Background
On February 28, 1984 (M&C G-5912) , the City Council approved the
acceptance of a health education/risk reduction grant from the Texas
.Department of Health in the amount of $38,000 for the period March 1,
1984 through September 30, 1984. Subsequently, on October 2, 1984 (M&C
G-6131) , the City Council approved the renewal of this grant in the
amount of $38,000 for FY 1985. The previous M&Cs are attached for your
information.
The contract requirements of the health education/risk reduction grant ,
entitled "Choices for Life", during this 19-month period were to develop
and offer a worksite health education/risk reduction program to small
and medium-sized companies in Fort Worth by meeting the following
specified objectives:
Objective Completion Date
1. Carry out a telephone health risk factor survey July, 1984
on 300 randomly selected individuals 18 and older
tn the Fort Worth area.
2. Develop course curricula for four core classes
covering lifestyle and risk factors; exercise;
nutrition; and stress management. Components of
curricula include:
4 Leader' s Guides - One on each core topic December, 1984
Handouts - as appropriate for each core topic January, 1985
4 Slide/tape programs - one on each core topic To be completed
by September
1985
3. Develop a promotional packet on the "Choices for March, 1985
Life" program for use in presentation to potential
participating companies.
4. Pilot testing of "Choices for Life" with two com- June, 1985
panies with 16 to 100 employees in Fort Worth.

5. Offer "Choices for Life" to 300 small and medium May, 1985
sized companies within the 76107 zip code area.
6. Provide "Choices for Life" to 15 small and medium In progress
sized companies (including 2 pilots) within Fiscal to September
Year '85. 1985
To meet the requirements specified in the grant , a comprehensive
reproducible worksite health promotion package that can be utilized by
health departments throughout the State of Texas must be produced.
All of the objectives listed above, with the exception of the slide/tape
programs, have been met or are in progress and will be completed by
September 30, 1985. Fulfillment of the contract with the State is
contingent upon completion of all the stated objectives, including the
slide/tape programs.
No additional funding is being provided- by the Texas Department of
Health or the City of Fort Worth for-this program activity in FY 85-86.
The recommendation submitted to City Council on June 18, 1985 (M&C
P-1771, p. 1) pertains to the production of the slide/tape program.
Approval of the purchase will assure the fulfillment of the health
education/risk reduction contract with the Texas Department of Health by
completing the "Choices for Life" education packet. If it is the
Council 's decision not to authorize this purchase, these funds will be
returned to the Texas Department of Health and the City will have
defaulted on the contract.
