HomeMy WebLinkAboutContract 54492-A5DocuSign Envelope ID: BFABDB78-D102-4595-8DE2-COOCCDFDF4FA CSC No. 54492-A5 FIFTH AMENDMENT TO CITY OF FORT WORTH CONTRACT 54492 BETWEEN THE CITY OF FORT WORTH AND MY HEALTH, MY RESOURCES OF TARRANT COUNTY FOR MENTAL HEALTH SERVICES This fifth amendment (the "Amendment") is made and entered into by and between the City of Fort Worth, a home -rule municipality of the State of Texas (hereinafter referred to as the "City"), acting by and through Fernando Costa, its duly authorized Assistant City Manager, and MY HEALTH, MY RESOURCES OF TARRANT COUNTY, a unit of local government in Texas ("Agency"). Each party shall be individually referred to herein as Party and collectively as Parties. RECITALS WHEREAS, the City entered into an Agreement with Agency to provide services more specifically described in the agreement, City Secretary Contract No. 54492, (the "Agreement"); WHEREAS, the Parties executed Amendment 1 and Renewal 1 to the Agreement through which the Parties agreed to increase the budget, amend the Reporting Forms and the Budget Modification Form and renew the Agreement for an additional year; WHEREAS, the Parties executed Amendment 2 through which the Parties agreed to amend the Agreement to expand the definition of resident services in the budget; WHEREAS, the Parties executed the Amendment 3 and Renewal 2 to the Agreement through which the Parties agreed to renew the Agreement and amend the Agreement to change memorialize prior budget modifications and amend the scope of services; WHEREAS, the Parties executed the Amendment 4 and Renewal 3 to the Agreement through which the Parties agreed to renew the Agreement and amend the Agreement to memorialize a prior budget modification and amend the budget; WHEREAS, the Parties desire to amend the scope of services to provide mental health services to clients of the new high impact pilot program which focuses on providing services and housing to those experiencing homelessness in seven target areas of the city; WHEREAS, this Amendment will amend the scope of services. AMENDMENT TO AGREEMENT NOW THEREFORE, for valuable consideration, the receipt and sufficiency of which are hereby acknowledged, the Parties agree as follows: A. The Scope of Services attached as EXHIBIT "A" to the Agreement is hereby deleted and replaced in its entirety with the scope of services consisting of three (3) pages attached hereto. B. All terms and conditions of the Agreement not amended herein remain unaffected and in full force and effect, are binding on the Parties and are hereby ratified by the Parties. Capitalized terms not defined herein shall have the meanings assigned to them in the Agreement. WORTH, TX DocuSign Envelope ID: B FAB DB78-D 1 02-4595-8DE2-COOCCDFDF4FA EXHIBIT "A" SCOPE OF SERVICES MENTAL HEALTH SERVICES MY HEALTH, MY RESOURCES OF TARRANT COUNTY (MHMR) will do the following: Employ one (1) full time case manager and one (1) part time program manager to provide mental health services as needed and accepted to 375 Directions Home permanent supportive housing clients to do the following: A. In General • Document the nature and extent of all services provided to chronically homeless individuals in the City of Fort Worth receiving mental health services hereunder (client") in a complete case file, with case notes in the HMIS system within 3 business days of a service being provided hereunder; Eligible clients are those currently on the Directions Home permanent supportive housing program and those who are chronically homeless in the City of Fort Worth and referred by Tarrant County Homeless Coalition from the Coordinated Entry list for permanent supportive housing and those households in the high impact pilot program; B. Mental Health Services • Maintain a priority mental health case load of highly vulnerable Directions Home permanent supportive housing clients; • Vulnerability score on assessment will determine placement on priority mental health case load; • Receive referrals based on assessment scores from permanent supportive housing agencies contracted by Directions Home for permanent supportive housing; • Also receive direct referrals from Directions Home permanent supportive housing case managers to the priority mental health case load based on observation; Page 4 of 6 Amendment 5 to CSC 54492 CoFW and MY HEALTH, MY RESOURCES OF TARRANT COUNTY DocuSign Envelope ID: BFABDB78-D102-4595-8DE2-COOCCDFDF4FA • The case load will not exceed 20 clients at any given time during the contract year; • Meet with clients on high priority mental case load to provide (as appropriate): o Referrals to resources o Counseling o Goal setting o Harm reduction planning • Determine frequency of meetings with clients based on assessment score, permanent supportive housing case manager observation and mental health services evaluation • Provide four group meetings a week throughout the Fort Worth specifically for Directions Home permanent supportive housing clients; • h1 group meetings, facilitate discussion and provide coaching/counseling regarding (as appropriate): o Life skills o Harm reduction o Socialization o Use of resources o Addiction o Depression, Bipolar, Schizophrenia etc. o Hoarding o Relationship issues o Community issues • Submit on time quarterly reports detailing progress on meeting targets: • Monthly, document clients participating in group meetings; • Monthly, send a list of clients on priority mental health caseload and include date assigned to priority case load, date of exit from case load and date of follow up screening Evaluation: Evaluation meetings will be held with Directions Home staff to continually evaluate program and Agency shall comply as necessary and in good faith. Financial reporting: Reimbursement Request and any necessary supporting documentation and reports will be submitted by the 15t1i of every month in format of Exhibit "C". DISCUSSION: The City Manager's Directions Horne Division approached the Purchasing Division to issue a Request for Proposals (RFP) seeking Firms to: 1) increase or maintain permanent supportive housing; 2) quickly house people in permanent housing; or 3) Improve overall system capacity. The RFP consisted of specifications explaining the need for various housing programs and overall improvement of the homeless system. Proposals were scored based on the strength of the proposed program, program performance, outcomes and quality, organizational capacity of the agency, and budget, The RFP was advertised in the Fort Worth Star Telegram on June 10, June 17, June 24, July 1 and July 8, 2020. The City closed the RFP on July 9, 2020. The City received 17 responses, Staff from the Directions Home Division and Neighborhood Services Department evaluated the proposals and found the below listed firms to be experienced and well equipped to perform the needed services, A waiver of the goal MNUBE subcontracting requirement is approved by the Office of Business Diversity, in accordance with the MNVBE or BDE ordinance, because the purchase of goods or services is from sources where subcontracting or supplier opportunities are negligible. Any renewals are contingent on funds being appropriated.
