Section S Any physician or other parson who makes �H a diagnosis in or treato a case of s yphili , gonorxhaa or c hen-; cruid, and aver; superintendent car managrer of a hcsj"'itml dip ; awry, or charitable or penal institution, in which there is a+ ass of venneraa,l diseases shall report suab. case immediately in writing, to the City physician, stating the name and address or the office number, age, sex, color., and Occupation of the diseased I Person, and the date of the onset of the disease, and the probable source of infootiono provided that the narrie and address of the diseased person need not be stated, ex t as h1dil/0"i"'I"It", #If", oally required in Section 6, and provided,, further, that all infor- mation and reports concerning persons having venereal disease shall be held secret in accordance with provisions in Section 11. The report shall be enolosed in a sealed envelope and sent to the city physician., who shall report weekly on the prescribed form to the State Board of Health, all cases reported to him. Section 3. It shall be the duty of every physician and of every other person who examines or treats a person having syphilis, gonorrhea or chanoroid,, to instruct him in measures 11 for preventing the spread of such disease,, and of the necessity for treatment until cured, and to hand him a oopy of the oircular of information obtainable for thit 640 ftbx the State Board of Health. Section 4. All city physicians shall use every avtilable means to ascertain the existence of, and to investigate all oases of syphilis., gonorrhea and chancroid within the city of Fort Worth* and to ascertain the sources of such infections. The city phy- sician is hereby empowered and directed to make such examinations of persons reasonably suspected of having syphilis., gonorrhea or obLanaroid, as may be necessary for carrying out the provisions of this Ordinance. Owing to the prevalence of such diseasse among prostitutes and persons associated with them,, all such persons are to be oonaidered within the above class* section 5. Upon the redeipt of a report of a, case of venereal disease, it *hall be the duty of the city physician to institute measures for protectior, of other persons front infection by such venereally diseased person. The city physioia'n is authorized and directed to qt=antine persons who have, or or$ reasonably suspected of Ilk having syphilis, gonorrhea or chancroid, whenever, in the opinion Of Said city Physician, quarantine is necessary for the protection of the public health. ') In establishing quarantine the ,,,city phy- SiOian Shall designate and define the limit$ of the area in which the person known to have,, or reason-ably 04S,00'ted Of vi syphilis, gonorrhea or 0hanoroid and his immediate attejjdxLjjt,, are to be quaxantined and no person, other than the attending physician Shall enter or leave the area of quarantine without the permis- Sion Of the city Physician. No one but the city Physician shall terminate said qiarantine, and this shall not be done until the quarantined person has become non-infectious, as determined by the city physician, or his authorized deputy through clinical examination and all necessary laboratory tests. Any person not cured, before release from quarantine, shall be required to sign the following statement after the blank spaces have been filled to the satisfaction of the health Officer. All such agreements shall be filed with the city physician, and kept inaccessible to the public as provided in Section 11. Section 6 (a) When a person applies to a physician or other person for the diagnosis or treatment of syphilis, gonorrhea, or o"horoid, it sh 1 be J `e' t` ` cS" tl'e p ith,,;, cr, person so consulted to inquire of, and ascertain, from the person seeking such diagnosis or trea,tzent, whether such ;person has heretofore consulted with, or, has been treated by any other hyslaian or person, and if so, to ascertain the name and address of the physician or Person last consulted. It shall ,be the duty of the applicant for diagnosis or treatment to Turnish this information, and a refusal to do so, or a falsification of the name and address of such physician or person consulted by such applicant shall be deemed a violation of this Ordinance. It shall be the duty of the physician or other person whom the applioa»nt consults to notify the physician far other p°arson ,, �s consulted; of the change of advisets. Should the physician or person previously consulted fail to receive such notice Within ten days after the °last date upon: which the patient was instruc- ted by him to appear, it shall be the duty of such physician or person to report to the city physician the n is and address of -4- venear0ally diseased person. Section 7. Any druggist or other person who sells any drug, oompound, specific or preparation of any kind used for or believed by the druggist or person to be intended to be used for the treatment of any of said venereal diseases, shall keep a record of the name and address of the person raking such purchase. A copy of said record shall be mailed each week to the city Physician and by him to the to to Board of Health. Section S. Any employ inspector, physicir ,a, nurse, s perinte dent of alin, c Or, hospital, d"Tu gist br o � rim who fails to perform the duties required of him in this ordMi a uc+e, or violates any of the provisions of this ordinaries, shall be deemed guilty of a misdemeanor and upon conviction therefor shall be fined in any sure not less than five nor more than fifty dollars, and each violation shall be a separate offense. +ectien. 1 . In addition to the remedies provided herein for the enforcement of the provisions of this ordinance, the city physician is hereby authorized and empowered to employ all measures provided by existing laws for ascertaining, handling, segregating azad controlling contagious or infectious dieeases. Section 14.
