(No M&C Needed) Cil'y SECRKI-ARY COUNTY Off" TA_RRANT S CONTRACT NO. __,,7tX WHETS, the City of Fort Worth (City) and Wendy Lopez and Associates, Inc. dba. hOPEZGARC1'A GROUP, (Engineer) made and entered into City Secretary Contract No. 2742€3, (the Contract) which was authorized by the City Council by M&C C-18911 on the 3sd day of January 2002; and subsequently revised by a previous amendment on June 19th, 2002 WHETS, the Contract involves engineering services for the following project: Water replacement Contract 2001; WM-M. WHETS, it has become necessary to execute Amendment No. 2 to said Contract to include an increased scope of work and revised maximum fee; NOW THEREFOR, City and Engineer, acting herein by and through their duly authorized representatives, enter into the following agreement, which amends the Contract: 1. Article I of the Contract is amended to include the additional engineering services specified in 'a proposal letter dated October 24, 2002, a copy of which is attached hereto and incorporated herein. The cost to City for the additional design services to be performed by Engineer total $5,935.90 2. Article II of the Contract is amended to provide for an increase in the maximum fee to be paid to Engineer for all work and services performed under the Contract, as amended, so that the total fee paid by the City for all work and services shall not exceed the sum of $ 71,536.90. 3. All other provisions of the Contract which are not expressly mended herein shall remain in full force and effect. Project Manager City of Fort Worth Department of Engineering 900 Monroe Street, 2nd Floor Fort Worth, Texas 76102 RE: Request for Amendment No. 2 Water Replacement Contract 2001, WM-M Project LGG Project No. 220008.00 Dear Mr. Chaubey: This letter is in response to your request for additional engineering design services. A detailed breakdown and summary of estimated fees are attached. A description of the additional work is as follows: The final plans along with application were submitted to Texas Department of Transportation (TXDOT) for TXDOT permit. The proposed waterline and sewer line crosses Belknap Street (US 377) at Gwynne Street intersection. ,Since the water and sewer lines crosses TXDOT right- of-way, TXDOT permit is required. Meeting was held between staff of LOPEZGARCIA. GROUP and TXDOT regarding the requirements for the issuance of the permit. The sanitary sewer line and waterline construction plans for Gwynne Street needs to be revised to meet the TXDOT criteria. Also, as per the City of Fort Worth, Water Department, Bird Street and Gwynn Street has to be taken out of the project. The changes in plans will be incorporated into the 90% plan submittal. Our engineering fee estimate for this work includes the following: • Prepare two (2) Traffic Control plan sheets per TXDOT standard. • Prepare one (1) sheet of construction plan based on TXDOT comments showing the re- alignment of proposed sanitary sewer line. • Revise waterline plan to meet TXDOT requirement. • Take out Bird and Gwynne Street from the plan set, and advertise separately. lopugardagrouptom 6850 Manhattan Blvd., Suite 300 Fort Worth,%xas 76120 (817) 446.1800 (817) 446-1113 Fax Mr. Rakesh Chaubey, P.E. City of Fort Worth October 24, 2002 Page Two The additional efforts are considered outside our original scope of work, and we therefore request a contract amendment to increase the lump stun fee amount by $5,935.90 (See attached LGGroup fee estimate).
