HomeMy WebLinkAboutContract 35495-A3 AMENDMENT NO. 3 STATE OF TEXAS S CITY SECRETARY CONTRACT NO. rj5gq} j-Raj (No M&C Needed) COUNTY OF TARRANT S WHEREAS, the City of Fort Worth (City) and Stream Water Group, Inc. , (Engineer) made and entered into City Secretary Contract No. 35495, (the Contract) which was authorized by the City Council by M&C C-22179 on the 12th day of June 2007; and WHEREAS, the Contract involves engineering services for the following project: Storm Water Utility Project - Loving Avenue and Channel Improvements, Project No. 00626; WHEREAS, it has become necessary to execute Amendment No. 3 to said Contract to include an increased scope of work and revised maximum fee; NOW THEREFORE, City and Engineer, acting herein by and through their duly authorized representatives, enter into the following agreement which amends the Contract: 1 . Article I of the Contract is amended to include the additional engineering services specified in a proposal letter dated April 14, 2011, a copy of which is attached hereto and incorporated herein. The cost to City for the additional design services to be performed by Engineer total $44,385. 2 . Amendment No.3: Revise Model for Design Change along 30th St due to proposed street closure by City of Fort Worth Revise Drainage Improvements Design Sheets and add design for 3e St closure at Prairie Ave(cul-de-sac)as directed by the City ❖ Design for Approximately 150 LF of 15"SS, 620 IF of 12"SS,185 IF of 10"SS, 65 LF of 8"SS, and 130 IF of 6" WL Relocation due to conflicts with Drainage Improvements.(Approximately 4 sheets Plan/Proftle, 1 Sewer Abandonment/Relocation Sheet, 1 cover sheet, I general notes and legend sheet, 1 layout and control sheet, 2 standard detail sheets) ❖ Additional survey for cul-de-sec and Sanitary Sewer Line design The street closure(30`h St at Prairie Ave)will reduce the construction cost,reduces the impact on environment due to construction and also meet the requirements of U.S.Army Corp of Engineers.
