WHEREAS, the City of Fort Worth and Meals on Wheels of Tarrant County Inc.
made and entered into City Secretary Contract No. 63179
WHEREAS, the City receives grant monies from the United States Department of
Housing and Urban Developm

20-MW-0010, Catalog of Federal Domestic Assistance No. 14.218;
WHEREAS, in response to the public health and economic impacts of the Coronavirus
-
resources and to mitigate the effects of social isolation, particularly among vulnerable populations
disproportionately affected by the pandemic.
WHEREAS, the Agency has identified individuals aged 62 and older as being
disproportionately affected by the COVID-19 pandemic, with ongoing demand for transportation
assistance among low- and moderate-income Fort Worth residents in this age group, even after
the height of the public health emergency;
WHEREAS, Agency seeks to utilize CDBG-CV funds to provide transportation services
to address challenges that emerged or were exacerbated due to the pandemic, including barriers to
accessing essential resources such as healthcare, food, and community-based services;
WHEREAS, the Agency requested an amendment from City in order to (1) revise the
Program Summary found in
of the Agreement to correct the HUD Matrix Code classification and add service
locations, and (3) incorporate the 2025 HUD Income Limits under Exhibit A-1;
WHEREAS, it is the mutual desire of City and Agency to amend the Contract to complete
the Program and meet Contract objectives.
NOW, THEREFORE, City and Agency hereby agree to amend the Contract as follows:
I.
The definition for National Objective found under Section 2 of the Agreement is deleted
in its entirety and replaced by the following:
National Objective means an activity that benefits low- and moderate-income persons.
For the purposes of this Contract, at least 100% of the clients served by the Program fall into a
"presumed benefit" category as defined in 24 CFR Part 570 and therefore are presumed to be
Income Eligible Clients.
II.
- PROGRAM SUMMARY, attached to the Contract, is hereby deleted
and replaced in its entirety with 6/25, attached hereto.
- 2024 HUD INCOME LIMITS, attached to the Contract, is hereby
deleted and replaced in its entirety with - 2025 HUD INCOME LIMITS,
III.
All terms and conditions of the Contract not amended herein remain unaffected and in full
force and effect, are binding on the Parties and are hereby ratified by the Parties. Capitalized terms
not defined herein shall have the meanings assigned to them in the Contract.

PROGRAM:
This program will provide transportation services for older adults to and from congregate meal
sites and related social activities hosted by Adult Activity Centers (AACs). Eligible
participants are Fort Worth residents aged 62 and older who are food insecure and lack reliable
access to nutritious meals. These individuals were among the most adversely affected by the
COVID-19 pandemic, which exacerbated both nutritional challenges and social isolation due
to prolonged periods of physical distancing and service disruptions.
The program is designed to support the continued recovery of this vulnerable population by
addressing their basic nutritional needs while fostering opportunities for meaningful social
interaction. By re-establishing access to communal meals and peer engagement, the program
aims to reduce persistent feelings of isolation and improve overall well-being. Transportation
participants must provide verifiable identification confirming residency within the City of Fort
Worth.
Eligibility documentation is required at client intake, and this is when required signatures are
obtained. AAC Directors, center aides, Park and Recreation Program Coordinators, trained
volunteers, or other staff may facilitate the intake process. Residency is verified through
Tarrant Appraisal District (TAD) by the AAC Program Manager and agency staff. CDBG
Funds will be used to pay for the cost of one-way trips provided to elderly Fort Worth residents.
Cost for the transit service shall be the same as the rate charged to the Agency by the vendor
or subcontractor.

PROGRAM GOALS:
Based on the nature of the service provided, agency will maintain documentation that verifies that
100% of clients served by the program are elderly, aged 62 and older per the Presumed Benefit
CDBG clientele categories and therefore are at or below 80% of Area Median Income (AMI) as
established or defined by the United States Department of Housing Urban Development (HUD).

Description of the Proposed Project [24 CFR 50.12 & 58.32; 40 CFR 1508.25]:
The grant will support Meals on Wheels (MOW) Congregate program, which serves nutritious meals and
provides transportation to older adults at 21 Adult Activity Centers (AAC) across Tarrant County, including 12
in Fort Worth. The program fosters social interaction through engaging activities and special field trips,
promoting health and reducing isolation. Transportation is provided for eligible clients, ensuring they can
access the centers. MOW's caseworkers assess client needs and eligibility, coordinating services based on
geographic convenience. The grant will help maintain a high quality of life for older adults by supporting meal
programs, education, and social engagement.

RECOMMENDATION:
It is recommended that City Council authorize execution of contracts related to existing appropriations in the Coronavirus, Aid, Relief, and
Economic Security Act Community Development Block Grant Funds in the amount of $250,000.00 to fund particular programs and activities with
the following agencies:
• The Presbyterian Night Shelter of Tarrant County, Inc. for Homelessness Prevention and Special Needs Support in the amount of
$142,905.00
• Guardianship Services, Inc. for Elderly Services/Aging In Place in the amount of $70,000.00
• Meals on Wheels of Tarrant County for Elderly Services/Aging In Place in the amount of $37,095.00
DISCUSSION:
The purpose of this Mayor and Council Communication (M&C) is to request approval of awards of federal grant funds, as approved by M&C 24-
0743, August 27, 2024, amending the City of Fort Worth's 2019-2020 Annual Action Plan, in regards to the use of funding received from the U.S.
Department of Housing and Urban Development (HUD) under the Coronavirus, Aid, Relief, and Economic Security (CARES) Act Community
Development Block Grant (CDBG-CV/CV3) program.
On August 6, 2019, the City Council, through M&C 19-0016, approved the City's 2019-2020 Annual Action Plan summarizing the major housing
and community development activities and proposed expenditures for use of federal grant funds from HUD under various grant programs including
the Community Development Block Grant (CDBG) program.
The CARES Act was signed into law on March 27, 2020, to respond to the impacts of the COVID-19 public health emergency. The CARES Act
included provisions to help local governments prevent, prepare for, and respond to the coronavirus, including providing for additional allocations of
formula grant funds under the CDBG program. The City Council previously awarded the additional allocations of CDBG-CV and CDBG-CV3
funding through M&Cs 20-0330, 20-0471, 20-0940, and 21-0016. Some of the funding received was allocated to cover administration expenses.
On August 27, 2024, the City Council, with M&C 24-0743, approved the staff's recommendation to again amend the 2019-2020 Annual Action
Plan and re-allocate $250,000.00 from administration to Public Service Agencies (PSAs) through a competitive Request for Proposal (RFP)
process to address the following priorities:
• Elderly Services/Aging In Place
• Homelessness Prevention and Special Needs Support

The 2019-2020 Annual Action Plan was amended by the City Council on May 19, 2020, to state how CARES Act grant funds were to be used, and
to amend the Citizen Participation Plan to reduce public comment period for CARES Act funds from thirty days to five days as authorized by the
CARES Act (M&C 20-0330). This 5-day public comment period began Monday, December 23, 2024, and ran through Friday, December 27,
2024. Notice of this public comment period was published in the Fort Worth Star-Telegram, on Sunday, December 22, 2024. Any comments
received are maintained by the Neighborhood Services Department in accordance with federal regulations.
These programs are available in ALL COUNCIL DISTRICTS.
FISCAL INFORMATION / CERTIFICATION:
The Director of Finance certifies that upon approval of the above recommendations, funds will be available in the current operating budget, as
appropriated, of the Grants Operating Federal Fund. The Neighborhood Services Department (and Financial Management Services) will be
responsible for the collection and deposit of funds due to the City. Prior to an expenditure being incurred, the Neighborhood Services Department
has the responsibility to validate the availability of funds. This is a reimbursement grant.
