Home My WebLink AboutContract 58578-A4R3Fourth Amendment and Third Renewal to CSC No. 58578 Page 1 of 2 FOURTH AMENDMENT AND THIRD RENEWAL TO CITY OF FORT WORTH CONTRACT NO. 58578 This FOURTH AMENDMENT AND THIRD RENEWAL is made and entered into by and between the CITY OF FORT WORTH, a home-rule municipal corporation of the State of Texas, acting by and through its duly authorized representative , and Fulcrum Management Solutions, Inc. Vendor WHEREAS, City and Vendor entered into an agreement identified as Fort Worth City Secretary 2023; WHEREAS, the parties previously renewed the agreement for successive terms commencing October 1, 2023 through September 30, 2024 and thereafter commencing October 1, 2024 through September 30, 2025; WHEREAS, the parties desire to amend the Agreement to update Exhibit A of the Agreement and increase the maximum spend to $99,225.00 for the third renewal term; WHEREAS, the Agreement provides that the term may be renewed for one additional one-year periods; WHEREAS, the parties wish to continue their relationship by exercising the third renewal option beginning October 1, 2025 and to expire September 30, 2026; and NOW, THEREFORE, City and Vendor agree to amend and to renew and extend the Agreement as follows: I. AMENDMENT City and Vendor hereby agree to amend the Agreement as follows: Exhibit B of the Agreement is deleted and replaced with the Exhibit A attached to this Fourth Amendment and Third Renewal. In accordance with Exhibit A, the maximum spend under the Agreement is increased to $99,225.00. II. RENEWAL The parties further agree to renew and extend the Agreement for its third renewal term, effective beginning on October 1, 2025 and ending on September 30, 2026 (3nd Renewal Term). This shall be the third of three possible one-year renewal periods. III. (1) All terms and conditions of the Agreement that are not expressly amended pursuant to this First Amendment and First Renewal shall remain in full force and effect.
