TO CITY SECRETARY CONTRACT No. 60140
WHEREAS, the City of Fort Worth (CITY) and JQ Infrastructure, LLC, (ENGINEER) made and
entered into City Secretary Contract No. 60140, (the CONTRACT) which was administratively authorized on
the 201 day of September, 2023 in the amount of $17,400.00; and
WHEREAS, the CONTRACT involves engineering services for the following project:
Devoyd Jennings Business Assistance Center at Guinn School — Structural Evaluation, CPN 105891; and
WHEREAS, it has become necessary to execute Amendment No. 1 to the CONTRACT to include an
increased scope of work and revised fee.
NOW THEREFORE, CITY and ENGINEER, acting herein by and through their duly authorized
representatives, enter into the following agreement, which amends the CONTRACT:
1.
Article I of the CONTRACT is amended to include the additional Engineering Services specified in
proposal attached hereto and incorporated herein. The cost to City for the additional design services to be
performed by Engineer totals $44,900.00. (See Attached Funding Breakdown Sheet, Page -3 of 3-)
2.
Article H of the CONTRACT is amended to provide for an increase in the fee to be paid to Engineer for
all work and services performed under the Contract, as amended, so that the total fee paid by the City for all
work and services shall be an amount of $62,300.00.
3.
All other provisions of the Contract, which are not expressly amended herein, shall remain in full force
and effect.

Dear Mr. Meza,
We are pleased to express our interest in providing structural engineering services for the referenced
project and offer the following revised proposal for your consideration.
SCOPE OF PROJECT
We understand the scope of the project to be repairs to the foundation of the Devoyd Jennings Business
Assistance Center (BAC) building, 1150 South Freeway, Fort Worth, Texas. The repairs will be based on
the recommendations contained in the structural assessment report prepared by JQ Infrastructure, LLC
dated January 5, 2024. These repairs include the following:
1. Installation of a root and moisture barrier along the north and east sides of the existing building.
2. Removal and replacement of the existing landscape hedges and turf and regrading of the site
along the north and east sides of the building to allow installation of the root and moisture barrier
and to promote drainage away from the building.
3. Review of the existing mature trees by an arborist to assist in the development of root pruning
procedures to be implemented during installation of the root and moisture barrier.
4. Repair of sections of the existing irrigation system if damaged by the installation of the root and
moisture barrier. Management or operation controllers for the existing system will not be altered.
5. Repair of cracking in the exterior multi-wythe masonry wall.
6. Repair of interior plaster finishes which will be deferred until the building foundation has been
stabilized but will be included in the repair documents.

CCA Landscape Architects, Inc. will be retained to provide landscape architecture and arborist services as
our subconsultant.
BASIC SERVICES
Our services will include the following:
1. One (1) site visit to review the site conditions.
2. Preparation of a site topographic survey with scope limited to the areas of work shown in attached
Exhibit C. The topographic survey will include:
a. Establish a minimum of two (2) temporary control monuments (typically a 5/8-inch iron rod
with yellow plastic cap stamped JQ TRAVERSE), provide coordinates (northing, easting, and
elevation) and graphically show on the topographic surveying exhibit.
b. All horizontal survey data will be surveyed relative to the North American Datum of 1983 (NAD
83), Texas State Plane Coordinate System, North Central Zone-4202 (surface coordinates).
c. Location of permanent improvements on the site.
d. Spot elevations on a 25-foot grid or break in grade for hard surface and 50-foot grid or break
in grade for natural ground.
Top of curb and gutter elevations for paving on and adjacent to the site.
Location of all sidewalks on and adjacent to the site.
Location of all buildings with finish floor elevations.
Location of visible utilities and appurtenances.
Contours on one foot intervals.
3. Preparation of construction drawings for the work. Documents will be submitted at 90% and
100% completion for review.
4. Preparation of technical specifications.
5. Preparation of addenda as required during the bidding period and answering questions regarding
structural plans and specifications.
6. Attendance at one (1) pre -bid meeting.
7. Construction administration:
a. A maximum of three (3) observations, construction administration, and/or pre -construction
meetings, including the preparation of observation reports. This includes two (2) site visits
during construction and one (1) at substantial completion.
b. Review of shop drawings and submittal data.
c. Review and respond to RFI's
d. Preparation of ASI's and change orders.
e. Review of change orders
f. Review of testing laboratory reports

Services excluded from basic services but not limited to:
1. Any scope of work not specifically stated above.
2. Design and detailing of sitework which is not in area impacted by the work, unless specifically
included above.
3. Preparation of front end documents including general and supplemental conditions and Division
1 specifications.
4. Preparation of City of Fort Worth Urban Forestry plans and calculations.
5. BIM modeling.
6. Opinions of probable construction costs.
7. Geotechnical investigations and material testing.
8. Design of temporary or permanent earth retention systems.
9. Detailing or specification of waterproofing or dampproofing. JQ may graphically indicate these
items as appropriate and designate these items as work specified elsewhere in the Construction
Documents.
10. Construction administration:
a. Additional observations or site visits beyond the number listed above.
b. Review of shoring drawings.
c. Preparation of record documents.
11. Special inspections or responsibility for special inspections as the registered design professional
in responsible charge (as defined by the International Building Code).
12. Assistance with the permitting process.
13. Testing of existing in -place materials or framing systems to determine strengths, properties of
materials or similar information.
14. Field measurements of existing structures or framing except as needed to complete the
construction documents. Existing structures or framing will be depicted in accordance with
existing documents if such documents are provided by others.
ALTERNATE SERVICES
1. Additional site visits beyond the number listed in basic services.
2. Preparation of City of Fort Worth Urban Forestry plans and calculations.

PAYMENT SCHEDULE
Invoices for progress payments of the basic fee will be billed each month for services performed during
the prior month on a percentage of completion basis in accordance with the following schedule:
