CHANGE ORDERS to DATE (INCLUDING THIS ONE) AS % OF ORIGINAL CONTRACT 2.83% MAX ALLOWABLE CONTRACT AMOUNT (ORIGINAL CONTRACT COST + 25%) $6,769,480.00 JUSTIFICATION (R QNS) FOR CE ORDER This Change Order No. 6 provides for the following: • The delivery of additional sewer grinder pumps to sewer operations storage for future installation • Installation of a 12 inch dia. force main in Hatch Road that eventually connect to Casino Beach Lift Station. • AdditonaI water services and water service reconnections • Drains for water heaters to allow installation of thermal expansion tanks • Substitution of 6" water line in Tyler Place Rd. and Sommerville Place Rd. in lieu of 8" water lines. • Installation of 6" water line by other than open cut method. The affected locations are currently provided water by Lakeside but must be converted to Fort Worth water per the terms of a Transfer Agreement with Lakeside. I Staff recommends the addition of 180 calendar days for the additional work. It is understood and agreed that the acceptance of this Change Order by the contractor constitutes an accord and satisfaction and represents payment in full (both time and money) for all costs arising out of, or incidental to, the above Change Order.
