FIRST AMENDMENT TO
CITY SECRETARY CONTRACT NO.63178
This contract ("Contract") is made and entered into by and between the City of Fort Worth
(hereafter "City") and The Presbyterian Night Shelter of Tarrant County, Inc. (hereafter "Agency"),
a Texas non-profit corporation. City and Agency may be referred to individually as a "Party" and
jointly as "the Parties".
WHEREAS, City receives grant monies from the United States Department of Housing
and Urban Development through the Community Development Block Grant ("CDBG") Program,
Program No. B-20-MW-48-0010, Catalog of Federal Domestic Assistance No. 14.218;
WHEREAS, City has received an additional allocation of CDBG grant monies under the
Coronavirus Aid, Relief, and Economic Security Act (CARES" Act), referenced by HUD as
"CDBG-CV3";
WHEREAS, the CDBG regulations and related HUD guidance publications establish all
requirements for the CDBG-CV3 funds except for those specifically described in this Contract as
deriving from the CARES Act;
WHEREAS, a national objective of the CDBG program is to benefit low and moderate
income citizens in accordance with 24 CFR Part 570, as amended from time to time;
WHEREAS, Agency submitted a proposal and was selected to use CDBG-CV3 funds to
provide eligible public services to individuals and families, of whom 100% must meet the HUD
definition of a presumed benefit population;"
WHEREAS, the Parties entered an Agreement on January 1, 2025 that inadvertently
makes references to the CDBG/CV and CDBG/CV2 program and the Parties wish to make related
corrections to the Agreement.
NOW, THEREFORE, the Parties understand and agree as follows:
I.
The definition for National Objective in Section 2. Definitions is deleted in its entirety
and replaced with the following:
National Objective means an activity that benefits low- and moderate -income persons.
For the purposes of this Contract, at least 100% of the clients served by the Program fall into a
"presumed benefit" category as defined in 24 CFR Part 570 and therefore are presumed to be
Income Eligible Clients.

All references to "CDBG/CV" and/or "CDBG/CV2" throughout the Agreement are
hereby deleted and replaced with "CDBG/CV3."
(1) EXHIBIT "A"- PROGRAM SUMMARY, attached to the Contract, is hereby deleted
and replaced in its entirety with EXHIBIT "A," Revised 07/25, attached hereto.
(2) EXHIBIT "A-1" — 2022 HUD INCOME LIMITS, attached to the Contract, is hereby
deleted and replaced in its entirety with EXHIBIT "A-1" — 2025 HUD INCOME LIMITS,
attached hereto.
IV.
All terms and conditions of the Contract not amended herein remain unaffected and in full
force and effect, are binding on the Parties and are hereby ratified by the Parties. Capitalized terms
not defined herein shall have the meanings assigned to them in the Contract.

PROGRAM SUMMARY
PRESBYTERIAN NIGHT SHELTER — JOURNEY HOME
January 1, 2025 to September 30, 2025 $142,905

Capitalized terms not defined herein shall have meanings assigned them in the Contract.
PROGRAM:
The Program provides homeless single men and women with case management services while in
the shelter and includes a 60-day follow-up once clients are housed. In recognition of the increased
housing instability and service gaps amplified by the COVID-19 pandemic, the Program
emphasizes continuity of care to support long-term housing success. Clients meet with case
managers bi-weekly to develop and implement individualized service plans aimed at reducing
barriers to permanent housing —many of which were intensified by the health and economic
impacts of the pandemic. Follow-up support is conducted through phone calls at 30 and 60 days
after program exit to ensure sustained stability and to address any emerging needs as clients
transition into permanent housing during the ongoing recovery period.
CDBG-CV3 funds will be used to pay Program Staff salaries and benefits for Journey Home
Guides (case managers) and services will be available at 2400 Cypress Street (men's shelter) and
2401 Cypress Street (women's shelter) Fort Worth, TX 76102, Monday through Friday, 8:00 AM
to 8:00 PM.
The program is designed to support the continued recovery of this vulnerable population by
addressing their basic nutritional needs while fostering opportunities for meaningful social
interaction. By re-establishing access to communal meals and peer engagement, the program aims
to reduce persistent feelings of isolation and improve overall well-being. Transportation services
are available to individuals enrolled in one of the agency's nutrition plans, and all participants must
provide verifiable identification confirming residency within the City of Fort Worth.
