SCOPE OF WORK; LOCAL PTIBLIC HEALTH SLJPPORT FUND5 PERFORMING AGENCY shall use direct assistance andlor financiai assistance, as specified in SECTION III., BUDGET, frorn RECEIVTNG AGENCY to develop local public health systems and infrastruciure through carrying out one o�' more of the following essenCial public health ser�rices as speciiied in P�RFORMING AGENCY'S FY d3 Service Delivary Plan, which is adopted by reference: � Monitor ihe health status of individuals in the cQmmuriity to identify comm�nity health p�roblems; � Diagnose and investigate community l�ealth problems and community health hazards, � Inform, educate, and empower the community with respect to health issues; � Mobilize community parknearships to identify and so�ve community health problems; o Develop policies and plans ihat support individual and community efforts to improve health; � Enfarce laws and rules that protect t�e public health and ensure safety in accordance with khase laws and rules; � Link individuals who have a need for commun'rty a�nd personaI healt� services to appropriate community and pri�ate provid�rs; � En�ure a competent workfnrce fax the provision of essential public health services; � Research for new insights and innovative solutions �o cominunity healtia problems; and � Evaluate the effective�ess, accessibility and quality of persana� and population based hea�th ser�ices in a community. For the purposa of these grarat fnnds LocaI Ptiblic Healih Systems are defined as Ckie human, inforrr�ational, financial and organizational resources, including public, private and �oluntary arganizataons that contribute to the �711}}I1C'5 health [Source: Mobilizing far Action thraugh Planning and Partners�ips (MAPP) irom the Natianal Associatio� of County and City Health Officials]. FINANCIAL ASSYSTANCE PERFQRMING AGENCY shall direct funds toward developing a functional and effective public heaith system(s) with the speci€ic goal of impro�ing public health capacity to respond to both emergency and contin�ing public health threats. PERFORMANCE MEASURES The following performance measur�(s} will be used to assess, in part, PERFORNIING AGENCY'S effectiveness in providing the services described in this contract Attachment, withaut waiving the enforceability of any of the other terms of the cantx�aet. For the General Re�enue and/or Preventive Health and Healtl� Services Black Grant fitnds the PERFOItNIING AGENCY must: 1. Base all activities an the PERFORMING AGENCY'S approved Wo�ksheet for Local Public Health Preparedness and Respanse for Bioterrorism, This worksheet (www.tdh.siate.tx.us/onhn/yubslplan.pd� should have been comp�eted prior to August 31, 2002. 2. Identify local pubiic health syskem members critical ta improving public health capacity to respond to both ernergency and continuing public health threats (i.e. hospitals, EMS, f�re, police, first responders). 3. Identify a small, but representaizve group of stakeholders who contribute to or benefit from public health to help guide the LPHS de�elapment process. 4. Convene rnembers af the LPHS far the puxpose of developing and carrying out a(3-6 page) LPHS strategic plan faz' impro�ing public health capacity to respond io bath ernergency and continuing publie health threats. Adopt the atiached appropriation ordinance increasing estimated receip�s and appropriations by $4Q,000 in the Grants Fund upon receipt af the grant; and 3. DISCUSSI�N: The Texas Department of Health (TDH) prorrides grant funds to iocal health depariments across the state to pro�ide essentia! pt�blic health services. However, in response to local Public Health Preparedness and Response far Bioterrorism, the focus of these funds f�as sh�ft�d. In addition to continuing the °Walk for the Health of It!" program that is currently funded by a TDH grant, the Public Health Department will use these funds to achie�e a scope of work aimed at identifying and derreloping a strategic plan ta improve public health capacity #a respond to both emergency and continuing public health threats. Public Health staff will still address the fallawing esser�tial health func#ians, as defined by TDH: • Inform, educate and empower peaple about health status; and � Mabilize community partnerships to identify and solue heaith prohiems. A biiingual Health Educatar will be hired ta infarm, educat� and empower the local Hispanic community regarding health disparities in areas of cardiovascular and ather chronic disease pre�ention activities. This educator will continue pramotion of the "Walk for the Health of I�1" program by initiating walking pragrams and educating citizens on the benefits af physical activity, aimed at reducing the inci�ence of cardiovascular disease in our communi#y. This educator will also assist in the mobi[izafion of community partnerships ta respond to emerging and existing public health threats to the cammunity as a whole. The waiver af indirect casts is requested due t� the labor-intensive nature af this grant.
