This is a total increase of 20%. This is for an unknown condition (not reflected in the geotech report); the building pad was found to be atop a layer of limestone. This required the removal of approx 3 ft of limestone rock for entire bldg footprint, haul off of rock, haul in select fill and increase thickness of concrete building pad by 1". Change Order #2 will increase contract duration by 10 days. JUSTIFICATION (REASONS) FOR CHANGE ORDER It is understood and agreed that the acceptance of this Change Order by the contractor consitutes an accord and satisfaction and represents payment in full (both time and money) for all costs arising out of, or incindental to the above Change Order. The subgrade removes an additional 2 feet of rock, rock digging not in original plan. 2. Export additional 2 feet of building pad subgrade (Est: 135 loads, bobtails, @$195 per load) Load price includes loading, hauling, and dump fees. 3. Import additional 2’ of select fill (Est: additional 115 loads @$335 per load) Load price includes materials, place, grade and compact. 4. Two additional pieces of equipment and added time of compaction. NOTE: If we are required to demobilize again it will be billed at $10,500.00, and $10,500.00 to mobilize back on site. We will not be able to provide a timeline for remobilization. Change in paving from lime to Flex Base Type D CO Price= Credit on original contract: $8430.50 1. Subgrade in paving change to additional 1 inches Thickened paving. 2. Original lime $17946.00, added 1” cut and haul = 10 loads of extra haul off @$195 per load= $1950.00. Credit $17946.00-$1950.00=$15996.00 3. One extra day for 3 pieces of equipment to achieve the added cut and haul, $3000.00. Credit (15996.00-$3000.00) =$12996.00 4. Added survey for changes $4565.50. Demolition is based solely on civil plans and deduction of utility cuts. Structural excavation of any kind. Cushion sand & bedding gravel. Flume, sidewalk stabilization, or excavation of any kind.
