Fort Worth ("City"), a home-rule municipal corporation situated in Tarrant, Denton and
Wise counties, Texas, acting by and through its duly authorized Assistant City Manager,
and Concentra Health Services, Inc., a Nevada corporation, d/b/a/ Concentra Medical
Centers ("Concentra").
WITNESSETH:
WHEREAS, City and Concentra have entered into a certain Contract for
Professional Services, ("the Agreement") effective as of November 9, 2004 ("Effective
Date"), same being City of Fort Worth City Secretary Contract No. 30880; and
NOW, THEREFORE, in consideration of the foregoing and mutual covenants and
agreements contained herein and for other good and valuable consideration, the receipt
and sufficiency of which are hereby acknowledged, City and Concentra hereby agree to
amend the Agreement as follows:
Section 1.
Appendix "B" — Schedule for Compensation, attached to City Secretary Contract
No. 30880 is hereby amended to include the following Services and Pricing Units:

*88% of TWCCFG until the abolishment of the TWCCFG and implementation of
networks at which time the discount rate may be negotiated directly between the City
or its third party administrator and Concentra
Year One and Year Two costs as set forth in this amendment include any and all
injury care fees due to the City. The City's third party administrator passes on a 2%
discount to the City, while Concentra discounts the remaining percentage to arrive at
the negotiated rate as stated above.

Section 2.
All other terms and conditions of City of Fort Worth Contract No. 30880 that
are not amended herein continue in full force and effect.

RECOMMENDATION:
It is recommended that the City Council authorize the City Manager to exercise the first of four one year
options to nanexm the agreement with C|onoantna Medical Centers (Connentna) beginning October 1' 2005
and expiring September 30, 2008.
DISCUSSION:
On September 14' 2004 (M&C the City Council authorized the City Manager to execute a contract
with Concent[a K8adioa| Centers to provide occupational health oona services for the City of Fort Worth. The
City's current contract with Concentna which began October 1. 2004 and expires September 30. 2005.
includes four one-year options for renewal.
Concentna is one of the largest occupational health oans providers in the notion. There one three clinics in
Fort Worth and 14 clinics in the K8atrop|eX. /\ majority of the een/ioea provided by Conmantro under this
contract include: medical treatment for on-the-job injuries, drug and alcohol testing and counee|ing, pre-
employment physicals and vaccinations.
Conmentra'a services are priced on o per unit basis and the medical treatment for on the job injuries is
discounted 17 percent lower than the Texas Workers' Compensation fee guideline. It is anticipated that the
cost for all een/imae during FY 2005-06 will be approximately $817'851.00.
M/VVBE - Conoentno is in compliance with the City's K8/VVBE Ordinance by committing to 1096 K8/VVBE
participation rnodioa| services for occupational injuries. The Qh/'a goal on this project is 1096.

The Finance Director certifies that funds are omai|ob|e, as appropriated in the FY 2005-06 General and
Workers' Compensation Funds adopted Budget.
