SECTION L SCOPE OF WORK:
The goal oi RECEIVlNG AGENCY P�ragn'ann is to pre�ent, cantrol, a�nd eliminate �aceine-
pz'eventable diseases by pro�iding and administering biologicals, promoting immunizations,
conducting vaccine-preventable disease surveillance, assessing vaccine coverag� le�els, and
appIying principles of epidemiology and autbreak controI measures within budgetary canstraints.
PERFORMING AGENCY shall irnplement an immunizaiion program %r chi�dren, adoIescents,
and adults, with special emphasis on accelcrating interventions to improve the immunizatian
coverage of children two years of age ar yaunger (less than 3b months of ag�). PERFORMING
AGENCY sha11 incarporat� traditional and non-traditional, systematic approaches designed to
elirninate barriers, expand immunization delivery, and establish unfform policies.
PERFQRMiNG AGENCY shall provide services as autlined in PERFORMING AGENCY'S
Fiscal Year {FY} 2003 Contract Renewal Budget and Work Plar� and revisions, if any, as agreed
to and approved by RECEIVING AGENCY Program. These docurrtents are adopted by
reference and made a part of this cont�act Attachrnent. Anq re�isions to these documents shall
be approved by RECEIVING AGENCY Pragram and transmitted in writing to PERFORMING
AGENCY.
PERFORMING AGENCY shall report al1 reportable candi�ions as specified in 2S TAC Part I
§§97.1 - 97.6 and 97.1d1 - 97.102.
PERFORMING AGENCY shall report aIl vaccine adverse event occurrences in accordance with
the Natianal Childhood Vaccine Injury Act of 19$5.
PERFORMING AGENCY shaIl inform and educate the public about vaccines and vaccine-
preventable diseases.
PERFORMTNG AGENCY shaIi develop policies and pla�s t�at s�tppart individual and
community-based immunization st�ategies and evaluate effectiveness, accessibility, and quality
of personal and population-based immunizarian services and program activities.

PERFORMING AGENCY shaIl ensure a health care workforce that is knowledgeable about
vaccines, vaccine-preventable diseases, and deiivery af vaccinatian services.
PERFORNIING AGENCY shall comply with written policies and procedures provided by
RECEIVING AGENCY Pragra3n in managing state-supplied vaccines, including guidelmes for
proper storage and handling of vaccines.
PERFORMING AGENCY sha11 not deny �accinations to recipients because they do not reside
within PERFORMING AGENCY'S jurisdictian.
PERFORNIING AGENCY shall maintain an accura�e, up-io-date list of cliaics and sites where
public sector (free or low cost} immunization services ar� offered in �PERFORMING
AC'rENCY'S local area.
No fee may be charged fox vaccines provided by RECE�VING AGENCY Program. All vaccines
abtained fram RECEPJING AGENCY Program. shall be used solely for pazposes of this contract
Attachment azad shall not be sold to agencies or individuals.
PERFORMIlVG AGENCY sha11 not collect vaccine adrninistration fees from Medicaid
recipients. Vaccine actm:inistration fees coIlect�d fram non-Medicaid patients shall be kept within
guidelinas esta�lished by RECETVIlVG AGENCY. In aecardance with 25 TAC §1.91, no one
shall be denied immunization services in public eIinics because of inability to pay the
administration fee. Fee schedules shall not be based on vaccine type, formulatian, or dose in
series. A copy o� PERFORMING AGENCY'S fee schedule shall b� submitted to RECEIVING
AGENCY Program by October 15th.
All equipment and vaccine used by PERFORMING AGENCY which are pravided by
RECEIVING AGENCY Program, s1�alI be accounted for as public prope:rty. RECEIVING
AGENCY Program wiIl inves�agaie equipment or vaccine lass, destructian, spoilage, or other
waste and may requiz'e PERFORMING AGENCY to replace or reimburse RECEIV�NG
AGENCY Pragram.
PERFORMING AGENCY sha11 provide the parent, managing conservator, or guardian of cach
patient with a form that is approved by RECEIVING AGENCY Program in eompliance with 25
TAC, Chaptez' lOQ to autharize participation in the statewide imrnunizatian registry (IrnmTrac).
PERFORMING AGENCY shall pro�ide RECEIVING AGENGY Program weekly c�ata transfers
of all vaccines administered in a fvrmat that is compatible for inclusion in the statewide
immunization registry (ImmTrac).
PERFORMIlrTG AGENCY shall comply with aIi applicable federal and stat� laws, rules,
regulat2ons, standards, and guidelines in e�ect on the baginning date af this contract Attachment

Witiun thirty {30) days of receipt of an amended standard{s} ar guideline(s), PERFORMING
AGENCY shall inform RECETVING AGENCY Program, in writing, if it will not continue
performance under this Attaehm�nt in compliance with the amended standard(s} or guideline(s).
RECEIVING AGENCY rnay terminate the Attachm�;nt immediately or within a reasonable
period of time as determined by RECEIVING AGENCY.
PERFORMANCE MEASURES
The following performance measure(s) will be used, in part, ta assess PERFORNIING
AGENCY'S effectiveness in providing the services described in this contract AttachmenC to the
service area, without waiving the enforceability of any of the tcrms af the contract.
PERFORMING AGENCY shall investigate l0a% of all reported vaccine-preventable
diseases in acco�rdance with RECEIVING AGENCY'S Vaccine-Preventable Disease
Surveillance Guidelines.
PERFORMING AGENCY shall investigate 100% of suspected l�epatitis B infections in
pregnant women to assure appropriate treatment and follow-up as directed by Health and

Safety Code §81.090 and RECEIVING AGENCY'S Vaccine-Preventable Disease
Surveillanc� Guidelines.
PERFORMiNG AGENCY shall submit to RECEZVING AGENCY Program the manthly
reports of dases administered by the fifteenth caIendar day of each rnonth for the previo�s
calendar month, in a format approved by RECEZVING AGENCY Program. The monthly
vaccine biologicals order farm must bc submitted in accordance with the schedule
provided to PERFORMING AGENCY by RE�EIVLNG AGENCY Program.
PERFORMING AGENCY �haIl i�np�ement an immunization reminde� andlor recall
system to notify parents or guardians of children Iess than 36 months of age when
immunizations are due. The notifications may be automated or manual and may consist
of mail or telephone contacts.
PERFQRMING AGENCY shall conduct irnmunization audits in chiIdcare facilities and
registered family homes as assigned by RECENING AGENCY Program.
PERFORMING AGENCY shall conduct immunization audits and surveys in public and
private schools as assigned by RECETVING AGENCY.
PERFORNIING AGENCY shall conduct annual assessments in 100% of sub-contracted
en�ities and WIC clinics using �he Assessment, Feedback, Incent�ves, and eXchange
(AFIX} methodology, PERFORMING AGENCY sha11 submit assessment results ta
RECEIVING AGENCY Program within two (2) weeks after completion.
PERFORMING AGENC� shatl conduct foliow-up monitoring visits of Texas Vaccines
iar C�ildren Progr�m (TVFC)-enrolled provider sit�s using the RECEIVING
AGENCY'S manitoring tool and met}aodology. PERFORMIlVG AGENCY shall submit
rnonitoring report to RECEIVING AGENCY Pragram within two (2) weeks after
completion of visit.
PERFORMZNG AGENCY shall recrui� and enrall providers into the TVFC program.

O�ertime Compensation Article is not applicable. Tn addition, PERFORMING AGENCY shall
comply with the foIlowing paragraphs:
PERFORMING AGENCY is authorized to pay employees who are not exempt under the
Fair Labor 5tandat'ds Act (FL�A), 29 USC, Chapter $, §201 et seq., for o�ertime ar
compensatory time at the rate of time and one-half per FLSA.
PERFORMING AGENCY is author�zed to pay employees who are �xempt under FLSA
on a straighi time basis for work perfornaed on a holiday or iar reguIar cornpensatory
time hours when the taking of regular campensatory time off would be disruptive to
normal business operations.
Autharization for payment under this provision is limited to work directly related to
immunization ac�iviti�s and shall be xn accordance �vith the amount budgeted in this
contract Attachrnent. PERFOR�/IING AGENCY shall document proper authorization or
approval for any work pez%rmed by exempt or non-exempt employees in excess of forty
(4Q) hours per work week.

It is recommended that the City Cauncil:
1. Authorize the City Manager to acce�t and execute a continuation to a grant contract with the Texas
Departmen� af Health for $� OO,ODO to be used to �increase immunization [e�els in pre-school aged i
children and pro�ide awareness and education ai the community level from S�ptem�er 1, 20�2, ta
August 31, 2003; and
2. Adopt the attached appro�riation ordinance increasing appropriations and estimated receipts by
$� O�,ODQ in the Granis Fund; and
3. WaiWe indirect costs of 35.97°l0 of reiated salary and frirrge benefits, which are the most rec�nt rat�s
as indicated by tY�e Finance Department; and
4. Authorize a r�on-interest bearing inter-fund {oan from ihe Gen�ral Fund to the Grants F�nd in the
amoun# of �$25,000 far interim financing for this grant p�nding contra�t execution to ensure a smoc�th
transit�on from ane grant fundir�g perEod to another.
DISCUSSI�N:
Qn August 9, 2QQ9 {I111&C G18705, �rdinance Na. 1�4742), the City Gouncil authorized a grant contract
with tf�e Texas Depar�men# of Health ta prorride immunization ou�treach to the commur�ity. Texas
immunization coverage I���Is for two-year old ch�ldren differ profoundly from �he national le�el. The
Texas coverage for two-year �[ds r�ached a plateau in 1996 at 74%. In 1998, immunizatEon levels far
Texas two-year ofds wer� seven percentage points be[ow the national level (81%}.
The fun�amental reason for under-immunization are those factors related to paverty. Removing
financial and access barriers can overcome these factars. T�xas paren#s orrerwhelmingly believe in the
importance of immunizations for their c�ildren, yet may not cansider t�� importance af timing, or may
nof know when imm�nizations are due. Variaus facus groups reuealed parental confusion about the
scheduEe, the need far numeraus shots, and the low awareness about immunization services.
Th� goal ofi this proposal is t� utilize immunizatian funds to acf�ie�e and maintain 9Q% vaccine
coverage far DTaP, PCV-7, [PV, MMR, HepB, Hib, and Varicella vaccines in a targ�t popu[ation of
pre�ch�al age childre� �#hose under 5 years of age).

OUTREACH TD THE C4MMUNITY
Ti�e City's Public Hea[th Depa�tment, Outr�ach Division is proposing a continued partnersF�ip with the
Fire D�partme�t to immunize more preschool age children. This partnership with emerge�cy personnel
makes sense for c�ur community. The personnel are medically trained, know thei� neighborhoods, and
these sta#ions are "safe havens" in the community. The statians are easiiy accessible, American
Disabilities Act compliant, and appealing to yaung children. Enlisting firefighters and param�dics to
became immunization partners creates an innovative collaboration in praviding needed services to the
communify. 7here will be no charge for th� immunizations.
As part of the advocacy role, outreach staff is extremely aware ofi the lack o� acc�ss to health services.
This is especially acute ir� the area of immunizatiot�. The targ�t group of infants and preschoolers in the
City is caught in this lack of access. An event in J�ne 20D0 provided immunizations to 259 children. Of
thaf total, 47°lo were 2 t� 4 years of ag�, infierring that these infants and toddlers are not getting
immunizations regularly from a primary care pr�vider. According �o th� 1998 Community Needs
Assessment, 21.9°14 of chi[dren less thar� 5 years of age had no insurance.
The program is currently operating in two fire stations locat�d in differe�t geographical quadrahts o� the
City. Ti�ese locations are close to "pockets of neecf". The locations are: Station 1 a, 31 a0 Azle Avenue,
7fi'I�6, and Station 17, 212 East Felix Street, 76115. Services are offered twa times a month for three
hours per station, total�ng twel�e hours per month. In six months, this program has served �[44 ehsldren
under the age of 5 years, and 47 v�rho were 6 years of age ar older; �19 immunizations were
administered. Accvrding to tY�e 20Qa US Census 6ureau, 5,14� children under the age of 5 resided in
zip cade 761 a6, and 228 resided in zip code 7f 915.
Training wi!! be #acilitafed by Public Health Dutreach nursing staff (�sing Texas Department of Heafth
guidelines) and the Emergency Pnysicians Advisary Board's standing pratocol from the Fire
�epartment. Paramedics vtiril! administer immurtizativns during the evening and weekenci ha�rs.
The program coordinatar and Outreach staff will distribute infarmation about the availability of serrrices
to recreation centers, apartments, neighborhood day-care centers, and faith-based organizatior�s ta
assure #he pubfic is in#ormed of the immunizatifln services.� �ata collection will be canduc4ed using the
Clinic Assessment Software Applicatior� Program. Data will be downloaded to the Texas Department of
Health's "ImmTrac" computer sy�tem. Ta ensure compliance, a reminderlrecafl system i�as �een
developed where a reminder postcard is maiied to the child's parant/s infarming them of #heir next
scheduled immunizationls. Ta motivate the parents fo cansistently camply with the immunization
schedule, various incentives are gi�en to them such as T�s�irts, stickers, safety pops, and first aid kits.
T�e waiver of indirect casts is requested in arder to maximize the eff�ctiveness of the program.
