AMENDMENT NO. TWO TO CITY OF FORT WORTH CONTRACT NO. 59060
This Amendment is made and entered into by and between the City of Fort Worth, a home -rule
municipality ("City") acting by and through Fernando Costa, its duly authorized Assistant City
Manager, and ATLANTIC HOUSING FOUNDATION, INC - DBA QUAIL RIDGE
APARTMENTS ("Landlord") acting by and through SEBERA ROMAN, it's authorized
representative, each individually referred to as a "party" and collectively referred to as the
"parties."
RECITALS
WHEREAS, on March 1, 2023, the City entered into an agreement with the Landlord for
rental assistance for a named tenant, City Secretary Office (CSO) Contract No. 59060 (the
"Agreement");
WHEREAS, ("Tenant") is a participant in the City's rental assistance
program;
WHEREAS, the Parties recently acknowledged that the header in the Agreement
incorrectly identified "2333 ESCALENTE AVE LLC" as the Landlord to the Agreement and
entered into an amendment to identify the correct Landlord, but due to an administrative error the
landlord was incorrectly identified as "ATLANTA HOUSING FOUNDATION, INC. DBA
QUAIL RIDGE APARTMENTS" (CSC No. 59060-A1);
WHEREAS, the Parties acknowledge that the proper Landlord to the Agreement is and
always should have been "ATLANTIC HOUSING FOUNDATION, INC - DBA QUAIL
RIDGE APARTMENTS"; and
WHEREAS, the Parties wish to memorialize the proper Parties to the Agreement and
amend the Agreement to capture its fill intent.
NOW THEREFORE City and Landlord do hereby agree as follows:
I.
AMENDMENTS TO AGREEMENT
The Parties acknowledge that the Agreement incorrectly identifies "ATLANTA HOUSING
FOUNDATION, INC. DBA QUAIL RIDGE APARTMENTS" as the Landlord to the
Agreement and the proper Landlord to the Agreement is and always should have been
"ATLANTIC HOUSING FOUNDATION, INC - DBA QUAIL RIDGE APARTMENTS."
Landlord agrees to be bound by all of the terms and conditions of the Agreement as if it were
listed as an original party thereto.

II.
All terms and conditions of the Agreement not amended herein remain unaffected and in
full force and effect, are binding on the Parties and are hereby ratified by the Parties.
