JUSTIFICATION (REASONS) FOR CONSTRUCTION ALLOWANCE
On March 12, 2025, FWCC was asked to place message boards on Kansas Street and S. Commercial to inform the public about daily road closures. These were set up on March
14, 2025. On April 8, 2025, a 4'x4' manhole along with 18" and 24" laterals were found at the intersection of Kansas Street and Commercial that were not referenced in the initial
plan set. FWCC was told to remove both the manhole and the pipe after reviewing the existing record drawings. On April 10, 2025, a 5'x5' manhole was found on Viola Street. It
was only connected to a pipe already marked for removal, and FWCC was told to remove the manhole as well. We are also adding 36" and 21" plugs to existing lines to ensure
debris will not collect in our proposed 21" and 36" lines as discussed in a site meeting on 5/2/2025. The item for the Message Board was not an existing item, but we requested an
invoice from the supplier to compare to and were able to verify the price. The items for removing 18" Storm Drain and 5' wide asphalt pavement repair were existing items whose
unit prices could be verified by comparing them to the bid amounts. The item for removing 24" Storm Drain was not an existing item, but was compared to the item for 36"storm
drain removal as it was similar and its cost was deemed to be reasonable. The items for removing the 4'x4' and 5'x5' Storm Drain Manhole were not existing items, but were
compared to removing S concrete inlets as they are similar in quantities of concrete removal, and the prices are shown to be reasonable. The prices for the 36" and 21" SD plugs
were compared to similar projects that had these bid items and were deemed as reasonable.
It is understood and agreed that the acceptance of this Construction Allowance Request by the contractor constitutes an accord and satisfaction
and represents payment in full (both time and money) for all costs arising out of, or incidental to, the above Construction Allowance Request.
The Project/Program Manager signing below acknowledges that he/she is the person responsible for the monitoring and administration of this
contract, including ensuring all performance and reporting requirements.

Mr. Aransen,
During the installation of the open cut storm drain on Kansas Street on April 8, 2025, a 4' x 4' storm drain manhole was uncovered that was
buried under the existing pavement. After some investigation, Fort Worth Civil Constructors (FWCC) identified an existing lateral coming
from S. Commercial Street that was tied into the storm drain manhole. FWCC requested direction on how to proceed and the response was
to remove the existing storm drain manhole and laterals. These were not shown on the plans as removal items.
During the installation of the open cut storm drain on Viola Street on April 10, 2025, a 5' x 5' storm drain manhole was uncovered that was
buried under the existing pavement. After some investigation, FWCC determined the 36" RCP shown to be removed on the plans was the
only pipe connected to this manhole. FWCC requested direction on how to proceed and the response was to remove the existing storm
drain manhole. This was not shown as a removal item on the plans.
On March 12, 2025, a request was made to put two message boards on Kansas Street and S. Commercial to alert the surrounding
community of the upcoming daily road closure on Kansas Street for the storm drain installation. On Friday March 141h these message
boards were put in place.
Please see the associated cost for these additions to FWCC's scope of work. Additionally, we respectfully request 3 days be added to our
contract time.

Re: Viola Storm Drain - COR # 3 — RCP Plugs
Mr. Aransen,
Plan sheet 18 shows the installation of the 36" storm drain line to end at Sta 7+15. The storm drain does not connect to an existing storm
drain or terminate at a structure. The RCP will need to be plugged to ensure material and debris do not collect inside the storm drain
system we are installing. Also, plan sheet 18 shows the removal of 255 linear feet of 36" storm drain beyond the limits of the proposed
storm drain installation. Once this storm drain has been removed, a plug will need to be installed on the existing storm drain to prevent
material and debris from collecting inside the existing storm drainage system.
During the site meeting held on May 9, 2025, the conflict with the existing gate valve vault and Lateral A-5 was discussed. The direction
received was to plug the 36" x 21" Wye at Sta 6+94 and move the lateral to the west to avoid the conflict with the valve vault.
Please see the associated cost for these additions to FWCC's scope of work. Additionally, we respectfully request 2 days be added to our
contract time.
