SCOPE OF WORK: PERFORNIING AGENCY shall design, irnplement and evaluate a Diabetes Awareness and Education in the Cornmunity (DAEC) Praject. The DAEC pro�ect shall target Hispanic Americans, African Amerieans, the elderly and ather groups at r�slc for developir�g Type 2 Diabetes. PERFORMING AGENCY shall comply with aIl applicabie federal and state laws, zules, regulations, standards, ar�d guidelin�s in e�fect on the beginning date of this contract Attachment unless amended. PERFORMING AGENCY shall develop the following products: � An action plan for local enhancement of the DAEC project during the terrn of this contract Attachment; and, � A sustainability plan. PERFORNIING AGENCY is responsible for de�signing, implementing, and e�aluating the DAEC project. ATTACHII�ENT — Page 1 PERFORMTNG AGENCY shall provide e�idence that PERFORMING AGENCY is meeting end-o%project outcome o6jectives: • Increase �isitiility of the D.AEC to a� least 15% of the target population; and o It�crease the communrty's understanding of diabetes by at I�ast 10% from baseline. AlI activities shall be performed in accordance with RECENIlVG AGENCY'S Progxazn guideIines as set out in the Request for Proposal (RFP) issued Apri1 17, 2002, and with PERFORMING ACxENCY'S objecti�es, activitics, work plan, detailed budget and revisions, if any, as adopt�d by RECENING AGENCY'S Program and transmitted in writing to PERFORMING AGENCY. Th�se docu�ents are incorporated by reference and rnade a part of this contract Attachment. PERFORMING AGENCY shall con�piy with the following: o Chapter 103 of the Health and SafeCy Code; • Healthy People 2010, United States (U.S.) Department of Health and Hurnan Services, November, 2000; � RE�EIVING AGENCY'S Quality Care: Client S�r-vices Standards far Public Health and Community Clinics, Texas Deparkme�t af Heal.th", revised June 1997; and, o Diabetes Today. A Course on How to Build Skills to Pran and Irrrplement Community Based Pragrams for Persons with Diabetes, U,S. Department af Heaith and Human Services, Septerr�ber 200a. Within thirty (30) days af receipt of an amended standard(s) or guideline(s), PERFORMING AGENCY shall inform RECEIVING AGENCY Pragram, in writing, if it will not cantinue performance under this Attachrr�ent in compliance with thc arnended standard(s) or guideli�ie(s). RECENING AGENCY rnay terminate the Attachment immediately or within a reasonable period o� tirne as determined by RECENING AGENCY. PERFORMANCE MEASURES The following performance measures wi11 be used to assess, in part, PERFORMING AGENCY'S effectiveness in providing the ser�ices described in this contracC A��achment, without waiving the enforceabil'rty of any of the other terrns of the contract. • Conduct at least 68 key diabetes education activi�ies. � Ensure t�at at least 21,OOD individuals who have or are at �sk for diabetes par�icipate in key activi�ies. � Publish a minimurn of 0 diabetes related articles. � Pravide a riunimum of 5 diabetes related radio spots. • Provide a minimum of ]. �elevision spots. Authorize the City Manager to accept and exec�te a can#inuati�n to a grant contract with the Texas Departm�nfi of H�a1th (TDH) and Texas Diabetes ProgramfCouncil (TDPIC) in the amount of $100,OOD to be used to increase diabetes awareness and education at the community le�el from September 1, 2D02, #o August 31, 20�3; and 2. A�opt the atiached appropriation ordinance increasing estimated receipts and appropriations by $100,000 in the Grants Fund; and 3. Autharize a nan-interest bearing interfund loan from t�e General Fund to the Grants Fund in the amount af $25,000 for inter�m financing for this grant pending cantract execution to ensure a smoofh transition from one grant funding period to another. DISCU5510N: Diabetes is an epidemic and a public health crisis in Texas. The incidence rate has increased tenfold in the last 60 years (3.7 per 1,0�0 in 1936, to 3Q.� per �,04Q in 1995). The estimated pre�alence of diabetes in Texas is 6.5°10. Diabetes impacts minorities dispropartionateiy, i.e., 11.4% far African Americans, and 9.3% for Hispanic Americans. Based on the cost methodo[ogy de�eloped by the University of Texas {"Direct and Indirect Costs of Diabetes in Texa�", 1992, by Dr. Da�id Warner, LBJ School of Public Affairs, et al.}, it is estimated that the annual cost of diabetes in Texas is approximately $4 billion. Using this same cost methodalogy, the financial burden of diabefes annually for the City of Fort Wo�th is estimated ta be $102,�68,Q00. Yet, we continue to spend hea[th care dallars fln dawnstream medical technology and treatment, rather than focusing on preveniion and interventian. Type 2 Diabetes may be pr�ventable through proper nuirition and physical activity. If is estima#e� that 50% of the blindness and 60% of the amputations are preventable. E�en with new medications and management methodology, diabetes is still the leading cause af blindness, kidney disease, and amputations in T�xas, ma�cing it on� of our mast serious, costly, and common public health threats. The main objectives of this Diabetes Awareness and Education in the Comm�nity (�AEC) continuation gran# is to cantinue #he develapment and impl�mentation af a community project that r�duces the hurder� of diabet�s, increases the understanding of diabetes, and builds cammunity and organizationai capacity. It is community focused and pre�ention oriented. It has achieved many objectives in the past four years. In this, the final year of the grant, another survey will be conducted to determine �h� effectiveness af the pragram in raising the level of knowledge and awareness in the commur�ity. A few of the activities accomplished by �he program in the years following the survey ha�re b��n: a) 84 community and carparate healih fairs reaching 42,000 eitizens; and b) 'k 17 educational presenfat9ons in the community reaching 1,385 citizens; and c} Bus bench and radio campaigns as follaws; + A#atal of 75 bus benches, 360 inte�ior bus placards, and 15 bus tails with over 5a million exposures in three years; and � Over 124 targeted radio spots and 80 public service announcements with over 1 millivn expos�res; and d) 119 restaurant surveys to gauge healthy menus; and e} Partnerships with the University of North Texas Health Science Center, the Tarrant County Health Department, the Tarrant County Diabetes Callaboration, Inc., the Fort Wor�h Independent School District, the City of Fort Wvrth Wellness Program, the Urban Inter-Tribal Center of Texas, the National Caunci! of La Raza, and athers; and f) The Cfi�ronic Disease 5elf Management Program (CDSM) The CDSM program was designed by Stanford University. If is a classroom-based six wee� program designed to empower people ta self manage t�eir chronic diseases. Alihaugh the program is not disease speci�ic, participants will gain the skills necessary to deal wifh the emotianal, medical, and physical aspects ofi their disease. Stanford University �as shown that thraugh fhis program, people with chronic �iseases have fewer incidences of hospitalization and report better Ee�els of health. In two years, 157 peopfe have gone through the program.
