department officer, a Fort Worth Fire Department arson
investigator, a City of Fort Worth marshal, or a law enforcement officer whose ordinary jurisdiction
does not include the City of Fort Worth intentionally discharges a firearm at an individual or
exchanges gunfire with an individual while in the course and scope of their duties while in the Fort
Worth city limits;
3. Any event in which a Fort Worth Police Department officer, a Fort Worth Fire Department arson
investigator, a City of Fort Worth marshal, or a law enforcement officer whose ordinary jurisdiction
does not include the City of Fort Worth is struck by gunfire or is seriously injured, excluding vehicle
accidents, while in the course and scope of their official duties while in the Fort Worth city limits; or
4. The death or serious bodily injury, excluding vehicle accidents, of any individual in the custody of,
or in the process of being taken into custody, by a Fort Worth Police Department officer, a Fort
Worth Fire Department arson investigator, a City of Fort Worth marshal, or a law enforcement
officer whose jurisdiction does not usually include the City of Fort Worth, while in the course and
scope of their official duties while in the Fort Worth city limits.
Office of the Police Oversight Monitor and CPI's
The Office of the Police Oversight Monitor (OPOM) monitors all CPI investigations. OPOM receives
immediate notification when a critical incident occurs, reviews body-worn camera footage, and monitors
the Critical Incident Review meetings and work product. Staff can attend interviews and make
recommendations to the Chief if there are any concerns.
Critical Police Incidents in 2021
A total of fifteen CPIs occurred in 2021. Ten of these incidents involved in-custody injuries, suicides, or
other death. The other five incidents included four officer-involved shootings and an accidental discharge
injury. Four of the fifteen CPIs involved policy violations, though three of these violations were unrelated
to the corresponding CPIs.

Critical Police Incidents in 2022
A total of seventeen CPIs occurred in 2022. Eleven of these incidents included officer-involved shootings,
of which five were fatal. The other six incidents involved in-custody suicides or injuries. None of the
seventeen CPIs has yet been determined to involve policy violations, but nine of the cases are still under
investigation.

Haltom City patrol officers were conducting a Narcotics investigation and attempted a traffic stop of one of the
subjects. The subject led officers on a pursuit that ended when the subject exited the vehicle armed with a rifle Fort Worth Officers
