TO CITY SECRETARY CONTRACT No.
WHEREAS, the City of Fort Worth (CITY) and HarrisonKornberg
Architects, LLC (ARCHITECT) made and entered into City Secretary Contract
No. 57475,in the amount of $12,000.00 (the CONTRACT) which was authorized
administratively on the 26th day of April, 2022, for the Programming
Services for the HUB Community and Aquatics Center; and
WHEREAS, the CONTRACT was subsequently revised by: Amendment Number 1
in the amount of $80,538.00 administratively authorized on (16th day of
April). Amendment Number 2 in the amount of $2,107,431.00 authorized by
M&C 22-1061 on the 13th day of December, 2022.
WHEREAS, the CONTRACT involves architectural services for the
following project:
The HUB Community and Aquatics Centers, CPN 103855; and
WHEREAS, it has become necessary to execute Amendment No. 3 to the
CONTRACT to include an increased scope of work and revised fee.
NOW THEREFORE, CITY and ARCHITECT, acting herein by and through their
duly authorized representatives, enter into the following agreement, which
amends the CONTRACT:

Article II of the CONTRACT is amended to provide for an increase in
the fee to be paid to Architect for all work and services performed under
the Contract, as amended, so that the total fee paid by the City for all
work and services shall be $2,232,599.00.
3.
All other provisions of the Contract, which are not expressly amended
herein, shall remain in full force and effect.
EXECUTED and EFFECTIVE as of the date subscribed by the City's
designated Assistant City Manager.

We appreciate the opportunity to submit our fee proposal for the additional design work to the City of Fort
Worth Community Center & Aquatic Center (FW-HUB), which will be performed by JQ Infrastructure.

Client agrees to make interim payments for services completed per the Prime Agreement.

Dear Mr. Harrison,
We are pleased to express our interest in providing land surveying services for the referenced project and
offer the following proposal for your consideration.
SCOPE OF PROJECT
We understand the scope of the project to be a topographic survey and Platting of that called 22.2519
Acres of land situated In Tarrant County. Tax Appraisal District Account No. 00461172 located at 4903
Pollard Smith Avenue and recorded in Volume 388-Y Page 107 of the Plat Records of Tarrant County,
Texas. Currently vested with Housing Authority of Ft. Worth.
LAND SURVEYING SERVICES
(JQI) Texas Land Surveying Firm Registration No. 10193718
Our land surveying services will include the following:
1. Boundary Verification
a. Title research with Tarrant County and right-of-way information.
b. Boundary lines (including a reference or basis of bearing), giving length and bearing on each
line, radius, central angle and arc length of curved lines. Show any existing boundary
monuments found and state on the drawing.
c. Show recording information of adjacent properties or reference to lot, block numbers if
platted.
d. Boundary, lot, setback and easement lines will only be depicted in a cad file, with no signed
and sealed exhibits produced.
e. No boundary and/or easement monumentation will be established.

5. Control Survey
a. Establish two (2) on -site survey control points to serve as a base line for the survey and
provide coordinates (northing, easting, and elevation) on the topographic survey and for
construction.
b. All horizontal survey data will be surveyed relative to the North American Datum of 1983 (NAD
83), Texas State Plane Coordinate System, North Central Zone-4202.
c. All vertical survey data with be surveyed relative to the North American Vertical Datum of
1988 (NAVD 1988).
d. The survey datum will be matched to the existing datum for the facility so that all elevations
on historical and new drawings are coordinated.
e. The survey control points/benchmarks will consist of a 5/8-inch iron rod with pink plastic cap
stamped JQ DALLAS ENG., Additional services may be required to set client specific
monumentation.
6. Topographic Survey
a. Establish a minimum of two (2) temporary control monuments (typically a 5/8-inch iron rod
with yellow plastic cap stamped JQ TRAVERSE), provide coordinates (northing, easting, and
elevation) and graphically show on the topographic surveying exhibit.
b. All horizontal survey data will be surveyed relative to the North American Datum of 1983 (NAD
83), Texas State Plane Coordinate System, North Central Zone-4202 (surface coordinates).
c. All vertical survey data with be surveyed relative to the North American Vertical Datum of
1988 (NAVD 1988).
d. Location of permanent improvements on and immediately adjacent to the site.
e. Spot elevations on a 25-foot grid or break in grade for hard surface and 50-foot grid or break
in grade for natural ground.
f. Top of curb and gutter elevations for paving on and adjacent to the site.
g. Location of permanent improvements on and immediately adjacent to the site.
h. Location of all sidewalks on and adjacent to the site.
i. Location of all buildings with finish floor elevations.
j. Location of visible utilities and appurtenances.
k. Flowline elevations on wastewater manholes, storm drainage manholes and inlets.
I. Contours on one foot intervals.
7. Final Plat or Replat
a. Preparation of a Final Plat, per the City of Ft. Worth requirements.
b. Process and address comment provided by the City to obtain approval at the preliminary and
final stages.
c. Unless otherwise prohibited by law, where no monument exists, set permanent iron pin
(monument) with cap stamped JQI or other suitable permanent monument at property
corners; drive pin adequately into ground to prevent movement and mark with wood stake;
state on the drawing(s) whether corners were found or set and describe each.
d. Provide any proposed easements per design on the plat.
8. Subsurface Utility Engineering: As part of Records Research effort, we will perform the following:

Qualitv Level "D" SUE and Qualitv Level "C" SUE
a. Contact the applicable "one call" agency and acquire records from all available utility owners
including local municipalities (cities, counties, etc.), and client.
b. Perform in -field visual site inspection from information derived from existing records or oral
recollections. Compare utility infrastructure and visual discrepancies with record drawings.
c. Interview available utility owners for needed clarification, resolution of found discrepancies,
and details not provided on the record drawings.
d. Information obtained by surveying and plotting visible above -ground utility features and by
using professional judgments in correlating this information
Qualitv Level "B" SUE (3'd Party Contractor)
a. Select and employ the appropriate suite of geophysical equipment to search for existing
utilities at the locations specified on the project. For metallic/conductive utilities (e.g. steel
pipe, electrical cable, telephone cable) electromagnetic induction, and magnetic equipment
will be employed. We will attempt to designate non-metallic/non-conductive utilities using
other proven methods, such as rodding, probing, and Ground Penetrating Radar (GPR).
b. Interpret the surface geophysics and mark the indications of utilities with paint on the ground surface
for subsequent depiction on deliverable utility maps.
c. Coordinate with the Client for survey the existing utility designating marks and above ground
appurtenances once all Field work has been completed.
SERVICES EXCLUDED FROM BASIC SERVICES
1. Any scope of work not specifically stated above.
2. Land Title Survey or ALTA survey.
3. Utility research. It is our understanding that this information will be provided to JQI by others.
4. Easement documents, Exhibit A and B offsite.
5. Tree identification survey of the site.
6. Geotechnical investigations and material testing.
7. Abstracting property for any existing Easements and or Rights of Way. If required an Abstractor
will be hired at additional cost.
8. Payment of any and all platting and plan review fees imposed by the City of Ft. Worth, Texas.
9. As -built survey for the site upon completion of construction.
10. Subsurface Utility Engineering (SUE) Level A. Vacuum excavation to verify the horizontal and
vertical location of the existing utilities.

Validity of Proposal: The proposal shall remain open and valid for a period of at least 90 days from the
designated date indicated above. Once the proposal is accepted during this period, the price quoted will
remain unchanged for the entire period of the resulting contract unless otherwise specified.

PAYMENT SCHEDULE
Invoices for progress payments of the basic fee will be billed each month for services performed during
the prior month.
