PURPOSE OF AGREEMENT
To purpose of this agreernent is to clarify the relationship between and respective
responsibilities of xhe City of Fort Worth (hereinafter the "City") and Cook Childrens
Medical Cez�ter (CCMC) and to establish the respcctive responsibilities of each part wiih
regard to the Wraparound Philosophy: Train-the-Train.er Series.
I, TRAINING DESGRIPTION
a. The City has con�racted wiih Mary Grealish, Gommunity Partners, Inc., �o
provide wraparound training to participa�ing com.�a.unity meinbers in the
development of a systern of care for severely emotionally disiurbed children.
b. The primary goals of the training include:
1. to train community members on how to txain. and instruct others in use
of the wrapa�round philosophy when serving children and families,
2. to i�fuse the use of the wraparaund philosophy within f1�e community
and area service pravid�rs, and
3. to assist in building sustainability of a eomprehensive system of care
within the community.
c. The train-fhe-f�airier session wi11 occur over a 4-day period:
1. May 5, 2003- May S, 2003.
2. 8:30am to 4:30 pm each d�y.
3. Training Room @ Lynn W. Ross Detention Center, 2741 I�imbo
Road, Fprt Worth, TX 761 I 1.
4. Each agency/service provider can send a ma�mum o�2 staff tio the
training.
5. Lunch and CEUs r�vill be pravided.
d. Expectations af the training participants include:
1, participants becoxning knowledgeable in the use and implementation
of the wraparound philosophy, and
2. increasing the number of qualified trainers on �Iie wraparound
philQsophy within our community.

IY. RE5PONSIBILITIES OF CITY
The City will be responsible for:
a. coordinating and implementing this training,
b. securing the training consultant,
c. providing copies of all necessar}r forms and maierials,
d. providing maals and CEUs as reasonably determined by the Ciiy, and
e. providing on�gning support and technical assistance to training attendees
in accardance with the U.S. grant received for this p�pose, hereinafter
"the grant".
III, RESFON�IBTLITIES OF CCMC
f� CCIVIC wi bE esponsible far:
i�� �z. selecting �taff to become trainers on use of the wraparound philosophy,
(��jA b. allowing se ected �taff to adjust their schedule to accommodate the training

time,
c. ensuring that each trainer will conduct a minimuxn of 1 full-day (8 hour)
training wiihin their agency/organization by Novemb�r 8, 2D43, and
d. ensuring each t�ainer wi11 attend a scheduled cultural competency trazn.ing in
August 20Q3 with FWISD, and
��e, providing this training to other agencie�/organizations without an identified

N. DOCUMENTATION
a. City responsibilities include:
1. maintaining a sign-in lag with each participants name,
agency/arganization, address, ph�ne, fax, azid email, and
2. providing participating agencies with a form for documenting their
�ndividual trainings.
b. CCMC respon�ibilitaes include:
1, providiz�g a11 requested informaiion from City, and
2, providing documentation of individual trainings.
V. NON-DISCRIMINATION
Each party shall ensure that no person, on the basis of race, color, natianal origin,
religion, sex, age, disabili�y, veteran stat-us, or political affiliatioan will be excluded
from participation in, be denied the benefits af, ar be �u�ject to discriminatio�
with respects to services provided by t�e parties to this agreement.

VI. TERMS OF THE AGREEMENT
This agreement shall be effective May 5, 20�3 and shall expire an September 30,
2008.
This agreement may be renewed by muival agreement o�Eboth par�ies by writien
cansent, signed by both parties.
Any party may terminate participation in this agreement with or without cause
upon thirty (30) days written notice io the other party unless otherwise pravided
for by the grant.
VII. STGNATURES
The signature of an officer or authorized agent to the Memorandum of Understanding
signifies that the party agrees to abide by its contents and all other laws governing fihis
agreement and thes� parties.
