In accordance with the Professional Services Agreement signed between Sentari
Technologies, Inc. ("Contractor') and the City of F=ort Worth ("Client" or "City") effective
as of November 6, 2013 and approved by M C C 26551 on November 5, 2013 (the
"Agreement"), this Appendix B-14C is incorporated into the Agreement and hereby
reads as follows:
1. The term of this work order shali be exterided to end when all services have
been completed under this work order as contemplated herein, but not later
than the end date set forth below.

Description of Work:
The Storm Water Managenrrnerit t)I i,siori (TWit1D) vrnthir°n tl"ie Transportation and Public
Works Department (TPW) within thie City of Fort Worth, Texas, has acquired a new,
Commercial-Off-The-Shelf (COTS) Work Order and Asset Management System
(WOAMS). The system will initially sI.,ippory the needs of the SWMD, though its
utilization may in the future: be expanded to other sections of TPW or to other City
departments. The desired WOAMS personnel nrnel *ra H:
1. Automate work, managercn a n' :r ac,txe s withM S000[)00[) fl°at maintain and repair
storm water infrastructrnre for the City.
2. Support asset management practices to optimize maintenance, repair, and
replacement of storm water infrastructure.
3. Integrate with cr.nrrent and arrticipatrer.a City systems, irnciva.fing the City's CRM,
project managern°,enr f f°aa l�;ral;; and GI ) syst:errns.
4. Convert/migrate certain related dwa fry:rn t xns in f nega cy systems to the new
WOAMS
5. Provide capabilities for inserting artditionari asset types, work activities, and/or
business processes into the initiate innpiernentaJcri, if desired.
