TO CITY SECRET ARY CONTRACT NO. 58713
STORMWATER FACILITYMAINTENANCE AGREEMENT
This AMENDMENT TO CITY SECRETARY CONTRACT NO., STORMWATER
FACILITY MAINTENANCE AGREEMENT is made and entered into by and between the
CITY OF FORT WORTH, a municipal corporation of Tarrant County, Texas, acting herein by
and through its duly authorized City Manager or duly designated Assistant City Manager,
hereinafter referred to as the "City", and South Freeway Fort Worth Owner, LP "Landowner",
acting herein by and through its duly authorized representative Chad Parrish as Member.
RECITALS
WHEREAS, the City of Fort Worth ("City") and South Freeway Fort Worth Owner, LP
("Landowner"), made and entered into City Secretary Contract No. 58713 (the "Agreement") to
authorize the use of on-site stormwater management facilities (the "Facility") to be constructed and
maintained on a portion of Landowner's property; and
WHEREAS, the Agreement was recorded with the property records of the Tarrant County as
document No. D223007579 which is one and the same as City Secretary Contract 58713.
WHEREAS, the Agreement contains exhibit(s) "A" and "B" that depicts the location and
design specifications of the Facility; and
WHEREAS, it advisable to execute this Amendment to the Agreement to revise exhibit(s)
"A" and "B" in the Agreement to depict changes location and design changes being added to the
original agreement; and
NOW THEREFORE, City and Landowner, acting herein by and through their duly
authorized representatives, enter into the following that amends the Agreement:

Exhibit A is hereby replaced with Exhibit A-1 attached hereto and incorporated herein to the
Agreement, consisting of a revised location description of the Facility.
2.
E�iibit B is hereby replaced with Exhibit B-1 attached hereto and incorporated herein to the
Agreement, consisting of revised design specifications of the Facility.
3.
This Amendment shall be filed with the City Secretary as an amendment to the Agreement
and also recorded among the land records of Tarrant County, Texas, and shall constitute a covenant
running with the land, and shall be binding on the Landowner, its administrators, executors, assigns,
heirs and any other successors in interests, including any property owners association.
4.
All other provisions of the Agreement which are not expressly amended herein shall remain
in full force and effect.

CITY OF FORT WORTH STORMWATER FACILITY
OPERATION AND MAINTENANCE PLAN
The only responsibility the Ciry of Fort Worth has in the operation and maintenance of
this Facility is inspection.
General Maintenance Procedures
The structural and funetional integrity of the Facility shall be maintained at all times by
reinoving and preventing drainage interference, obstructions, blockages, or other adverse
effects into, through, or out of the system.
Periodic silt removal shall occur when standing water conditions occur or the pond's
storage volume is reduced by more than 10%. Silt shall be removed and the pondlbasin
returned to original lines and grades shown on the approved engineering plans. In
addition, coirective measures are required any time a basin does not drain completely
within 72 hours of cessation of inflow. NO STANDING WAT�R IS ALLOWED in
basins designed for dry detention purposes.
Accumulated litter, sediment, and debris shall be removed every 6 months or as necessary
to maintain proper operation of the basin. Disposal shall be in accordance with federal,
state and local regulations.
Detention facilities shall be mowed monthly between the months of April and October or
anytirr►e vegelation exceeds 12-inches in height.
To prevent debris froin entering and clogging the downstream storm sewer system a wire
mesh screen or similar screening device shall be installed over the outlet until final
acceptance.
4. PREVENTIVE MAINTENANCE/INSPECTION
Visual inspections of all components will be conducted every 6 months.
A log shall be kept of maintetiance actions, and inspections. The log should
docuinent the condition of the detei�tion system's primary coinponents, mowing,
and silt, litter and debris rcmoval da:es. Document aeration of the basin bottoms
and replanting to prevent the sealing of the basin bottom.
Written maintenance and repair records shall be maintained by the party or
parties signing the attached Agreement and shall be provided to the City upon
request.

INTRODUCTION
Stormwater inspection and maintenance requirements are provided in accordance with City
of Fort Worth Stormwater Criteria Manual dated September 29, 2015. This report is
prepared to address the maintenance component of the herein described development to
verify the effective, efficient, and enduring service of a particular stormwater measure. This
plan contains preventative and corrective maintenance tasks and procedures.
II. PROJECT DESCRIPTION
Existing Conditions
The McPherson Boulevard Development is a 16.67-acre site bound by South Freeway to
the east, an auto body shop to the north, a residential neighborhood to the west and an
open area to the south. The site is located outside of the 100-year FEMA floodplain. The
existing site is vacant. The site currently slopes from north to south and most of the
drainage discharges via sheet flow to the property to the south. A smaller portion of the
drainage discharges into the TXDOT right-of-way (ROW) in South Freeway. Additionally,
there is some upstream flow from the adjacent properties to the north and west that
passes through the site. Overall, the combined on-site and off-site drainage areas total
23.94 acres.
Proposed Conditions
The proposed conditions of the site will include a 259,250-square foot warehouse facility.
The site improvements will include two new entrances off of the South Freeway. There will
be vehicular parking on the east side of the building and a truck court with tractor-trailer
parking on the west side of the building and along the proposed McPherson Blvd access
driveway entrance.
III. STORMWATER MAINTENANCE OBJECTIVE
The stormwater system proposed for this development is intended to treat, attenuate and
convey the stormwater from the development. This maintenance plan is prepared to verify
the systems in place are operating efficiently and reliably. The Owner shall verify the long-
term/perpetual operation, maintenance, repair, and safety of the stormwater management
facilities.
Maintenance procedures are required to maintain the intended operation and safe condition
of the stormwater management facility by reducing the occurrence of problems and
malfunctions. To be effective, maintenance shall be performed on a regular basis and
include such routine procedures as training of staff, periodic inspections, silt and debris
removal and disposal, control of mosquitoes and other insects, and review of maintenance
and inspection work to identify where the maintenance program could be more effective.

management facility and to restore the facility's intended operation and safe condition.
Based upon the severity of the problem, repairs shall be performed on an as-needed or
emergency basis and may include such procedures as structural repairs, mosquito control,
removal of debris, sediment and trash which threaten discharge capacity, erosion repair,
and restoration of vegetation.
IV. CONSTRUCTED STORMWATER MANAGEMENT SYSTEM ELEMENTS
The on-site stormwater management system a series of subsurface conveyance systems
comprised of precast inlets, manhole structures, drainage pipes, which convey stormwater
runoff to and from the stormwater management BMPs and ultimately to the proposed off-
site discharge points.
Maintenance of Conveyance Systems
The proposed conveyance systems have adequate access for inspection and/or
maintenance. The use of the proposed conveyance systems is necessary to manage
runoff.
All conveyances systems including inlets, manholes and pipes are expected to receive
and/or accumulate debris and sediment. These systems must be inspected for clogging
and excessive debris and sediment accumulation at least annually as well as after every
storm exceeding 2 inches of rainfall. Sediment removal should take place when all runoff
has drained from the conveyance network and the systems are reasonably dry. Disposal of
debris, trash, sediment, and other waste material should be done at suitable
disposal/recycling sites and in compliance with all applicable local, state, and federal waste
regulations.
All structural components must be inspected for cracking, subsidence, breaching, wearing,
and deterioration annually. The condition of surrounding and above lying materials shall be
inspected for evidence of potential failures or deterioration.
The routine equipment expected to be utilized for the maintenance tasks may include a jet
vacuum vehicle, shovels, lighting equipment and a wheel barrel or truck for the hauling off
of debris. Water, mosquito control chemicals, and concrete repair materials may also be
required depending on the condition of the structure.

Maintenance of Detention Basin
The proposed detention basin provide adequate ability for inspection and/or maintenance.
The detention basin, including all inlet and outlet pipes, manholes, outlet control structures,
and appurtenances must be inspected for clogging and excessive debris and sediment
accumulation at least annually. Sediment removal should take place when all runoff has
drained from the conveyance network and the systems are reasonably dry. Disposal of
debris, trash, sediment, and other waste material should be done at suitable
disposal/recycling sites and in compliance with all applicable local, state, and federal waste
regulations.
Mowing and/or trimming of vegetation must be performed on a regular schedule based on
specific site conditions. Grass should be mowed at least once a month during the growing
season. Vegetated areas must be inspected at least annually for erosion and scour.
Vegetated areas should also be inspected at least annually for unwanted growth, wh ich
should be removed with minimum disruption to the bottom surface and remaining
vegetation.

IV. MAINTENANCE MEASURES AND PROCEDURES
All the inspection work to be in accordance with regulations from local, state and federal
requirements. inspection and maintenance personnel should be equipped with the
following personal protective equipment (PPE):
o Fieflective Vests
o Glasses
o Hard Hats
o Flashlights
o Ear protection
Confined-space entry should not be required for any inspection or maintenance tasks. If
confined space entry is deemed necessary, inspection and maintenance personnel shou Id
be certified by Occupational Safety and Health Administration (OSHA) for confined-space
entry.
